Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Dianna Manalo and explained the reason for the visit. Dianna Manalo's Administrator's Certificate expires on November 15, 2027. Facility is licensed for 6 non-ambulatory residents and a hospice waiver for 4. Facility is a single story home that has 5 bedrooms, 2 bathrooms, activity room with a TV, dining room, kitchen and an attached 2 car garage. LPA and Administrator toured the facility. The last emergency drill was conducted on April 14, 2026. Smoke detectors/carbon monoxide detectors tested operational. The fire extinguisher in the kitchen is fully charged. LPA inspected the first aid kit. LPA observed the first aid kit has all the required items LPA observed medications are stored locked in a cabinet in the dining room. LPA observed the kitchen is clean and organized. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed bedroom 2 is locked and unoccupied. The Administrator reported that bedroom 2 is being remodeled and and new flooring is being installed. LPA observed all resident rooms, except for bedroom 2, have the required furnishings and bed linens. LPA measured the hot water in the shared bathroom in the hallway. Hot water measured 105.8 degrees Fahrenheit. LPA observed the bathroom is clean and operational. The garage is kept locked. The garage is used for storage of supplies and furniture. LPA observed a 3 day emergency food and water supply stored in the garage. LPA and Administrator toured the backyard. No bodies of water observed. There is a shaded area with a table and chairs for residents to sit outside. Both exits are operational. LPA reviewed 5 resident files and medications. LPA reviewed 2 staff files. Both staff members are background cleared and associated to the facility. Both staff have CPR/First Aid training and the required annual training. LPA observed Staff 1 (S1) did not have a current health screening but had a valid TB test. Deficiencies are being cited per Title 22 division 6 of the California Code of Regulations. LPA met with Administrator Dianna Manalo and explained the reason for the visit. The Administrator's certificate expires November 15, 2025. The facility is a single story home that has 5 bedrooms and 2 bathrooms, activity room, dining room, kitchen and a 2 car garage. Facility is licensed for a capacity of 6 non-ambulatory residents and a hospice waiver for 4. LPA and Administrator toured the facility. LPA observed the see something say something poster (PUB 475) posted in the entry way of the facility. LPA observed that the resident bedrooms are clean and organized. LPA observed all the resident bedrooms had the required furnishings. . LPA observed both bathrooms are clean and operational. Hot water measured 111.9 degrees Fahrenheit in both bathrooms. Smoke detectors/carbon monoxide detectors tested operational. The last emergency drill was conducted on April 18, 2025. The fire extinguisher in the kitchen is fully charged. LPA observed the kitchen is clean and organized. LPA observed a two day perishable and seven day non-perishable food supply on hand in the kitchen. LPA observed the knives and sharp objects are kept locked in a kitchen drawer and the cleaning supplies are kept locked under the kitchen sink. The activity room has 4 reclining chairs and a sofa. There is also a TV in the activity room. Medications are kept locked in the cabinet in the dining room. The garage is inaccessible to residents and kept locked. The garage is used for storage of supplies and furniture. LPA observed a 3 day emergency food and water supply stored in the garage. LPA and the Administrator toured the backyard. Both exit gates are operational. No bodies of water observed. LPA reviewed 4 staff files. LPA observed that 1 out of 4 staff files (Staff 3) did not have 8 hours of the required Dementia training. No other discrepancies observed. LPA reviewed 6 resident files and medications, no discrepancies observed. LPA inspected the first aid kit. The first aid kit has all the required items. Deficiencies are being cited per Title 22 division 6 of the California Code of Regulations. The facility is a single story home that has 4 bedrooms and 2 bathrooms, living room, dining room, kitchen and an activity room and a 2 car garage. LPA and Administrator toured the facility. LPA observed the see something say something poster (PUB 475) posted in the entry way of the facility. LPA observed that the resident bedrooms are clean and organized. LPA observed all the resident bedrooms had the required furnishings. LPA observed that the living room is being used as a shared bedroom for two residents. The living room does have double doors leading outside to the front court yard. LPA observed both bathrooms are clean and operational. Hot water measured 110.0 degrees Fahrenheit. Smoke detectors/carbon monoxide detectors tested operational. The fire extinguisher in the kitchen is fully charged. LPA observed the kitchen is clean and organized. LPA observed a two day perishable and seven day non-perishable food supply on hand in the kitchen. LPA observed the knives and sharp objects are kept locked in a kitchen drawer and the cleaning supplies are kept locked under the kitchen sink. The activity room has 4 reclining chairs and a sofa. LPA observed that 1 out of 3 staff files (Staff 2) did not have a completed health screening. LPA observed that 2 out of 3 staff did not have 20 hours of the annual required training (Staff 2 and Staff 3). LPA reviewed 4 resident files, no discrepancies observed. LPA reviewed 4 resident medications, no discrepancies observed. LPA inspected the first aid kit. The first aid kit has all the required items. Deficiencies are being cited per Title 22 division 6 of the California Code of Regulations.
