Type of visit: Required - 1 Year

On 3/2/26, Licensing Program Analysts (LPAs) Kyle Wellington and Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPAs phoned Licensee, Mirjana Bujosevic, who informed LPAs the facility is still unoccupied as the home's renovations continue. Licensee arrived during the visit to meet with LPAs. Licensee said the facility renovations are almost complete and there have not been any structural changes to the facility. Licensee stated she is unsure if the facility will reopen at this location or if it will move to another one. The facility is licensed to serve six (6) non-ambulatory elderly residents and has an approved hospice waiver for two (2) residents. LPAs toured the facility with Licensee and did not observe any residents in care or evidence to suggest the home is currently occupied. LPAs observed the facility has operating utilities along with a working refrigerator. The facility has the required furniture in all rooms, a charged fire extinguisher and operating smoke detectors. No bodies of water or pools were observed on the property. The facility's annual licensing fees have been paid. LPAs informed the Licensee to notify the Department when renovations have been completed and when they plan to admit new residents or if the Licensee plans on closing the facility and not accept new residents or if the Licensee wants to request a change in the facility's location. During today's visit, no deficiencies were issued. Type of visit: Required - 1 Year

On 2/18/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA phoned Licensee, Mirjana Bujosevic who reported the facility is currently unoccupied as the home's renovations continue. Licensee arrived during the visit to meet with LPA. Licensee reported they have not made any changes to the structure of the facility and all renovations have been superficial including new flooring/carpet, kitchen cabinets, and paint. The facility is licensed to serve six (6) non-ambulatory elderly residents and has an approved hospice waiver for two (2). LPA toured the facility with Licensee and did not observe any residents in care or evidence to suggest the home is currently occupied. LPA observed the facility has operating utilities along with a working refrigerator. The facility has the required furniture in all rooms and a charged fire extinguisher near the front entrance. No bodies of water were observed in the premises. The facility's annual licensing fees have been paid. Licensee estimated the renovations will be completed by the end of 2025. Licensee reported they will notify the Department when renovations have been completed and they plan to admit new residents. During today's visit, no deficiencies were issued. Mar 1, 2024Facility evaluation reportReport on file

Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Jacqueline Shaw Ross arrived at the facility unannounced to conduct a annual visit. LPA was met by Licensee, Mirjana Bujosevic and explained the purpose of the visit. Licensee informed LPA Shaw Ross that the facility does not have any residents in care due to extensive remodeling is still underway. LPA Shaw Ross toured the facility inside and outside and noted remodeling of flooring, kitchen and garage were still underway. LPA Shaw Ross observed required signs posted throughout the facility, appropriate furnishings still in place in all rooms, kitchen supplies, linen and toiletries. The backyard appeared well-kept with a newly built gazebo. Licensee will notify the Department when remodeling is completed and new residents are admitted. Annual Dues were paid on 2/24/2024. No deficiencies were cited on today's visit.
