Sep 23, 2025Facility evaluation reportReport on file

Type of visit: Required - 1 Year

On 09/23/2025, Licensing Program Analyst (LPA) M Vega arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility. Administrator (AD) David Murchison was notified of Licensing visit and provided assistance with the visit. LPA toured facility with Administrator inside and out. The facility was observed to be at a comfortable temperature, of 73 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. Fire extinguisher was observed with a service date of 02/2025. All 6 residents’ bedrooms were observed to be with comfortable temperature. Bathroom water temperature was tested and recorded reading of 110.8 degrees F. Medications observed to be locked in a cabinet in the kitchen. LPA reviewed MAR, it was administered correctly at the time of inspection. Cleaning supplies were observed to be in a locked cabinet in the laundry room. An outdoor shaded seating area was observed for residents in care. LPA reviewed Staff and Resident files. Resident files observed to have updated information. Continuation on LIC 809C LPA is requesting the following documents be submitted for annual updating to the Fresno CCL office by 10/07/25: Designation of Facility Responsibility (LIC 308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Proof of Liability Insurance, Emergency and Disaster Plan (LIC 610 E) Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020A), Surety Bond. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with facility staff Camile Agarin Continual Administrator's Certification expires 05/06/2025. There are currently 5 residents who reside at this home and there is 1 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector. Water temperature was tested at 107 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. Resident 1 does not have a Physician's Report updated annually as required. The facility staff files are not available for Licensing to review. Resident 1's medication was not logged correctly on the Centrally Stored Medication record. The facility does have an Emergency Disaster plan, but it is on the older version the form. The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file.
