Licensing Program Analyst (LPA) Imaculada Vasquez arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by Caregivers, Jose Itchon and Michelle Hashimoto and granted entry. Administrator, Claire Itchon Camua arrived five (5) minutes later. LPA began inspection with introduction and visit purpose. There are currently Five (5) residents living at the facility. The single story facility consists of three (3) bedrooms and two and a half (2.5) baths. Resident record review began- Two (2) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility meets documentation requirements. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 118.9 degrees F. Laundry chemicals, cleaning chemicals, and sharped objects are observed in a locked cabinet. Medications are locked in a cabinet in office. LPA verified there is a telephone working at this location (760) 322-8528. Food Service- Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables food on hand. Food prep areas are clean and organized. (Continued on next page) (Continued from Page 1) LPA began review of employee records- Two (2) records were reviewed. LPA reviewed employee records for first aid, fingerprint clearance, personnel/job application, health screening, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Administrator certification present and current, expires on 12/02/2029. LPA made observations throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. One (1) Fire extinguisher is tested or replaced annually and were last done so on 03/17/2026. The facility is conducting quarterly emergency disaster drills. The last disaster drill was conducted on 06/08/2026. Licensing Program Analyst (LPA) Seo Jeon conducted an unannounced annual required visit. Upon entry, LPA was greeted by Claire Camua, Licensee, and informed them of the purpose of the visit. At the time of the visit, there were zero (0) staff member and zero (0) resident present. LPA toured the facility and did not observe any resident or sign of anyone residing at this facility. Facility Overview: The facility is a one-story home with three (3) bedrooms and three (3) bathrooms, including an attached garage. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 105°F. Fire extinguisher located at kitchen has current inspection tag. Food Service: The facility’s kitchen was clean and equipped to prepare food. Care & Supervision/Administration: The administrator holds a current administrator’s certificate. Continued on LIC809-C..... Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced annual required visit. LPA was granted entry and met with Administrator Claire Camua, who was informed of the purpose of the visit. At the time of the visit there was three (3) staff and four (4) residents present. LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted interviews. LPA observed the following: Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. The outdoor area was observed to be free of hazards with outdoor furniture and shaded area for clients. Detergents, cleaning solutions, and sharp and dangerous objects were observed to be locked and inaccessible. The smoke detector and carbon monoxide was operational, and the hot water temperature met department requirements. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. LPA reviewed two (2) staff files and training. All staff have the required personnel records on file and criminal record clearance and updated training along with CPR/First Aid. Two (2) resident files were reviewed and possessed all required paperwork which included Admissions Agreement, Needs and Service Plan, and Physician's Report. The listed administrator possesses a current administrator's certificate. Resident medication was centrally stored and locked in a cabinet located in the staff office. LPA reviewed medications prescribed to the residents and found all medication with required labeling found to be in place. Facility has an updated emergency and disaster plan and infection control plan. Facility conducts quarterly disaster drills. All facility exits were clear from obstructions. LPA observed emergency supplies and first aid kit with all required items. Fire extinguishers were fully charged and inspected. No deficiencies are being cited per Title 22, Division 6 of The California Code of Regulations.
