Type of visit: Required - 1 Year

On 12/23/2025, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the facility annual inspection. LPA met with facility Administrator Aurora Galinato, introduced himself and explained the purpose of the visit. This is a 3 bedroom, 2 bathroom, 2 living rooms, kitchen, and dining room, there is a fenced back yard and a fenced front yard with seating and substantial covered porch for resident activities, leisure and visitors. All three bedrooms are double Resident occupancy. LPA noted that each room has proper furnishing, linins, and lighting. LPA noted that the bathrooms have liquid soap and paper towels and water temperature tested throughout the facility is within regulation permitters of 105*-120*(f). LPA observed at least 2 days of perishable foods and 7 days of non permeable foods on hand for 6 residents and staff. LPA noted that the emergency water supply is located in the garage. LPA noted that the medications are locked in the dining room and there is a complete first aide kit also located with the medications. LPA noted that there are smoke detectors located throughout the facility and carbon monoxide detectors are in each residents room. LPA noted that the facility has Personal Protective Equipment (PPE) and incontinence supply on hand. LPA noted that the facility is clean and in good repair. LPA observed two fire extinguishers both primed and in the green and currently tagged as working. LPA observe all door ways and passage ways to be free and clear of obstacle. LPA noted that required postings are in the front room and hallways of the facility. LPA conducted a review of Emergency Disaster Plan, Infection Control Plan, Staff and Resident Files and a cursory review of Centrally Stored Medication Records (CSMR). LPA noted that there were no violations or citations as a result of the physical inspection of the facility. Administrator and LPA conducted a full review of the annual care tools module and noted no violations or citations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Rankin arrived at 10:07 am to conduct a 1-year annual visit to the facility above. LPA met with Administrator Aurora Galinato and explained the purpose of the visit. A tour of the inside and outside of the facility was conducted. The following was inspected and noted during the annual visit: Physical Plant & Environment Safety: The facility has 3 resident bedrooms, 2 bathrooms and currently occupies 4 residents. The facility has smoke and carbon monoxide detectors that were tested and working properly during visit. The lighting and lamps are sufficient for the use of the facility and for resident comfort. Toilet, hand washing and bathing facilities are operational and secured grab bars are present. The showers have non-skid mats or textured bottoms. The pathways are clear of any obstructions. Disinfectant, cleaning solutions and poisons are inaccessible to residents in care locked under sink and in outdoor locked sheds. The facility has a fenced backyard for client use with plenty of shade. The facility has telephone and internet service for resident use. The fire extinguishers were charged and an appointment for inspection was scheduled for 12/3/24. Operational Requirements: The facility has a current plan of operation on file with the department. The Facility is operating in compliance with the granted fire clearance. The facility has current liability insurance which expires on 12/21/2024. The facility is approved for a capacity of 6. The fire clearance is granted for 6 Non-Ambulatory. Hospice is approved for 5. Continued 809-C Personnel Records & Training: Staff files had current 1st Aid/CPR, file reviewed for required documentation, files were found complete. Administrator certificates expires 11/06/2025. Staff have annual training completed for all subjects/topics and hours for 2024. Resident Records & Incident Reports: Facility does submit incident reports to the department when required. LPA reviewed 4 resident files, ensured updated Appraisals Needs and Services Plan were documented and all other required documents were available, all records were in compliance. Food Service: The facility handles and prepares food safely. The facility has 2-day perishables and 7-day non-perishables and plenty extra, to meet the food service requirement. All food is covered, stored, and marked appropriately. Food, snacks, and drinks are available when the residents want them. Facility is reviewing dry good storage regularly to ensure it is free of expired items. Cleaning solutions and equipment are stored separately from food supplies. Incidental Medical Services: Facility provides assistance in arranging transportation to medical and dental appointments when needed. Residents have on-call Doctor that visits facility. The facility uses the Medication Administration Record (MAR) along with the Centrally Stored Medication and Destruct Records (CSMDR). LPA reviewed and discussed residents’ medications, medication practices and documentation are in complaisance. Disaster Preparedness: The current emergency disaster form was reviewed and discussed with administrator. The facility conducts quarterly disaster drills. Emergency exits and telephone numbers were posted. A set of keys is available for staff on all shifts to access full facility in an emergency. Residents with Special Health Needs: The facility does accept dementia residents in care. The facility has 3 gates, each in good repair. Facility currently has two residents on oxygen. The facility currently has 1 hospice resident in care. The facility does not currently have any residents receiving Home Health services. