Type of visit: Required - 1 Year

On 09/17/2026, Licensing Program Analyst (LPA) Graham Gunby arrived to the facility unannounced to conduct a Required 1- year annual inspection. LPA was unable to open the CARE tool during this visit. LPA met with the Co-Administrator, Farah Cuccia, and explained the purpose of today's visit. The facility is licensed for six (6) non-ambulatory residents, hospice waiver of two (2). LPA was informed there are no residents receiving hospice services. During today's inspection, LPA and Co-Administrator conducted a tour of the interior and exterior of facility to ensure the health and safety of residents in care. Areas toured included but not limited to: residents bedrooms, bathroom, kitchen, backyard and the common areas. LPA observed toxins, medication and sharps to be locked and secured. LPA observed fire extinguishers to be last serviced on 04/09/2026. Water temperature was measured in the kitchen sink at 106.7 degrees Fahrenheit. LPA reviewed four (4) residents files and three (3) staff files. All resident and staff files contained the required signed documentation. Staff files contained current and up-to-date CPR/First Aid training. No deficiencies are being cited during today's visit. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the CARE tool. LPA met with Assistant Administrator and explained the purpose of the visit. Today's visit, LPA and Assistant Administrator conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. LPA observed facility to be at a comfortable temperature of 73*. LPA observed carbon monoxide and fire alarms to be present and operable. LPA observed bathrooms to be clean and sanitary with nonskid mat. LPA observed facility to have at least two days of perishables and seven days of nonperishable foods present. LPA observed sharps, toxins and medications to be properly locked and secured, inaccessible to residents in care. In areas toured, no immediate health safety and personal rights violations observed. LPA conducted file review of six residents records and two personnel records. LPA observed residents in the care to have updated LIC 602 with documentation of behavioral expressions. LPA observed personnel records to have completed annual training in file. File review of Emergency Disaster Plan to be recently reviewed and updated on June 27, 2025. File review of Facility Infection Control Plan to be recently reviewed and updated June 24, 2025. File review of fire drill was recently conducted on June 17, 2025. CARE tool completed with Assistant Administrator and found facility to be in substantial compliance. Type of visit: Required - 1 Year

On 8/22/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection. LPA met with caregiver and explained the purpose of the visit. Assistant Administrator arrived to the facility after tour was completed. Today's census is six residents with one on hospice services, facility is licensed for six non-ambulatory hospice waiver of two. LPA and Caregiver conducted a tour of the interior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to: four resident bedrooms, two bathrooms, laundry room, dining room kitchen and the common areas. LPA observed presence of fire extinguisher to be present with service date of April 29, 2024. LPA observed the facility to have the required poster posted in the hallway. LPA observed facility to have 2 days of perishables and 7+ days of non perishable foods present for residents in care. LPA observed sharps, toxins, and medications to be locked and inaccessible to residents. LPA observed five residents in the dining room and one resident watching television in the common areas. LPA conducted a full file review of facility's Medication Administration Record and facility's Centrally Stored Medication Record. Medication count was conducted with Assistant Administrator. File review conducted for six residents records. LPA completed the CARE inspection tool and no deficiencies cited.
