Allegation investigated: Uncleared staff providing care to the residents

On 05/12/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to deliver complaint findings for the allegation above. LPA Pascua met with Facility Designated Administrator (FDA), Marinela Placintar and explained the purpose of the visit. Current census was 6. A brief interview with FDA Placintar. It was alleged that there is an uncleared staff member providing care to the residents. Based on information gathered, the determination of the exclusion order of S1 is still pending, however S1 is able to be present in the facility at this time. As a result of this investigation, this Department found the allegations to be UNSUBSTANTIATED. A complaint allegation finding of Unsubstantiated meant that although the allegations may have happened or was valid, there was not a preponderance of the evidence to prove that the alleged violation occurred. There were no deficiencies observed or cited at this time. An Exit Interview was conducted and copy of this report was provided to the facility at the end of this visit. On 05/12/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Facility Designated Administrator (FDA), Marinela Placintar and explained the purpose of the visit. Current census was 6. A brief interview with FDA Placintar was conducted. This facility is licensed to served 6 residents, all of whom may be non-ambulatory. This facility has a hospice waiver for 2 and has a dementia plan on file. LPA Pascua reviewed 5 resident files and 3 staff files. All files were complete and up to date. FDA Placintar has an active administrator certificate LPA Pascua initiated a tour of the facility with Licensee Placintar. The interior of the physical plant was in good condition and sanitary. Fire extinguishers appeared to have been annually inspected by Jorgenson Co and is valid until 02/26/2023.. The kitchen area was toured. LPA observed a non-perishable and perishable foods in the cabinets and refrigerator. Additional perishable food supplies were identified in the garage. LPA observed a locked centralized stored medication cabinet located in the kitchen. Along with the administrator, the LPA observed, reviewed, and compared resident medication and medication dispensing logs. First Aid Kit was present and contained all of the required components. A tour of the bathrooms was conducted. Hot water temperature was measured and observed to be within the required range of 105-120 degrees. A linen closet was located in the hallway and presented a sufficient amount of linens to adequately supply and meet the needs of the residents at this time. A tour of the bedrooms was conducted. Resident furniture was observed to be sufficient to meet their needs at this time. Laundry area was toured. Laundry detergent, bleach, and all other cleaning supplies were observed to be locked and made inaccessible to the residents at this time. Common areas were toured. Living room, dining area and all other areas intended for resident use were observed to be furnished and maintained in compliance at this time. A tour of the garage was conducted. Additional perishable food supplies were identified. The exterior of the physical plant was toured. Perimeter fence was observed to be stable and gates were in good repair. The following forms and documents were requested to be updated and submitted into CCL -LIC 308 -LIC 400 -LIC 500 -LIC 610 As a result of this visit, no deficiencies were observed or cited during this annual visit. Type of visit: Case Management - Legal/Non-compliance

Unannounced case management visit made out to this facility on 11/200/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility staff person, Joseph Romasanta, who was briefly interviewed at this time. This LPA requested that the facility staff person go ahead and contact the facility designated Administrator to inform them that CCL was present at this time. The facility designated Administrator, Marinela Placintar, arrived later to this facility. A brief discussion was held with the facility designated Administrator at this time. The purpose of this visit was explained to the facility designated Administrator at this time. This LPA stated that the nature of this visit was to inform this facility, and it's representative, that the person, Kenroy Anderson, was no longer fingerprint cleared and had been excluded from being employed or even be present in any licensed care facility through this Department at this time. On 04/04/2025, Licensing Program Analysts (LPAs) Arielle Pascua and Triel Lindstrom arrived unannounced to this facility to conduct an annual visit. LPAs met with Facility Designated Administrator (FDA), Kenroy Anderson and explained the purpose of the visit. It was learned on 04/03/2025 via email that the facility had a fire in the garage of the facility and residents were no longer residing at the facility and have been moved to other facilities at this time. LPAs conducted a tour of the facility to ensure that there were no residents in care. LPAs observed that there are currently no residents in care at this time. LPAs conducted a tour of 3 resident bedrooms. Bedroom 1 did not have a bed in the room at this time or belongings at this time. Bedroom 2 had two resident beds, one of which was a hospital bed. LPA Pascua observed resident belongings in the chest of drawers however, it was learned that this resident was out at the hospital at this time. Bedroom 3 had 2 resident beds with belongings such as incontinence supplies, pajamas, clothing, and pictures. Living areas, dining areas, and other areas intended were toured. A tour of the garage was conducted. LPAs observed black soot throughout the garage area. It was stated that the fire department has been at the facility and the fire is undergoing investigation. LPAs reviewed 4 resident files. A resident files were incomplete at this time. The following deficiencies were cited on the following LIC 809-D per Title 22, Division 6 of the California Code of Regulations. On 04/26/2024, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to conduct an annual visit. Upon arrival, LPA observed two construction workers at the facility who stated that they are currently working on flooring at the facility. LPA called Licensee, Marinela Placintar and notified her of CCL's arrival. It was learned that the facility does not have any residents in care at this time. LPA asked to walk around the facility to ensure no residents were in care. LPA observed flooring that was currently being installed throughout the whole facility at this time with no residents in care. Licensee Placintar stated that she would notify LPA when construction would be done and would start accepting residents. Licensee Placintar states that the floor may take a couple of weeks.
