Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today at 08:00am. Upon arrival, the LPA was greeted by staff who then contacted the Administrator, Liana Tandretsyan and informed them of the visit. The Administrator arrived at approximately 08:20am and the reason for the visit was explained. Entrance interview conducted. Beginning at 08:22am, the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed: Kitchen: The LPA inspected the kitchen/food service area at approximately 08:23am. Knives and sharps were observed in a kitchen drawer locked and inaccessible at the time of the visit. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. At 08:25am, the hot water temperature was measured in the kitchen sink, and it measured at 107.4 degrees Fahrenheit. Common Areas: This includes the living room and dining room. Furniture in the common areas was observed to be in good condition at the time of the visit. The facility maintained a comfortable temperature. Report Continued on LIC 809C... Report Continued from LIC 809... The LPA observed an adequate amount of emergency food and water. Required postings were observed throughout the common space. There is a working telephone on premises. Cameras observed in the common areas. Activities were observed in the living room. The LPA observed a fire extinguisher to be fully charged with a purchase date of 03/09/2026. No hazards/obstruction observed inside or out. Laundry Room: Washer and dryer were observed locked and inaccessible at the time of the visit. Cleaning supplies and detergents are kept in a locked cabinet. Restrooms: There is one (1) restroom for residents’ use. Bathroom was clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathroom was sufficiently stocked with supplies and paper towels. Starting at 08:35am, the hot water temperature was measured in bathroom, and it measured within the required range of 105 – 120 degrees Fahrenheit. Bedrooms: There are three (3) bedrooms for residents’ use. All bedrooms are designated as shared / double occupancy. Bedrooms were observed to be furnished appropriately with appropriate furnishings, and sufficient lighting. Additional clean linens and towels were observed in a closet by the main hallway. Staff bedroom observed on premises locked and inaccessible at the time of the visit. Backyard / Outdoors: The backyard has a covered patio area with patio furniture for residents’ use. The LPA observed two (2) locked sheds used for storage purposes. There is one (1) side gate with latching mechanisms. All passageways were observed to be clear of any obstructions. No bodies of water noted at the time of the visit. Report Continued on LIC 809C... Report Continued from LIC 809C... Records: Record review began at approximately 08:40am Five (5) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, consent for treatment form, and current needs and services plan. All files were in order. Three (3) personnel files were reviewed for, but not limited to: personnel records, health assessments, criminal record clearances, first aid/CPR training, and the appropriate yearly training. All personnel files were complete. The Administrator’s Certificate is valid until 09/30/2026. Infection Control / Emergency Disaster Planning: During today’s visit, the LPA reviewed the facility's infection control plan. The facility’s policies and procedures as they pertain to infection control are adequate. The LPA also reviewed the facility's emergency disaster plan, which was observed to be complete and recently reviewed/updated. Emergency disaster drills conducted quarterly as per regulation; last fire drill conducted on 04/07/2026. Medications: Medications review began at approximately 10:25am. Medications are centrally stored and kept in a locked cabinet by the kitchen. All medications were labeled, stored, and locked inaccessible to residents in care. Medications appear to be administered as prescribed at the time of the visit. No citations issued. Exit interview conducted. Licensing Program Analyst (LPA) Sandra Urena arrived at the facility unannounced to conduct a required annual inspection. The LPA was greeted by staff and informed them of the reason for the visit. Administrator Liana Tandretsyan arrived shortly thereafter. The LPA and the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detector were tested and operational at the time of the visit. The fire extinguisher was fully charged and was last serviced 03/25/2025. The LPA observed required postings throughout the common space. KITCHEN: Knives and cleaning supplies are stored inaccessible. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. The hot water temperature measured at 113.8 degrees Fahrenheit. BEDROOMS: There are three residents’ bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. There are three designated client rooms and one staff room. There was a linen closet in the hallway with extra towels and linens. BATHROOMS: Bathrooms were clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathrooms were sufficiently stocked with soap and paper towels. The hot water temperature measured within the regulations. Continues on LIC 809C... OUTDOOR AREA: The backyard has a covered outdoor area equipped with furniture for client use. There are two side gates for client use and are single-latched. No bodies of water noted. The washer and dryer are inaccessible in a washer and dryer room. Cleaning supplies and disinfectants are kept in locked cabinets. RECORDS: Records review began at 11:45 a.m. Residents’ records were reviewed for, but not limited to care plans, medical records, admissions agreement, consent forms. All records were in order. Personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All files were in order. MEDICATIONS: Medications review began at 12:45 p.m.; medications are centrally stored and locked in a cabinet in the kitchen