Licensing Program Analysts (LPA) Daiquiri Boyd arrived unannounced to conduct the Annual Inspection. LPA met with and explained the reason for the visit with Administrator (AD) Teresa Long. During this visit, LPA toured the facility inside & out. Resident rooms contained required furnishings and lighting. The bathrooms were found to be clean with faucets delivering hot water at 116.7 degrees. LPA observed required hygiene items, towels, extra bedding, and linens which were stored and available for use. The kitchen was clean, with necessary items and appliances, all sharp objects were in a locked cabinet. LPA observed required food supply and paper product storage. Medications are locked and centrally stored in a locked kitchen pantry closet. Common and activity areas were clean and occupied by residents throughout. There are visitation areas available inside and out. Doors and passageways are unobstructed throughout the facility. The fire extinguisher was serviced 8/5/26. Temperature in the home was 74 degrees. Smoke/carbon monoxide detectors were seen throughout the home and smoke alarm was tested and working. Backyard has shaded patio and self closing and self latching doors. LPA reviewed three resident files and three staff files. All files had required documents. LPA requested Licensee to submit the following documents to Licensing by 08/20/26: LIC500 (Personel Report), LIC9020 (Resident Roster) No deficiencies were cited on this day. This Annual Inspection was postponed due to an outbreak of Covid. Staff Britney Arrendondo met us at the facility and brought the staff files. During this visit, LPA toured the facility inside & out. Resident rooms contained required furnishings and lighting. The bathrooms were found to be clean with faucets delivering hot water at 113.8 degrees. LPA observed required hygiene items, towels, extra bedding, and linens which were stored and available for use. The kitchen was clean, with necessary items and appliances, all sharp objects were in a locked cabinet. LPA observed required food supply and paper product storage. Medications are locked and centrally stored in a locked kitchen pantry closet. Common and activity areas were clean and occupied by residents throughout. Allegation investigated: Licensee does not ensure that resident's care needs are being met while in care Licensee does not ensure that waste at the facility is handled in a manner that does not pose a health or safety risk to resident in care Staff do not accord dignity to resident in care.

Licensing Program Analyst (LPA) Daiquiri Boyd conducted the complaint investigation visit to the facility. During this visit LPA delivered investigation findings regarding the above allegations. The Department has investigated the complaint alleging: Licensee does not ensure that resident's care needs are being met while in care, Licensee does not ensure that waste at the facility is handled in a manner that does not pose a health or safety risk to resident in care, and that staff do not accord dignity to resident in care. Based on the interviews conducted and facility records reviewed, the above allegation is UNSUBSTANTIATED. Through interviews, staff confirmed their responsibilities for providing care and dignity and reported that they attended to residents as required, which is contrary to allegations and statements made by complaint report. LPA could not confirm through evidence, that resident's needs were not met, that resident was not afforded dignity, or that waste was mishandled. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
