Type of visit: Required - 1 Year

On 07/09/26, Licensing Program Analysts (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit and requested to meet with Administrator. LPA met with House manager (S1) Alicia Lomeli. Administrator Renee Arreguin was called and arrived later during visit. LPA toured facility with S1. All four residents were present during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Knives were observed locked in the kitchen. Medications observed kept locked in medication closet. Fire extinguishers were observed with a service date 01/16/26. Last fire drill was completed on 6/18/26. Temperature maintained for refrigerator at 34 degrees F. 2 day supplies of perishable and 7 day supplies of non-perishable food were observed. Cleaning supplies observed stored and locked under kitchen sink. Gardening tools observed locked in garage cabinet. All bedrooms were observed to have required furnishings and with adequate lighting. Bathroom was toured. LPA observed with securely fastened grab bars and non-skid mats. Hot water temperature was tested at 105.1 degrees F in bathroom. Extra linens observed in hall closet. Outside of facility toured and observed to be free of debris with adequate outdoor seating available for residents. All residents and a sample of staff files were reviewed. Carbon monoxide and smoke detectors were tested and observed to be operational. No deficiency cited during visit. Exit Interview conducted. LPA received copies of Lic 308, Lic 610E, Lic 9020, and current liability insurance. The following documents is requested to be submitted to the Fresno CCL by 07/14/26: Lic 500 and Lic 610E. Allegation investigated: Staff threatens the residents while in care

On 06/28/25, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannonced to deliver findings on the above allegations. LPA met with Administrator, stated the purpose of the visit and was allowed entry into the facility. During the investigation, LPA conducted interviews, reviewed records and toured the facility. Based on the information received, and although the allegation may have happened, there is not a preponderance of evidence to prove that the alleged violation occurred, therefore the allegation is Unsubstantiated. Type of visit: Required - 1 Year

On 06/28/25, Licensing Program Analyst LPA L. Salazar arrived at the facility unannounced to conduct the required annual inspection. LPA was greeted by Staff S1, stated the purpose of the visit and was allowed entry into the facility. S1 contacted Administrator, Renee Arreguin Rodriguez who arrived to the facility shortly after LPAs arrival. Administrator Certificate #7012073740 Exp 07/29/26. LPA toured the facility inside and out. LPA observed 4 residents in care at the time of visit. Facility is a 3 bedroom / 2 bathroom house. Resident bedrooms were observed to have the required lighting/furnishings and are free from odor and passageway obstruction/fire hazards. Bathrooms were observed to have operational lights, running water, and non- slip floors. Hot water temperature tested at 105 degrees F. Required postings were observed. Cleaning supplies were observed to be locked in the laundry room. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Carbon monoxide detector was observed to be operational. Fire Extinguisher was observed with a service date of 01/21/25. First aid kit was observed to contain all required items. LPA reviewed resident and staff files. 3 out of 4 residents in care attend day program. 1 out of 4 residents do not attend. LPA asked R1 if they would like to attend program and R1 replied, "yes!". LPA observed R1's IPP which states, Resident R1's IDteam agreed that due to decline in health, it was best for R1 to not return to day program. R1 has not been in day program since their admission date of 01/22/2024. . Interviews with Administrator state that the responsible party of R1 would like R1 to attend day program and hoping for even part time so R1 can have the socialization interaction. (Continuned on LIC 809) Continued from LIC 809) LPA requested the following documents be submitted to the Fresno CCL office by 07/18/2025: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents (LIC9020A), and Surety Bond. On 06/13/24, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the required annual Inspection. LPA was greeted by Direct Support Personnel (DSP II), stated the purpose of the visit and was allowed entry into the facility. DSP II contacted the Administrator, who arrived to the facility shortly after. LPA toured the facility inside and out. Administrator on record is Rene Arreguin, Certificate #6020726740, Exp. 10/28/24. LPA observed 2 residents in care at the time of visit. Facility is a 4 bedroom 2 bathroom home. The front bedroom is designated as an office Bathrooms were observed to have grab bars by the toilets and a grab bar in the shower used by residents. Residents in care receive Regional Center services. Resident bedrooms were observed to have the required lighting and furnishings and were free from odor and free from any passageway obstruction / fire hazards. Facility temperature was 78 degrees F. Bathrooms were toured and observed to have operational lights, running water, and non- slip floors. Hot water temperature tested at 107 degrees F. Trash can with lid and hand washing postings were observed. Medications were observed to be locked in a cabinet located in the entry. Cleaning supplies were observed to be locked in the garage. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Carbon monoxide and smoke detectors were tested and observed to be operational. Night lights were observed in the hallways. Fire Extinguisher was observed with a service date of 02/07/24. First aid kit was observed and contained all required items. Internet devices and a working phone line were observed to be available for residents in care. (Continued on 809-C) (Continued from 809) Required postings were observed for Non-discrimination LGBTQ-A+, Personal Rights of Residents in RCFE (87468.1 and 87468.2), facility's visitation policy and LETUSNO Complaint Poster, (PUB475), Ombudsman poster and Resident Council Rights were observed. No residents are receiving Hospice services residents or receiving Home Health care service. A sample of resident and staff files were reviewed and observed to have the required forms and training records.
