On 10/16/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Licensees, Tessie and Eric Melad who were informed of the purpose of the visit. The facility has a fire clearance to serve six (6) non-ambulatory elderly residents. The facility also has an approved hospice waiver for two (2) and LPA was informed none of the current residents are on hospice. During today's visit, there was two (2) staff and two (2) residents present. LPA toured the facility with licensee and observed the facility is made up of a one-story home with three (3) resident bedrooms, two (2) bathrooms, a kitchen, dining area, living room, laundry room, staff room, and attached garage. Resident bedrooms had the required bedding, furniture, and lighting. Bathrooms had grab bars and non-skid mats in the showers. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility met Departmental requirements for a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked kitchen cabinet. Licensee tested one (1) of the smoke alarms/carbon monoxide detectors and LPA found it to be operational. LPA also observed a charged fire extinguisher mounted in the kitchen. The facility's certificate of liability insurance expires on 07/01/2026. Resident files reviewed had Departmental required records. LPA reviewed the facility's Certificate of Completion noting their last earthquake drill was conducted on 07/07/2025. Long Term Care Ombudsman's contact information, residents' personal rights, complaint procedures, emergency disaster plan, and facility sketch are visibly posted near the front entrance. During today's visit, LPA did not observe any issues or concerns. Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced annual required visit. LPA was granted entry and met with Administrator Eric Melad who was informed of the purpose of today's visit. At the time of the visit there was two (2) staff and two (2) residents present. LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted interviews. LPA observed the following: Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. LPA observed outdoor furniture and shaded area for clients. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. LPA reviewed staff files and training. Records review for Staff One (S1) and Staff Two (S2) revealed S1 and S2 have not conducted the required twenty (20) hours of annual training. A deficiency will be issued under Health and Safety Code 1569.625(b)(2) along with a plan of correction. Records review revealed S1 and S2 do not have a current CPR and first aide training on file. A deficiency will be issued under Health and Safety Code 1569.618(c)(3) along with a plan of correction. Two resident files were reviewed and possessed all required paperwork which included Admissions Agreement and updated Physician's Report. The listed administrator does not have a current and/or pending administrator certificate with the last administrator certificate expiring in 2023. A deficiency will be issued under Title 22 Regulation 87405(a) along with a plan of correction. Resident medication was centrally stored and locked in a closet located in a kitchen cabinet. LPA observed Resident One (R1) medication that require refrigeration stored in the kitchen's refrigerator with the facility's perishable foods and was accessible to residents in care. A deficiency will be issued under Title 22 Regulation 87465(h)(1)(A) along with a plan of correction. LPA reviewed medications prescribed to two (2) residents and found R1's PRN medication had been administered without the facility staff documenting the date and time the PRN medication was given. A deficiency will be issued under Title 22 Regulation 87465(d)(3) along with a plan of correction. LPA reviewed the facility's emergency and disaster plan and infection control plan. Records review and interview with S1 revealed facility has not conducted the required quarterly fire drill this year. A deficiency will be issued under Health and Safety Code 1569.695(c) along with a plan of correction. LPA observed emergency supplies in the garage, a charged fire extinguisher, and first aid kit with all required items. Licensing Program Analyst (LPA) Jacqueline Shaw Ross made an unannounced visit to the facility for the purpose of an annual review. LPA was greeted by Administrator, Eric Melad and explained the purpose of the visit. A tour of the facility was conducted inside and out. At the time of visit, all three residents were home and one additional staff was present. The facility is approved for six (6) elderly residents ages 60 and over all of whom may be non-ambulatory. There is a hospice waiver for two (2) residents. The facility is a four (4) bedroom two (2) bathroom one story home. Each residents have their own bedroom. The fourth bedroom is reserved for staff. During the tour the following was observed: Resident bedrooms had the required furnishings and were observed to be in good condition. Bathrooms had required signage, hand rails, non-slip mats. Night-lights were observed in the hallways. Fixtures and furniture for an operational facility are present and in good repair. All passageways were free of obstructions, charged fire extinguishers and the fire alarm system was operable, medications are kept centralized and locked, hazardous items are kept inaccessible residents. Hot water was tested at 134 degrees Fahrenheit which is above the maximum temperature of 120 degrees Fahrenheit. Administrator adjusted the hot water temperature and will contine to monitor the temperature, adjusting it accordingly. Backyard area is free from obstructions. Kitchen/Food Service: LPA observed the entire kitchen, food is stored properly and dishes are clean and in good condition. There is a sufficient supply of perishable and non-perishable foods. Area was observed to be clean and functional. Care & Supervision: Facility has sufficient care staff employed. Administration: Emergency exiting plans, telephone numbers and Ombudsman information and other required signage are posted throughout the facility. Drills are conducted regularly. The last drill was 3/20/2023. Record Review and Client/Staff Files: LPA reviewed current staff and all staff have has Criminal Background Clearance, current CPR/First Aid certification, and trainings are current. The Administrator's certificate expires 10/21/2023. Resident records were reviewed and the following deficiency was observed: -File review for R1 did not contain a current Physician's Report. The last Physician report was completed in 2020. LPA informed Administrator Physician's reports must be updated annually. Citation issued. Physician reports for all other residents are current. All other documents in residents files were up to date. Medication Review: LPA reviewed medication and medication log. Residents' medications are being dispensed according to physician's orders.