area next to the refrigerator; medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record. No errors observed during the medication review. Licensing Program Analyst (LPA) Emily Peraldi conducted a pre-licensing visit to the above noted facility. At 2:30 p.m., the LPA met with applicant, Liana Tandretsyan. This is a new facility. A dementia program was included in the plan of operation. A Hospice Waiver for six (6) has been requested. At 2:34 p.m., a physical plant tour was conducted inside and out. An approved fire clearance was received, clearing them for five (5) non-ambulatory residents and one (1) bedridden resident. The facility is one story. The facility has four (4) resident bedrooms. Resident bedroom #3 has a direct exit to the outside. Bedridden resident is allowed in resident bedroom #3. All resident rooms are set up with beds, nightstands, lamps, chairs, chest of drawers and closet space. The beds are furnished with box springs, comfortable mattress and clean linen; which includes, a mattress pad, top and bottom linens, pillowcases, blanket (if needed). Lighting in the rooms appeared adequate. The bedrooms were large enough to allow for easy passage between the beds and furniture with a wheelchair or walker. In addition, no bedroom was used as a passageway to another room, bath or toilet. There are no staff rooms – ‘awake night staff only’ - is required. All rooms were free of odors. All window screens were clean and maintained in good repair. There are three (3) bathrooms in the facility, and the bathroom located inside the garage is designated for staff. The resident bathrooms has a shower with non-skid materials/ mats. The toilet and shower have grab bars. The hot water temperature was tested in the bathroom and the kitchen and was found to be within the range of 105*F and 120*F. Resident and staff records will be stored in a cabinet which is currently located near the kitchen in the laundry room. Medications will be centrally stored in locked kitchen cabinets. The first aid supplies were complete, including a thermometer and a current version of a first aid manual. They were stored near the living area. Continued on LIC 809-C. Kitchen knives are stored in a locked kitchen drawer. The supply of dishes, utensils, pots, pans and drinkware is adequate. The freezer was maintained at zero degrees Fahrenheit (0*F) and the refrigerator was maintained at 40*F. The supply of nonperishable food is adequate. There are no pesticides (poisons) or toxins stored in any food storage area or preparation area. Appliances in the kitchen were clean and all appeared functional. Trash cans are located inside a kitchen cabinet. Laundry and house cleaning supplies are stored inside the locked laundry room. No flies or other vermin were observed. The common areas were appropriately furnished, and the lighting was adequate. There is a television and other entertainment equipment, games and/or activity supplies in the living room. There was sufficient space to accommodate both indoor and outdoor activities. Night lights were maintained in hallways and passageways to nonprivate bathrooms. All ramps were secure and non-slippery and were positioned at the level where wheelchairs and walkers may enter and exit the facility safely. Alarms on all exterior doors were engaged at the time of visit and functional. In addition, the physical plant is consistent with the submitted facility sketch/floor plan. The facility had emergency lighting, which included flashlights, or other battery powered lighting, and batteries. The facility had emergency food and water supplies. The facility has a furnace, which is able to heat rooms that residents occupy to a minimum of 68 degrees Fahrenheit; and, they have central air conditioning and are able to cool rooms to a comfortable range, not to exceed 85 degrees Fahrenheit. The facility smoke alarm system is hard wired. At 3:56 p.m., the smoke detector and carbon monoxide detectors were tested and functioned properly. The facility has a central entry point for symptom screening and temperature checks for staff and visitors. There is hand sanitizer available throughout the facility. There is a fire extinguisher near the kitchen area. It was fully charged and purchased on 02/03/2024. The supply of extra bed and bath linens is adequate. Personal hygiene items (shampoos, soaps) were adequate and are stored in the laundry room. Extra incontinence supplies will be stored in the laundry room and in the hallway. There is a functioning telephone on the premises. The emergency exiting plans/sketch are posted throughout the facility. The emergency telephone numbers are posted near the entrance of the facility. Other required postings are also posted near the entrance of the facility. Continued on LIC 809-C. The exterior passageways were clean and clear of any obstructions. There is a covered patio area at the back of the house with tables and chairs where residents can sit. The property is gated. There are gates with a self-latching mechanism at the side of the house designated for an emergency exit. There are two (2) locked storage shed in the back yard. There are no bodies of water on the premises at the present time. The garage is not accessible from the house; the garage door was locked. The garage will be used for storage and remain inaccessible to residents. Component III was conducted in conjunction with this pre-licensing visit. No corrections required. Exit interview conducted. A copy of the report was issued. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. COMP II by CAB successfully completed Method: Phone Call Applicant/administrator participated in COMP II at CAB telephone call with analyst at CAB. Identification of the applicant and administrator was verified by presenting photo ID via phone. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB.
