Licensing Program Analyst (LPA) Jose Tan conducted an Annual Required visit and inspection of this facility today. LPA met with the Co-Administrator Daisy and explained the reason for the visit. At 9:18 AM, LPA with the assistance of the administrator, conducted a physical plant tour inside and out. Required postings were observed in the entry area. The smoke alarms are hardwired and inter connected. There is a carbon monoxide detector located in the hallway that functions properly. The fire extinguisher is located in the kitchen and last inspected on 07/15/26. The internal temperature is set at 75.0°F. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Bedrooms: There were five (5) bedrooms. Four (4) are designated for the residents' use. One (1) room is designated for staff use The resident bedrooms in use were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at a range of 114.3°F to 114.8°F LPA observed appropriate grab bars and non-skid mats. All trash cans in bathrooms had fitted lids to protect from cross contamination. Common Areas: These included the living room and dining area. The common areas were properly furnished. The auditory alarms on all exit doors were on and functional at the time of the visit. Passageways and exits were clear of obstruction. Dual smoke and carbon monoxide detectors were located throughout the facility, tested and observed to be operational. Outside areas: LPA toured the outside area of the facility and observed appropriate outdoor furniture, with a covered shaded area for residents. There is no body of water in the facility Laundry Room: Laundry machines are placed outside of the property. Disinfectants, laundry detergents and hygiene supplies were stored and inaccessible to residents in care. Extra linens were stored in a separated linen closet and readily available. Garage: There is no garage in the property, only two (2) car ports at the front. MEDICATION: LPA observed centrally stored medication and First Aid kit locked inside the closet located in the hallway next to bathroom #1 inaccessible to residents in care. LPA observed First-aid kit is complete and has new manual. PRN medications have written orders from a physician. Records: LPA reviewed records of four (4) residents and four (4) staff members. Resident and staff records appeared to be complete and updated. Disaster drill was last conducted on 06/1/26. Required posting observed in facility (complaint hot line poster). On 8/27/2025 Licensing Program Analyst (LPA) Perchui Milena Khurshudyan arrived at this facility to conduct a required Annual Inspection. Upon arrival LPA was greeted by the Administrator Estela Monderin, who granted access to the facility. LPA introduced herself by showing her badge and explained the reason for the visit. LPA Khurshudyan reviewed the required postings on the hallway wall next to the kitchen area. The inspection tool was used to complete the visit. At 9:45am LPA jointly with the Administrator began a physical plant tour of the facility and the following was observed: This is a single-story building with four (4) bedrooms, all rooms designated for residents’ use and one staff bedroom designated for staff use only. There are two (2) bathrooms, kitchen, common areas: living and dining rooms, and outdoor areas. Facility has an approved fire clearance for five (5) Non-ambulatory residents. The facility also has a Hospice waiver for five (5) residents. Common Areas: The facility maintains a comfortable temperature at 78°F. The living room and dining area appeared generally clean and were properly furnished. No obstructions and or tripping hazards throughout the facility were observed. Kitchen: At approximately 10:20am LPA toured the kitchen area and observed enough supplies of non-perishable for minimum 1 week and perishable food for 2 days at the facility. Sharps/knives were properly stored and locked inside the kitchen cabinet. There is a fire extinguisher on the kitchen countertop, which was last serviced on 7/10/2025. Chemicals observed to be locked under the sink and inaccessible to residents in care. Continue on LIC809-C Bathrooms: At 10:30am LPA observed all bathrooms are clean and in good repair. Properly supplied with toilet paper, soap and paper towels. The hot water temperature measured at 110.8°F. LPA observed appropriate grab bars and non-skid mats. All trash cans in bathrooms had fitted lids to protect from cross contamination. Smoke detectors/carbon monoxide. Dual smoke and carbon monoxide detectors were located throughout the facility, and at 11:35am they were tested and observed to be operational. Outside areas: At approximately 11:40am LPA toured the outside area of the facility and observed appropriate outdoor furniture, with a covered shaded area for residents. LPA discussed the importance of maintaining care and supervision to meet the needs of residents. There are no bodies of water. Laundry Room: Laundry machines are placed outside of the property. Disinfectants, laundry detergents and hygiene supplies were stored and inaccessible to residents in care. Extra linens were stored in a separated linen closet and readily available. Garage: There is no garage in the property. Between 10:35am to 12:00pm, LPA reviewed records of three (3) residents and four (4) staff members. Resident and staff records appeared to be complete and updated. MEDICATION: LPA observed