Licensing Program Analyst (LPA) Christian Gutierrez conducted an annual continuation visit. Upon arriving at the facility, LPA met with Caregiver Kevin Brewer who assisted with the visit. On 10/09/2025 LPA conducted an initial visit. During today's visit LPA Gutierrez reviewed the following CARE inspection tool domains: Resident rights/information, resident records/incident reports, and residents with special health needs. Four (4) resident files were reviewed and included physician’s reports and appraisal needs and service plan. LPA reviewed medications for residents, remaining two residents and they are being administered as prescribed by the physician. No deficiency was observed during today’s visit. Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Caregiver Kevin Brewer and explained reason for visit. Administrator Louise Brewer was notified by telephone. Facility is a Residential Care Facilities for the Elderly (RCFE) and is licensed to serve adults ages 60 and over. Six (6) total bed capacity (including 2 non-ambulatory) bedrooms # 2 and 4 approved for one (1) ambulatory each. Waiver granted for four (4) hospice care. Facility is a single-story home located in a residential area consisting of four (4) bedrooms, two (2) bathrooms, kitchen, dining room, living room, family room, attached garage, and backyard with seating and shade. LPA toured the facility and observed the following: Each resident bedroom has the required furniture and bedding. The Smoke detectors were observed throughout the facility and are properly operating. There is one carbon monoxide detector in hallway. The facility has one (1) fully charged fire extinguisher which is kept in family room. Cleaning supplies and toxic substances were observed to be accessible to residents from unlocked kitchen door leading to attached garage cleaning supplies on floor. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 40 degrees F. LPA observed medication in refrigerator to be unlocked in medication box. Facility was observed to have sufficient supply of 2 days perishable & 7 days non-perishable foods. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The resident bathrooms have the required grabs bars and non-skid mats. During tour of facility LPA observed medication closet in hallway to be unlocked. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents in back yard. Passageways and exits are free of obstruction. LPA reviewed residential infection control plan 9282, and emergency disaster plan 610E at time of visit. Last drill conducted was on May 10th, 2025. LPA reviewed two (2) resident medications and both had missing medications. ***Due to time constraints, LPA was not able to complete the annual inspection for this facility. LPA will do a continuation of this inspection. ***. Deficiencies have been noted on LIC 809D under Title 22 Regulations. Type of visit: Post Licensing

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced post-licensing inspection visit on 2/18/2025 and was greeted by Administrator Lea Loaiza. Administrator Louise Brewer arrived shortly after. LPA Ramirez explained the purpose of the visit. The facility is located on a residential street and is a single store dwelling. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Physical Plant and Environment safety: Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to residents, were observed to be inaccessible to residents. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected four (4) resident rooms. All resident bedrooms contained required furniture, linens and lighting. Bedroom#2 is currently unoccupied. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees F. LPA Ramirez observed grab bars near toilets and inside shower. LPA Ramirez observed no-slip mat in showers. Shower was observed to be wheelchair accessible. Food Service: LPA Ramirez observed sufficient supply of nonperishable for one week and perishable foods for a minimum of two days in the facility kitchen area. Soaps, detergents, and cleaning compounds were observed to be stored away from food supplies. Freezers and refrigerators were observed to be clean and within temperatures of 0-degree F (-17.7 degree C), and refrigerators with maximum temperature of 40-degree F. (4 degree C). Planned Activities: LPA Ramirez observed board games, magazines, and other activities for residents. Residents Rights-Information: LPA Ramirez observed the following postings in common areas throughout the facility: Complaint Poster (PUB 475), personal rights, and nondiscrimination notice. LPA Ramirez observed facility land line. LPA Ramirez reviewed three (3) residents medications. Disaster Preparedness: The facility has the Emergency Disaster Plan (LIC610D/9 pages) in place. Last documented emergency drills were conducted on 11/24/2024. LPA Ramirez observed facility sketches with exits and emergency exits routes throughout various