Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 6/23/26 to conduct a Annual Inspection utilizing the CARE inspection tool criteria. LPA met with staff and explained the purpose of the visit. Administrator was present to assist. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. Residents interviewed stated they are happy with care. Exit gate from the side yard will be repaired to open/close freely. LPA discussed PIN re.: Admin Cert LPA reviewed 2 resident files. Files are complete. Care plans will be kept current and reviewed with resident of POA. LPA did not review staff files as owner operators are both Admin Certified. On 5/12/26, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Administrator. On 5/6/26 R1 experienced a fall while toileting independently. The fall resulted in a a leg fracture. LPA reviewed records and received copies. LPA interviewed licensees. R1 medical assessment and care plan stated R1 transfers independently. The fall was unwitnessed. 9-1-1 was called to assist. As a result of today’s inspection, no deficiencies were noted. Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 5/28/25 to conduct a Annual Inspection utilizing the CARE inspection tool criteria. LPA met with staff and explained the purpose of the visit. Administrator was present to assist. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. Residents interviewed stated they are happy with care. Water temperature will continue to be monitored and fire extinguisher will be recertified. LPA advised developing a deep cleaning routine. LPA discussed new heat safety PIN and and pending Admin Cert .webinar. LPA reviewed 4 resident files. Files are complete LPA and licensee discussed organization or files. LPA reviewed 1 staff files. Staff file was complete. No deficiencies are being cited as a result of todays inspection. Allegation investigated: Facility staff are not ensuring that an appropriately skilled professional is assisting the resident with injections
On 7/26/24, Licensing Program Analyst (LPA) Kevin Mknelly LPA Mknelly arrived and met with Administrator to investigate and deliver investigation findings. LPA reviewed resident R1 records and interviewed the licensee. LPA finds that facility met Tittle 22 requirements. R1 was a resident 5/4/24- 7/5/24. LIC 602 stated R1 independently managed his injections. Discharge inventory found R1 had excess supplies of alcohol pads for his tests and injection. Licensee observed R1 tests and injections. R1 had made proceudres. Previous allegations that the department investigated on 6/28/24 that were unfounded. LPA received copies of R1's LIC 602, inventory and emergency contact. This agency has investigated the above complaint allegations. We have found that the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint. Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection following receipt of information relating to resident (R1). LPA met with Lita Greening, Administrator and Andy Greening, Administrator, and stated the reason for today's inspection. LPA discussed the specific information received from an outside source relating to (R1) with both Administrators. LPA and Administrator toured the facility, including the following areas: resident bathrooms, refrigerator/freezer food, smoke/monoxide alarms, filtered water from the refrigerator, and reviewed documentation related to the medication records. LPA observed that an antibiotic medication for (R1) that was filled on 6/21/24. Administrators confirmed the medication was picked up on 6/22/24 and the 8:00 pm dosage was administered on 6/22/24. (R1) was taken to the hospital on Sunday, 6/23/24, and was then transferred to another hospital/rehab in his health care group, where he remains for up to two weeks. LPA interviewed one resident (R2) about the food and water served at the facility. LPA observed (3) residents to be in their rooms during the inspection. Also discussed was Regulation 87224/Eviction Procedures. LPA printed a copy of the regulation for easy reference. There are no deficiencies issued in this report. Exit interview. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. The home is clean and residents interviewed stated they are happy with care. On 11/29/23, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Administrator. On 11/22/23, R1 was able to leave without assistance when unsupervised and a contractor had left the front door open. R1 was found by emergency responders at a nearby park and returned to the home uninjured. R1 has dementia and is unable to leave unassisted. As a result of the investigation, the following deficiencies were cited on 809-D, per Title 22 Regulations, Division 6. (A)This poses an immediate Health and Safety risk to clients/residents in care. (B) This poses a potential Health and Safety risk, or personal rights violation, to clients/residents in care. Report reviewed.
