On today's date, Licensing Program Analyst (LPA) William Vanegas made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by caregiver staff, LPA explained the purpose for the visit. Administrator (AD) Cristina Evangelista was notified via telephone and could not arrive to assist due to being at a doctor's appointment. LPA observed that Cristina Evangelista has a valid Administrator certificate which expires on April 27, 2026. LPA Vanegas began a tour of the facility and observed the following. The facility is a single-story house and consists of the following. Seven bedrooms four of which are private resident bedrooms, two of which are staff bedrooms, three shared resident bathrooms, a living room, a dining room, a kitchen, a courtyard, and an attached two car garage. LPA Vanegas observed two residents in care, lounging in the living room area. LPA Vanegas observed three caregiver staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on a wall in the dining room measured 20 X 26 inches. LPA Vanegas inspected the four resident bedrooms, and they were observed to be free of any hazards. LPA Vanegas observed the resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, a reading lamp, and enough storage space for personal belongings. All resident beds had clean linens in good repair meaning no strains or tares. LPA observed additional linens are stored in a hallway closet. LPA inspected the three shared resident bathrooms, and observed resident bathrooms to be clean and free of any mildew and debris. Bathrooms are equipped with grab bars and slip resistant floor mats. Faucets and toilets were operational, and hot water temperature measured between 107.9 and 114.8 degrees Fahrenheit. LPA observed the two staff bedrooms are kept locked and inaccessible to residents in care. CONTINUED ON LIC809-C LPA Vanegas observed kitchen area to be clean, and free of any mildew or debris. LPA Vanegas observed a dishwasher, microwave, gas stove, and refrigerator. Refrigerator was observed to be at the appropriate temperature, and all appliances tested operational. LPA Vanegas observe a two day supply of perishable food and a seven day supply of non-perishable food, as well as a sufficient amount of emergency water and emergency food in the case of a natural disaster. LPA Vanegas observed the outside of the facility to be free of debris and hazards a long exit routes. LPA Vanegas observed side doors to be self latching and unlocked. LPA Vanegas observed an outdoor shaded sitting area, and for the backyard to be large enough to participate in outdoor activities upon resident request. LPA Vanegas observed all smoke detectors and carbon monoxide detectors to be operational. LPA Vanegas observed all fire extinguishers to be fully charged and up to date. LPA Vanegas reviewed P&I with DSP Milani Villasis. All balances were accurate and documented accordingly. LPA Vanegas reviewed three staff files, and four resident files. All files (Resident and Staff) had all required documents and all annual staff training was complete. LPA Vanegas reviewed medication administration log and medications for all four clients. No discrepancies were observed. Based on observations made during today's inspection no deficiencies will be issued per title 22 division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by caregiver staff after explaining the purpose for the visit. Administrator (AD) Cristina Evangelista was notified via telephone and could not arrive to assist due to being at a doctor's appointment. LPA observed that Cristina Evangelista has a valid Administrator certificate which expires on May 24, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents with a hospice waiver for two. The facility is a single-story home with four private resident bedrooms, two staff bedrooms, three shared resident bathrooms, a living room, a dining room, a kitchen, a courtyard, and an attached two car garage. LPA accompanied by caregiver staff conducted a tour of the physical plant. On today's visit, LPA observed four residents in care, one of which is received hospice care, and three caregiver staff present. LPA observed residents eating their breakfast in the dining room as well as eating in their respective bedrooms. LPA observed the See Something, Say Something poster (PUB 475) mounted on a wall in the dining room measured 11 by 17 inches measured. LPA inspected the four resident bedrooms, and they were observed to be free of any hazards. LPA observed the resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, and a lamp. All resident beds had clean linens and blankets. LPA observed additional linens are stored in a hallway closet. LPA inspected the three shared resident bathrooms. Resident bathrooms are clean. Bathrooms are equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 109.5 and 118.4 degrees Fahrenheit. LPA observed the two staff bedrooms are kept locked and inaccessible to residents. LPA observed the kitchen has a two-day perishable and seven-day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. The four burner gas stove lights unassisted. CONTINUED ON LIC809-C LPA observed kitchen knives are stored in a locked kitchen cabinet. LPA observed cleaning supplies are stored in a locked kitchen cabinet under the sink.Fire extinguishers are located in the dining room and garage. Fire extinguishers were observed to be charged and purchased on January 10, 2025. LPA tested the wired smoke detector/carbon monoxide detector which tested operational. LPA observed the facility conducted their last emergency disaster drill on January 4, 2025. The centrally stored medication is kept in a locked storage cabinet in the kitchen. LPA observed the First Aid kit has all the required components. The door leading to the attached two car garage is kept locked and inaccessible to resident. The garage is used for storage and laundry. LPA observed chemicals and toxins stored in the garage. LPA observed the facility has an emergency food and water supply stored in the garage. LPA and caregiver staff conducted a tour of the exterior portion of the facility. LPA observed the exterior portion to be clear of obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. The perimeter gate on the southside of the facility is self-latching and can be opened in an evacuation. There is a fountain located in the courtyard. The fountain is not in operation and is adequately fenced for resident safety. LPA reviewed the four resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed four residents’ medication and medication records. LPA reviewed the Personal and Incidental expense monies (P&I) records for four out of four residents. LPA reviewed six staff files. All staff are background cleared and associated to the facility. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. AD advised LPA via telephone that a caregiver staff had permission to sign the report. Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by Jose Villasis, caregiver and LPA explained the nature of the visit. Four residents reside at this facility. LPA accompanied with caregiver began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage cabinet located in kitchen. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the bathroom and LPA measured the hot water temperature which measured 110.6 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored locked underneath kitchen sink. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which has sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided for residents in their bedroom. Smoke detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment. LPA observed a fire extinguisher with service date of July 18, 2023, in dining room. LPA spoke with alert residents during regarding the quality of their care LPA began review of records. LPA reviewed four resident records. All the required documentation was present and current in the Continued on LIC809-C residents’ files reviewed. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.
