Type of visit: Required - 1 Year
Licensing Program Analysts (LPA) Daiquiri Boyd arrived unannounced to conduct the Annual Inspection. Administrator was called to meet LPA at the facility. LPA met with and explained the reason for the visit with Administrator (AD) Teresa Long. During this visit, LPA toured the facility inside & out. Resident rooms contained required furnishings and lighting. The bathrooms were found to be clean with faucets delivering hot water at 110 degrees. LPA observed required hygiene items, towels, extra bedding, and linens which were stored and available for use. The kitchen was clean, with necessary items and appliances, all sharp objects were in a locked cabinet. LPA observed required food supply and paper product storage. Medications are locked and centrally stored in a locked kitchen pantry closet. Common and activity areas were clean and occupied by residents throughout. There are visitation areas available inside and out. Doors and passageways are unobstructed throughout the facility. The fire extinguisher was serviced 8/6/25 and the gauge is in the green. Temperature in the home was 75 degrees. Smoke/carbon monoxide detectors were seen throughout the home and smoke alarm was tested and working. Backyard has shaded patio and self closing and self latching doors. LPA reviewed three resident files and two staff files. All files had required documents. LPA requested Licensee to submit the following documents to Licensing by 9/7/25: LIC500, Proof of Liability Insurance No deficiencies were cited on this day. Type of visit: Case Management - Incident
Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced visit to the facility to discuss an Incident Report submitted to the Department. Staff at the home called Administrator (AD) Teresa Long, who arrived at the home a short time later. LPA explained to AD that I was there to discuss the incident that was reported on 1/5/26 with resident (R1) (See Confidential Names list on LIC811). AD pulled up a video of the night of the incident. The video showed R1 coming out of one of the reisdent's rooms sliding glass door at 19:59 pm (and approach the side fence. R1 then climbed over the fence and into the neighbors property, which was in the front of the neighbor's side fence, where he walked away from the home. Staff on duty never heard an alarm go off and didn't realize he was not in the home. At the 8 pm medication pass, it was realized that the resident was not in the home. Administrator was then notified and as protocol was followed. Licensee has added extra fence boards to the fence. There is no longer a place for residents to place their foot to hop over easily. Staff now take R1 out of the facility on regular walks every day to help him to not feel so restless. No deficiencies were issued on this day.the state’s words, verbatim · CDSS document, Apr 20, 2026

There are visitation areas available inside and out. Doors and passageways are unobstructed throughout the facility. The fire extinguisher was serviced 8/6/25. Temperature in the home was 75 degrees. Smoke/carbon monoxide detectors were seen throughout the home and smoke alarm was tested and working. Backyard has shaded patio and self closing and self latching doors. Allegation investigated: Facility staff do not maintain passageways free from obstruction
On 9/12/24, Licensing Program Analyst (LPA) M. Medina conducted an unannounced complaint visit to deliver complaint findings. LPA introduced self, stated purpose of visit, and allowed entrance by staff. Executive Director, Teresa Long contacted by telephone and arrived to conduct complaint visit with LPA. This department investigated the above allegation, during the investigation, facility was toured, interviewed conducted and records reviewed. Based on information gathered during investigation, This Department has found that the above allegation is UNFOUNDED, meaning they were false, could not have happened, and/or were without reasonable basis. We have therefore dismissed the complaint. Exit interview was conducted and a plan of correction developed and reviewed. A copy of this report provided to Administrator for facility records. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) M. Flores arrived at the facility unannounced to conduct a required annual visit. LPA was granted entry by Administrator Assistant, Jaycee Sanderson (AD1). The residence was set at 75 degree F temperature and free of passageway obstructions inside and outside. LPAs observed six bedrooms in the residence. Residents' rooms were toured and inspected. Rooms were found to be clean, and furnishing was in good condition. Hot water temperature was measured at 105.9 degrees F. Kitchen toured, supply of food observed, and food stored properly for perishable and nonperishable. Medication and knives are locked in the kitchen area. Cleaning supplies were locked in the laundry room. Smoke detectors and carbon monoxide are dual detectors, they were checked and operating. Fire extinguishers were charged and was serviced on 8/2/2023. Emergency disaster drills are conducted quarterly, last drill completed on 9/01/23. First Aid kit is fully equipped. There was outdoor seating for the residents. Outdoor area was clean and free of obstruction. An exit interview was conducted, and a copy of this report was provided to AD1 whose signature confirms receipt.
