On 08/20/2026, Licensing Program Analysts (LPA) Melina Oropeza arrived at this facility unannounced to conduct a Required 1 Year Annual Inspection Visit. LPA was met by staff and backup administrator, Venice Andrews and Licensee, Marinela Placintar joined 15 minutes later. LPA explained the purpose of the visit to backup administrator, Licensee and staff. This facility is licensed to serve 6 residents of which all may be non-ambulatory. This facility has a hospice waiver for 4 and a dementia plan on file. LPA and backup administrator inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility to ensure compliance with Title 22 regulations. Facility is a 6 bed facility with a current census of 6. There is entry door is leading to the living room, kitchen with a hallway to the bedrooms and bathrooms. Chemicals and medications noted to be locked to residents in care. LPA also conducted the care domain tool. No bodies of water were observed at the facility. Hot water temperature was measured at 114 F degrees Fahrenheit in resident bathroom sink, which is within the required range of 105 to 120 degrees Fahrenheit. All necessary documents were in place. LPA observed the following posted on the facility wall: Facility license, sketch, See Something Say Something poster, Ombudsman poster, Theft and Loss Policy, Resident Bill of Rights, Rights of Resident/Family Councils. LPA observed the facility to have adequate food supply of 7 days non-perishables and 2 days perishables in place. Resident rooms were sanitary and had the required furniture and furnishings. LPA observed, fire extinguisher inspected on 06/08/2026 and current, smoke and carbon monoxide detectors, central heating and air in the facility. The first aid kit was found in compliance A tour of the facility exterior grounds was conducted. A review of the facility perimeter fence, side gates, and all other exits was conducted and in good repair. LPA reviewed four (4) staff files. All staff is fingerprint cleared and associated to the facility and staff have current First Aid or CPR certifications on file. Facility is conducting initial and continuing training as required. LPA reviewed five (5) resident facility files. All necessary documents were in place. There were no deficiencies observed or cited during today's annual visit. Allegation investigated: Uncleared staff providing care to the residents

On 05/12/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to deliver complaint findings for the allegation above. LPA Pascua met with Facility Designated Administrator (FDA), Marinela Placintar and explained the purpose of the visit. Current census was 6. A brief interview with FDA Placintar. It was alleged that there is an uncleared staff member providing care to the residents. Based on information gathered, the determination of the exclusion order of S1 is still pending, however S1 is able to be present in the facility at this time. As a result of this investigation, this Department found the allegations to be UNSUBSTANTIATED. A complaint allegation finding of Unsubstantiated meant that although the allegations may have happened or was valid, there was not a preponderance of the evidence to prove that the alleged violation occurred. On 09/09/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA met with Licensee, Marinela Placintar and explained the purpose of the visit. The purpose of this visit was to conduct an annual visit. There were two staff members present, Christopher Nnam and Damian Campbell. This facility is licensed to serve 6 residents of which all may be non-ambulatory. This facility has a hospice waiver for 4 and a dementia plan on file. Current census is 4. A brief interview with Licensee Placintar was conducted. A review of 4 resident files were conducted. 4 out 5 residents were obtaining hospice services from various hospice agencies. A review of 3 staff files were conducted. 3 out 3 staff files were complete and up to date. The Facility Designated Administrator (FDA), Kenroy Anderson has an active certificate #7027437740 and expires on 01/02/2026. A tour of the facility was conducted. The interior of the physical plant was in good condition and sanitary. Fire extinguishers appeared to have been annually inspected by Jorgenson Co on 07/16/2025. The kitchen area was toured. LPAs observed a non-perishable and perishable foods in the cabinets and refrigerator. Additional food supplies were identified in the garage. LPA observed a locked centralized stored medication cabinet located in the kitchen. Along with the administrator, the LPA observed, reviewed, and compared resident medication and medication dispensing logs. First Aid Kit was present and contained all of the required components. A tour of the bathrooms was conducted. Hot water temperature was measured and observed to be within the required range of 105-120 degrees. A linen closet was located in the hallway and presented a sufficient amount of linens to adequately supply and meet the needs of the residents at this time. A tour of the bedrooms was conducted. Resident furniture was observed to be sufficient to meet their needs at this time. Laundry area was toured. Laundry detergent, bleach, and all other cleaning supplies were observed to be locked and made inaccessible to the residents at this time. Common areas were toured. Living room, dining area and all other areas intended for resident use were observed to be furnished and maintained in compliance at this time. A tour of the garage was conducted. Additional perishable food supplies were identified. The exterior of the physical plant was toured. Perimeter fence was observed to be stable and gates were in good repair. The following forms and documents were requested to be updated and submitted into CCL -LIC 308 -LIC 400 -LIC 500 -LIC 610 As a result of this visit, no deficiencies were observed or cited during this annual visit. On 09/17/2024, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA met with Licensee, Marinela Placintar and explained the purpose of the visit. The purpose of this visit was to conduct an annual visit. There were two staff members present, Emmanuel Akinleye and Sandy Laney. This facility is licensed to serve 6 residents of which all may be non-ambulatory. This facility has a hospice waiver for 4 and a dementia plan on file. Current census is 5. A brief interview with Licensee Placintar was conducted. A review of 5 resident files were conducted. It was observed that all residents had a dementia diagnosis and did not updated Needs and Services plans on file. 4 out 5 residents were obtaining hospice services from various hospice agencies. A review of 3 staff files were conducted. 3 out 3 staff files were complete and up to date. A tour of the facility was conducted. The interior of the physical plant was in good condition and sanitary. Fire extinguishers appeared to have been annually purchased on 08/30/2024. The kitchen area was toured. LPAs observed a non-perishable and perishable foods in the cabinets and refrigerator. Additional food supplies were identified in the garage. Allegation investigated: Staff hit resident

On 10/17/2023, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a complaint visit. LPA met with Facility Designated Administrator (FDA), Kenroy Anderson and explained the purpose of this visit. The purpose of this visit is to delivery complaint findings for the allegation above. Current census was 6. A brief interview with FDA Anderson was conducted. It was alleged that a staff member hit a resident. During the course of this investigation LPA reviewed facility records and conducted interviews. Based on interviews conducted it was learned that it was alleged that an individual who was delivering medication witnessed a staff member hit a resident. It was reported that the individual delivering medication peaked into the window and stated that the resident was being fed at the time and pushed the staff member’s hand away and in response the staff member hit the resident. Unsubstantiated It was learned through this interview that this individual could not recall where the resident was hit. An interview with facility staff was conducted. It was learned through staff interviews that the facility conducted an internal investigation and found no evidence to show that the resident was hit. Staff members present at the time of the incident denied hitting any residents. LPA also conducted an interview with the resident, however, due to medical conditions could not complete the interview. LPA reviewed facility records of pictures that were taken after the alleged incident where it was observed that the resident did not have any redness, marks or bruising around their face or body. Based on the information gathered, it is unclear if the staff member hit the resident. Based on information provided through interviews and records reviewed, this allegation is deemed UNSUBSTANTIATED, meaning that there was not a preponderance of evidence to prove or disprove that the allegation occurred as reported. There were no deficiencies observed or cited at this time.
