At 9:00am, on 6/1/2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Licensee Jane Quines, announced who he was and the reason for the visit. Licensee and LPA conducted a full tour of the facility. This facility is a single-story residential home with six private resident bedrooms, two live-in staff bedrooms, and two and a half public bathrooms. There is a family room, dining area, kitchen and living room. LPA noted a variety of puzzles, games, books, resident appropriate exercise equipment and a piano for resident use in the living room. Access to the laundry room is next to the kitchen. Laundry detergent and cleaning chemicals are located in the laundry room and an adjacent storage closet all locked for resident safety. LPA noted that the side yard and the front yard both have seating and shade for residents and visitors. LPA noted fresh fruit and snacks in the kitchen for residents to enjoy freely. The facility has wired smoke detectors in each room that were tested by MACE Security company on 2/3/2026 and the sprinkler system was tested on 3/31/2026 by Mid-Coast Fire Protection. There is a carbon monoxide detector in the hallway and family room, functioning normally. LPA observed a fire extinguisher near the kitchen that was tagged current and in the green compression range, serviced on 11/14/2025. This facility has two hot water heaters, LPA tested facility hot water at a faucet connected to each measuring 112.3°F and 118.9°F, within regulation temperatures 105-120°F. LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility is clean and in good repair with no obstructions in hallways, doorways or exits. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records, finding no violations. Staff and resident files are locked in the hallway. LPA conducted a staff and resident file review. LPA and Licensee conducted a review of the annual care tool modules. There were no deficiencies cited at this time. LPA met with Licensee/Administrator Jane Quines, announced who he was and the reason for the visit. Licensee/Administrator and LPA conducted a full tour of the facility. This facility is a residential home with six single occupancy resident bedrooms, two live-in staff bedrooms, two full shared use bathrooms, and one half public bathroom. There is a family room, dining area, kitchen and living room. LPA noted a variety of puzzles, games, books, resident appropriate exercise equipment and a piano for resident use in the living room. Access to the laundry room is next to the kitchen. Laundry detergent and cleaning chemicals are located in the laundry room and an adjacent storage closet all locked for resident safety. LPA noted that the side yard and the front yard both have seating and shade for residents and visitors. LPA noted fresh fruit and snacks in the kitchen for residents to enjoy freely. The facility has wired smoke detectors in each bedroom and hallway leading to bedrooms that are all working, there is a carbon monoxide detector in the family room above the thermostat that is functioning normally. LPA observed a fire extinguisher near the kitchen that was tagged current and in the green compression range, serviced on 11/19/2024. LPA tested facility hot water at 111.9*(f), within regulation temperatures 105*-120* (f). LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility is clean and in good repair with no obstructions in hallways, doorways or exits. The facility has a built-in natural gas generator for power outages. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records, finding no violations. LPA conducted a staff and resident file review. At 1:15pm on 07/15/2024. Licensing Program Analyst (LPA) Jeffries arrived unannounced at the facility to conduct the annual facility inspection. LPA met with Administrator Jane Quines announced who he was and the reason for the visit. Administrator and LPA conducted a tour of the facility inside and outside. The facility has an out door area with shade and benches for the residents. LPA noted that facility has an industrial generator, powered by gas for full power during outages The facility has a large living room another large activity room, large dining room and large kitchen. The facility has six single resident occupancy bedrooms, 3 bathrooms that are shared. All residents rooms have working smoke detectors, proper linin, lighting, and storage for residents. There is a internal sprinkler system that is serviced annual by Culbert Plumping on 03/12/2024. LPA observed smoke detectors that are hard wired in all rooms and two carbon monoxide detectors all in working order. LPA noted that all hallways, passages and doors were not obstructed. The facilities first aide kit is complete, located in the dining room cabinets. There is a locked medication cart in the living room area. There is an additional small refrigerator for medication that requires refrigeration where they use a locking case inside the refrigerator for that type of medication storage. LPA conducted a medication audit and found all medication documents to be in good order. LPA reviewed all staff falls and found all documents to be in good order. LPA reviewed all resident files and found all documents to be complete and in good order. LPA observed at least 7 days of perishable foods and at least 2 days of non perishable foods on hand at the facility. LPA noted that the facility is clean and in good repair. LPA did not discover any violations, citations, or technical during the facility physical tour. Administrator and LPA reviewed all modules of the annual inspection. LPA found no citations, violations, or technical during the review of all the modules of the annual inspection. LPA noted at this time there are no citations, violations or technical issued as a result of this annual facility inspection.