locations of the facility. LPA Ramirez observed emergency food supply located in pantry. See 809-C Residents with Special Needs: No large bodies of water were observed LPA Ramirez observed signs posted indicating “No smoking - Oxygen in Use” in various locations of the facility. LPA Ramirez observed several oxygen tanks in resident rooms secured in stands. Knives, sharps or other items that could pose a danger to residents with dementia, were observed to be inaccessible. Auditory devices were observed to be in working order. Health Related Services/Incidental Medical Services: The medications are centrally stored in the medication closet and in bubble packs and/or original containers. LPA Ramirez observed Centrally Stored Medication and Destruction Record. The facility provides incidental medical services. Staffing: Administrator Certificate for Louise Brewer 06/22/2026. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility. Personnel Records Training: Staff files are maintained at the facility. LPA Ramirez observed required annual training, CPR and First Aid for one (1) out of the one (1) personnel record reviewed. LPA Ramirez observed TB testing results, Health screening, fingerprint clearance and job application for one (1) out of the one (1) personnel record reviewed. Infection Control: There are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place. Operational Requirements: The fire clearance is approved for six (6) residents over the age of 59 years old. Bedrooms #2 and #4 may retain one (1) non-ambulatory in each room. This facility may retain no more than two (2) hospice residents. There were two (2) residents during hospice during time of inspection. Resident Records/Incident Reports: LPA reviewed resident records for three (3) residents in care. Resident records are maintained at the facility. Admission Agreement, Physician's Report (including T.B and Ambulatory Status), Consent for Medical Treatment, Preplacement Appraisal Information, Resident Pre-Appraisal, Care Plan/Appraisal/Needs and Services Plan, Resident Rights were observed. Licensing Program Analyst (LPA) Kimberly Ramirez made an announced visit and met with Licensee/Administrator Louise Brewer to conduct a Pre-Licensing evaluation. An application was submitted to Community Care Licensing Department (CCLD) on for an initial application of a Residential Care Facilities for the Elderly (RCFE) to serve adults ages 60 and over. Bedrooms# 2 and 4 is approved for one (1) ambulatory and one (1) non-ambulatory. Structure: Facility is a single-story home located in a residential area consisting of four (4) bedrooms, two (2) bathroom, kitchen, dining room, living room, and attached garage, and backyard with seating and shade. Front entry has a wheelchair ramp. Bedroom Clients: Bedrooms are equipped with a bed, night stand, chair, lamp, and overhead lighting. Bathrooms: Two (2) full bath equipped with working toilet, wash basins, bathtub/shower. Linens & Hygiene Supplies: All beds had the required linen/supplies which include, pillowcase, mattress pads, fitted sheet, blanket and bedspreads. Adequate supply of linens is stored in linen closets. Emergency Phone Numbers, Exit Plan: Emergency numbers are posted and readily available for review. One (1) fully charged fire extinguisher was observed. Facility has a land line telephone. Food Service: Dishes, cups, and flatware are stored in the kitchen cupboards, inspected and in good repair. Knives, cutlery, and other sharp kitchen utensils were observed locked and inaccessible. Adequate food supply is stored in the kitchen and consists of the following: 2-day perishables, and 7-day non-perishables. Emergency water supply was observed. Smoke Detectors: Where observed and tested. Appliances: Refrigerator, oven, microwave, dishwasher and washer/dryer are in good condition. The residence is equipped with central heating and air conditioning. Toxins: Cleaning supplies, and toxins are locked only accessible to staff. Water Temperature: Hot water was tested in all bathrooms, and kitchen sink. Water temperature was within normal limits 105 degrees Fahrenheit (40.5 degrees C) and not more than 120 degrees Fahrenheit (48.8 degrees C). Medication, First-Aid Kit & Book: Designated centrally stored medications cabinet, and the first-aid kit has been inspected which has at least the following: tweezers, scissors, antiseptic, bandages, gauze, thermometer; including a current First Aid manual. Resident & Staff Files: Designated area for files will be in locked closet. Pools/Jacuzzi/Body of Water: No bodies of water were observed. Fire Clearance: Fire clearance was approved on 9/12/2024 for four (4) ambulatory and two (2) non-ambulatory only. Component III: Component III was reviewed during inspection. Pre-Licensing is complete, and this facility has no deficiencies. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information.
