On 5/11/26 Licensing Program Analysts (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by (S1) Daisy Navarro, LPA were granted entry. 5 residents were present during inspection. House Manager (HM) Nancy Thompson was also present during LPA’s arrival. LPA toured facility with HM. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were checked and observed locked in medication room. Clients’ MARS was reviewed. Fire extinguisher reviewed with a service date of: 3/11/26. Fire drill completed on 4/8/26. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in garage. Resident’s bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 105 to 106.3 degrees F in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at -6 degrees F and refrigerator temperature maintained at 33 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. All client’s files and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. Allegation investigated: Staff financially abuse residents in care.

On 2/10/26 Licensing Program Analyst (LPA) M. Garza arrived at the facility to complete an unannounced complaint visit. LPA met with House Manager, Nancy Thompson, explained reason for visit and was permitted entry into the facility. Licensee, Kristie Rackley was contacted and arrived some time later. A tour of the facility inside and out was completed. A health and safety check was completed on residents in care. Residents observed in common area and in rooms. During visit LPA requested and reviewed documents and completed interviews. Interviews and records reviewed disclosed S1 has not work at the facility at any point in time and the allegation took place at another facility. The Department has found that the complaint allegation is UNFOUNDED, meaning that the allegation was false, could not have happened or is without a reasonable basis. The complaint is dismissed. Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with facility staff Anna McDonald, Continual Administrator's Certification for Jessica Serna expires 10/25/2025. There are currently 5 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 105 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA reviewed Infection Control Plan, and Emergency Disaster Plan, staff, and resident files. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. On 5/21/24, Licensing Program Analyst (LPA) M. Medina arrived to conduct an Annual Required Inspection. Administer, Anna McDonald contacted by telephone and not available to conduct today's inspection. LPA met with House Manager, Nancy Thompson during today's inspection. Currently, there are six (6) in care. All residents attend an in home Day Program, Monday-Friday 9:30am - 12:30 pm. Facility toured inside and outside. Living room and dining room have adequate furnishings and lighting. Resident bedrooms have all required accommodations. Residents bathroom observed to be clean and all fixtures working properly. Hot water temperature in bathrooms measured at 118 degrees F. LPA observed a supply of extra bed linens and personal hygiene and grooming products. Kitchen toured, a 7 day of non-perishable and 2 day of perishable available. Medications are locked and secured in medication room. Medications observed to have original labels and to be administered as prescribed. A fire extinguisher is present and was serviced during today's visit on 3/08/24. Smoke detectors and carbon monoxide detector tested and observed operational. Last fire drill was conducted on 4/2024 according to facility records. Outside of the facility toured. All exits open free of obstruction. Gate observed to be self latching. No outside hazards were observed.