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA's) Miller and De Leon arrived at 9:00 am to conduct a 1 year annual visit to the facility above. LPAs met with Administrator Aurora Galinato and explained the purpose of the visit. A tour of the inside and outside of the facility was conducted. The following was inspected and noted during the annual visit: Infection Control: The facility has a current Infection Control Plan. The facility has a sign in and out binder for visitors at entry with hand sanitizer. The bathrooms have toilet paper, paper towels, hand soap, and hand washing signs. The facility has EPA approved disinfectants spray and cleaners. The facility has a 30 day supply of PPE. Quarantined or isolated individuals will have meals and medication delivered to rooms. Staff are trained on infection control and the use of Personal Protective Equipment (PPE). All trash cans and wastebaskets have tight fitting covers. Physical Plant & Environment Safety: The fire extinguishers were last charged and inspected on July 18, 2023, and they are charged. The facility has 3 resident bedrooms, 2 bathrooms and currently occupies 4 residents and employs 4 full time staff and 1 Administrator. LPAs were authorized to enter and inspect facility. The facility has smoke and carbon monoxide detectors that were tested and working properly during visit. The lighting and lamps are sufficient for the use of the facility and for resident comfort. Toilet, hand washing and bathing facilities are operational and secured grab bars are present. The showers have non-skid mats or textured bottoms. The pathways are clear of any obstructions. Facility is well lit inside and outside for safety. Disinfectant, cleaning solutions and poisons are inaccessible to residents in care locked under sink and in outdoor locked sheds. The facility has sufficient space inside and outside for activities and visiting. The facility has a fenced backyard for client use with plenty of shade. The facility has telephone and internet service for resident use. Continued 809-C Operational Requirements: The facility has a current plan of operation on file with the department. The Facility is operating in compliance with the granted fire clearance. The facility has current liability insurance and expires on 12/21/2024. The facility is approved for a capacity of 6. The fire clearance is granted for 6 Non-Ambulatory. Hospice is approved for 5. Staffing: The facility currently employes 4 full time staff and 1 Administrator. Staff records are kept confidential. Files reviewed had current 1st Aid/CPR, Personnel Records/Application, Health screening with TB results, Criminal Record statements, and Finger print clearance. Administrator file was reviewed for Continuing Education requirements and current Administrator Certificate. Administrator Certificates expires 11/6/2025. Personnel Records & Training: The facility keeps confidential files for each staff member. Staff have annual training completed for all subjects/topics and hours for 2023. Resident Records & Incident Reports: The facility keeps separate files on each resident confidentially. Facility does submit incident reports to the department when required. LPA reviewed 5 resident files for signed Admission Agreements, Personal Rights, Safeguard for property and valuables, LIC. 602A Physicians report, Appraisals Needs and Services Plan, Emergency and ID forms, all forms were legible and records are kept confidential. Food Service: The facility handles and prepares food safely. The facility has 2-day perishables and 7-day non-perishables and plenty extra, to meet the food service requirement. The freezer is kept at 0 degrees and the refrigeration is kept at 40 degrees or lower. All food is covered, stored and marked appropriately. Food, snacks and drinks are available when the residents want them. Cleaning solutions and equipment are stored separately from food supplies. Kitchen staff are observed for personal hygiene and food sanitation practices. Continued 809-C Incidental Medical Services: Facility provides assistance in arranging transportation to medical and dental appointments when needed. Residents have on-call Doctor that visits facility. The facility uses the Medication Administration Record (MAR) along with the Centrally Stored Medication and Destruct Records (CSMDR). LPA reviewed residents’ medications, no labels were altered, no medications were expired and all medications were kept in their original containers. Disaster Preparedness: The current emergency disaster forms were posted. The facility conducts quarterly disaster drills. Emergency exits and telephone numbers were posted. A set of keys is available for staff on all shifts to access full facility in an emergency. Residents with Special Health Needs: The facility does accept dementia residents in care. The facility has 3 gates on side of the home, each in good repair. Facility currently has two residents on oxygen . The facility currently has 1 hospice residentf in care. The facility does not currently have any residents receiving Home Health services. The facility does not have delayed egress, locked doors or gates. Exit door alarms are working.
