On 8/26/2025, Licensing Program Analyst (LPA) J. Leffall arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by staff (S1) Leanna Lopez. Administrator (A1) Mark Achazo arrived shortly after LPA’s arrival. 0 residents were present during visit as they were attending day program. LPA toured facility with S1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were observed locked in kitchen cabinet. Clients’ MARS was reviewed with samples of resident’s medications. Fire extinguisher reviewed with a service date of: 4/17/25. Fire drill completed on 8/17/25. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in closet. Residents bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 108.5 to 110.8 degrees in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at 0 degrees F and refrigerator temperature maintained at 39 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. All clients’ files and samples of staff’s files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. On 10/10/2024, Licensing Program Analyst (LPA) K. Kaur arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit and was allowed entry by staff Allen “Mark” Achazo. Administrator was contacted and arrived at a later time. Facility tour was conducted with staff. The facility was observed to be at a comfortable temperature, in good repair, and no passageway obstructions observed inside or outside. Common areas were properly furnished and well-lit throughout. The dining room is equipped with a table and chairs, living room is equipped with adequate sofas and recliners for four residents, adequate outside space for rest and recreational under a Pergola. Backyard gate is self-closing and self-latching. At 11:33 AM LPA did not observe 7-day supply of non-perishable foods. 2-day supply of perishable foods was observed. Knives are locked in the kitchen cabinet. Cleaning and Chemical supplies are kept in locked cabinet under the kitchen sink and garage cabinet. Fire extinguisher in kitchen was serviced on 8/29/2024. Residents' bedrooms were observed to be adequately furnished with bed, dresser, and adequate lighting. Mattresses and linen were in good condition. At 11:20 LPA observed dust buildup on fan and walls, and AC vent residue streaks on bathroom walls. Extra linen and towels are available. Carbon monoxide and smoke alarm detectors installed and operational. Grab bars installed in showers and by toilets, non-skid mats in place. Resident's records contained Personal Rights, and current Physician's Report. Resident’s files have updated emergency contact information. Staff files were reviewed for good health. It was verified that there are at least one staff on duty who is CPR certified. At 12:34 LPA observed Facility Liability insurance was expired as of August 2024. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22,Division 6. LPA is requesting the following documents be submitted to the Fresno CCL office by 10/07/2024: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Emergency and Disaster Plan, Personnel Report (LIC500), Register of Facility Clients/Residents LIC9020.
