Type of visit: Required - 1 Year

Licensing Program Analysts (LPA) Kimberly Lyman and Taylor Simerly conducted an unannounced visit to Grand View Villas Inc. The purpose of today’s visit was to conduct the Annual Required inspection. LPAs were allowed entry into the facility and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents, of which one may be bedridden. Facility has an approved hospice waiver for five residents and there was one resident on hospice care today. Administrator Rozina Amlani has an administrator certificate valid until August 28, 2026 and is waiting for her updated certificate. The facility has valid liability insurance through May 13, 2027. Upon entry, facility appears clean, safe and sanitary. LPAs toured the facility along with Administrator. LPAs toured the physical plant, checked food service, first aid kit and reviewed records. Facility is a one story home with 7 bedrooms (1 for staff), 7 bathrooms, living room, family room, dining room, kitchen, and an attached 2 car garage with laundry machines. The garage is kept locked and used for storage. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. LPAs observed two residents with half bed rails and one resident with full rails. Resident restrooms were checked. LPAs observed that the water was returned off in R5s restroom. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 113.3 and 115.5 degrees F in all facility restrooms. Resident toiletries and personal hygiene supplies were adequately stocked at time of visit. R2s bathroom did not have a cover on the overhead bathroom light and fan. Common areas were clean and clear of hazards, doorways were free of obstructions. LPAs observed the first aid kit with all the elements including thermometer, tweezers and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher is fully charged. The outside grounds were toured and there is ample shaded seating for residents. LPAs observed a fountain in the backyard. Exit gates are unlocked and operational. CONTINUED ON LIC 9099C DATED August 20, 2026. Emergency food was observed. The emergency disaster plan was reviewed during the visit. Facility provided documentation of last fire drill conducted on July 10, 2026 and drills are conducted quarterly. Facility provides activities in the form of exercise, games and music. LPAs reviewed resident and staff files. Resident files contained required documents including admission agreements, physician reports and resident appraisals. Staff files reviewed contained required documentation of medical clearance, training and criminal record clearance. LPAs reviewed medication storage and administration. Medications are stored in a locked cabinet. LPAs observed staff are taking blood pressure for two residents with parameter medication orders. None of the staff are skilled professionals. Medications appear to be administered per physician order. Licensee to forward an updated LIC 500 and LIC 308 to LPA by close of business on September 3, 2026. Based on the observations made during today's visit, deficiencies are being cited per Title 22 division 6 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Rozina Amlani and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents of which 1 may be bedridden. Facility is a single story home with 7 bedrooms (1 is for staff) and 7 bathrooms, kitchen, dining room, living room with a screened fireplace and a two car garage. The garage is used for storage and kept locked. The Administrator's Certificate expires on August 28, 2026. LPA and Administrator toured the facility. LPA observed the See Something Say Something Poster (PUB 475) in the main entry way of the facility. LPA observed the fireplace in the living room is screened. There is a large screen TV in the living room. The Administrator reported the fireplace is not used. LPA observed there is a two day perishable and a seven day non-perishable food supply on hand in the kitchen. Knives are kept locked in a kitchen cabinet. LPA observed the 5 burner gas stove lights unassisted. The fire extinguisher in the kitchen is fully charged. LPA observed all resident rooms had the required furnishings and bed linens. LPA observed all bathrooms are clean and operational. Hot water measured 115.7 degrees Fahrenheit in bathroom number 3 and the bathroom in the hallway. LPA and Administrator toured the garage. The garage is kept locked and used for storage. LPA observed the emergency food and water (3 day supply) stored in the garage. LPA observed the first aid kit has all the required elements. The last emergency drill was conducted on June 5, 2025. Smoke detectors/carbon monoxide detectors tested operational. LPA observed medications are kept locked in the hall closet along with the first aid kit. LPA observed the facility has a dedicated internet device for resident use (tablet). LPA and Administrator toured the backyard. There is a water fountain the backyard. There tables with umbrellas and chairs to sit outside. Both exit gates are operational. No obstacles or hazards observed in the backyard. LPA reviewed 4 resident files and medications, no discrepancies observed. LPA reviewed 2 staff files, both staff members are background cleared and associated to the facility. Both staff members have the required training. No deficiencies observed during the visit. LPA explained the reason for the visit. Facility is a single story home with 7 bedrooms (1 is for staff) and 7 bathrooms, kitchen, dining room, living room with a screened fireplace and a two car garage. The garage is used for storage and kept locked. LPA observed the See Something, Say Something poster posted in the entry way of the facility. LPA and the Administrator toured the facility. LPA observed all bathrooms are clean and operational. Hot water measured 112.0 degrees Fahrenheit to 115.0 degrees Fahrenheit. LPA observed the kitchen is clean and organized. LPA observed a two day perishable and a 7 day non-perishable food supply on hand in the kitchen. The knives are kept locked in a kitchen drawer. Cleaning supplies are kept locked under the kitchen sink. LPA observed the medication is kept locked in the hall closet. The fire extinguisher in the kitchen is fully charged. The smoke detectors and carbon monoxide detector tested operational. LPA and the Administrator toured the resident rooms. All resident rooms had the required furnishings. LPA toured the garage. The garage is used for extra supplies and emergency food and water. The garage is kept locked. LPA toured the backyard. There is a raised fountain in the backyard. LPA observed a shaded seating area with tables and chairs to sit outside. Both exit gates are operational. No obstacles or hazards observed in the backyard. LPA reviewed 3 staff files, all staff are background cleared and associated to the facility. All staff had the required training including CPR training. LPA reviewed 5 resident files, no discrepancies observed. LPA reviewed 5 resident medications and medication records. No discrepancies observed. LPA inspected the first aid kit, the first aid kit has all the required elements. LPA verified the facility has WiFi. LPA verified with the Administrator that the facility does not have an internet device dedicated for internet use. LPA consulted with the Administrator concerning reporting requirements and resident rights. No deficiencies are being cited as a result of this visit.
