On 06/04/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced Annual required visit using the new Care Inspection Tool. LPA was met by the Administrator Rosemarie Famison as the purpose of today’s visit was explained. The facility is licensed to serve 6 adults ages 60 and older of which (3) may be non-ambulatory and (1) bedridden. There is an approved hospice waiver for (6). Facility fees are current. Administrator certificate is active (7028457740 Exp: 08/11/26). Liability insurance is active (PCI42577191-04 EXP05/25/27). The facility is a single-story structure located in a residential neighborhood and consists of the following: a living, dining room, kitchen, (4) bedrooms, (1) staff bedroom, (2) bathrooms, an outdoor laundry/ storage area, a ramp that is accessed from bedroom #1, a detached garage, a backyard with shaded sitting. There is a working land line. There are no bodies of water or firearms on the premises. All resident rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for residents personal belongings was observed. Walls and floors were in good repair. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries available. The water temperature properly measured between 105-120 F.. A supply of perishable and non-perishable food was observed, toxins and knives are stored and inaccessible to residents. There no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards. Fire extinguishers were charged, smoke detectors and carbon Monoxide are operable. Last fire drill was conducted on 04/20/26. LPA observed a first aid kit and manual. LPA reviewed 3 staff files and 5 resident records, and 5 medication administration records (MAR). On 05/5/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced Annual required visit with a primary focus on infection control measures, using the new Care Inspection Tool. LPA was met by the Administrator, Rosemarie Famison and the purpose of today’s visit was explained. The facility is licensed to serve 6 adults age range 60 and older. There are currently three (6) residents in placement. The facility consists of living and dining rooms, kitchen, 4 bedrooms, 2 bathrooms, front and back yards. LPA reviewed 3 staff files and 6 residents and found that they include all required documents. LPA Shirley and Rosemarie toured the physical plant. There are no bodies of water or firearms on the premises. All resident rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for client personal belongings was observed. Walls and floors were in good repair. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured 110.7 F. All fire extinguishers were operable. A comfortable temperature is maintained in the facility. LPA observed the facility to be clean and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning agents, toxins, and sharps were inaccessible to clients. The kitchen was inspected and there is enough perishable and non-perishable food available which is stored properly. Fire extinguishers were charged, smoke detectors and carbon Monoxide were operable. LPA checked first aid kit; and found that it was compliant with a manual. LPA also observed that all mandated posters were posted. There were no deficiencies observed during today’s visit. On 05/8/24, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced required annual visit using the full CAREs Inspection Tool. LPA met with Administrator, Rosemarie Famison and explained the purpose of today’s visit. The facility is licensed to serve elderly developmentally disabled residents ages 60 years and older. LPA reviewed all resident files and found they contained the required documents. LPA reviewed two (2) staff files and found they contained the required documents, training, and certification. LPA received a copy of the Liability Insurance. LPA Felisa and Rosemarie toured both inside and outside of the facility. The facility is a one-story structure located in a residential neighborhood. The facility consists of (4) client bedrooms, (2) bathroom, living room, kitchen, dining area, garage used for storage. The washer and dryer stored in the backyard. Facility maintains all required posting throughout the facility. All bedrooms were toured. Bedrooms 1-4 are occupied by residents and contain the mandated furniture. LPA observed all rooms to have the required furniture including a bed, nightstand, and chair(s). All beds had the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillow. LPA observed ample lighting in all the bedrooms. LPA Shirley and Rosemarie toured the kitchen and found it to be clean and sanitary. All appliances were in good working order. Knives were locked and stored. The medications were locked and stored in a cabinet and inaccessible to the resident. LPA observed a 3-day supply of perishable and a 7-day supply of nonperishable foods. The water temperature measured 111 degrees. Con'd 809-C The (2) bathrooms have grab bars and are clean and operational. First aid kit is fully stocked with manual. No firearms are stored at facility and no bodies of water present. This facility is in good repair. LPA Shirley and Rosemarie walked through all common areas. In the living room, kitchen, dining room there is ample seating and space for all residents. All rooms and walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. All rooms, hallway, and living room have working smoke detectors. There is a charged fire extinguisher in the dining area. The backyard is clean and clear of obstructions and hazards, shaded patio area and there are no bodies of water present.