centrally stored medication and First Aid kit locked inside the closet located in the hallway next to bathroom #1, and inaccessible to residents in care. LPA observed First-aid kit is complete and has new manual. Facility has a Dementia Care Program. PRN medications have written orders from a physician. The facility serves residents with dementia and the facility has trained staff to meet the needs of residents who are diagnosed with dementia. Facility has two (2) staff for AM shift and two (2) awake caregivers for PM shift, there is one (1) caregiver on call. Administrative: Annual fee is current. All required signs are posted. LPA collected LIC500, LIC9020, and Certificate of Liability Insurance renewed on 8/23/2025. The Administrator Certificate expires on 2/6/2026. LPA Interviewed three (3) caregivers and one (1) out of three (3) residents who was able to communicate. No citation issued during this visit. Exit interview conducted. An unannounced annual visit was conducted by Licensing Program Analyst (LPA) Perchui Milena Khurshudyan on 8/19/24 at 9:45 am. Upon arrival LPA met with Estela Monderin the facility Administrator, who granted access to the facility. LPA explained the reason for the visit. Shortly after the Co-Administrator , Tajdari Daisy Shirin arrived and helped with physical plant tour and staff/residents files. During today's visit, LPA conducted a physical plant walk through, at approximately 10:20am, to ensure that the facility is in compliance with rules and regulations under California Code of Regulations, Title 22. The following was observed: Facility is licensed for capacity of five (5) residents, of which five (5) may be Non-Ambulatory. Facility also has a hospice waiver for five (5) residents. There are four (4) bedrooms designated for residents’ use. Bedrooms are appropriately furnished and have appropriate lighting. There are two (2) bathrooms in the facility. LPA observed bathrooms have soap, paper towels and hand washing signs. The hot water temperature measured at 10:45 to be 106°F. Extra towels and linens were readily available. There are grab bars for each toilet and shower, bathrooms have non-skid mats. All trash cans in bathrooms had fitted lids to protect from cross contamination. The facility has alarms on all exit doors. Centrally stored medications are locked in the cabinets located in the hallway area. PRN medications have written orders from a physician. The facility serves residents with dementia. Potentially dangerous items are kept inaccessible to residents with dementia. There is staff to meet the needs of residents with dementia. Facility has 2 staff for AM shift and 1 awake caregiver for PM shift. Continue on LIC809-C LPA observed the kitchen area, there was sufficient stock of one-week non-perishable foods and two days of perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. LPA observed that sharp objects were stored in a locked drawer inaccessible to residents in care. The kitchen has a working gas stove, microwave, refrigerator, and freezer. Extra emergency food was properly stored inside the storage cabinet. The common areas which include dining and living room appeared clean and were properly furnished. Temperature was comfortable it was measured at 11:00am to be 74°F. The fireplace was closed and non-operational. No obstructions and or tripping hazards throughout the facility found. The Facility has three (3) fire extinguishers, LPA observed they are last purchased on 07/18/2024. Laundry room is located outside in the separate area. LPA observed all chemicals and detergents are kept locked and inaccessible to residents in care inside the locked cabinets next to the laundry area. Smoke detectors and carbon monoxide monitors were tested at 11:15am and observed to be functional. LPA observed a clean covered patio and backyard furniture to accommodate the five (5) residents. Exit areas are free of obstructions and hazards. LPA checked inside of the locked shed it was used for facility maintenance purposes. LPA discussed the importance of maintaining the care and supervision to meet the needs of residents. The facility does not have a swimming pool or body of water. There is no garage. Between 11:30am to 1:30pm, LPA reviewed records and files of five (5) residents and three (3) staff/caregivers. A review of staff and resident records appeared to be complete. Resident’s files contain a signed admission agreements and a medical assessment, and all other required documentarians. A review of staff records indicates that all facility staff and who required caregiver background checks have received criminal record clearances, LPA reviewed LIC501s, LIC503s, LIC9052s, and Training and Education certificates. There are no residents with prohibited conditions residing at the facility. Continue on LIC809C LPA observed the kitchen area, there was sufficient stock of one-week non-perishable foods and two days of perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Proper medication dispensing instruction are followed and checked for contamination. All medications are properly labeled and checked for expiration dates. First-aid has all proper items and is current. LPA collected LIC500, LIC9020, Copy of the Administrator certificate, The Infection Control. Facility is in compliance with Title 22 Regulations at this time. No citations issued during this visit.
