Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced, required Annual Inspection. The facility file and personnel report was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Caregivers Sheila and Kenneth Amoranto. LPA spoke with another caregiver over the phone, Kevin Amoranto, who arrived later during the visit. The facility's license shows a maximum capacity of six (6) non-ambulatory residents, all of which may be bedridden. Additionally, the facility is approved for six (6) hospice waivers. During today’s inspection there were six (6) residents in care, with one (1) currently receiving hospice services. LPA and Caregiver Kevin Amoranto toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Hot water temperature at taps accessible to clients were all compliant: one common bathroom sink was 106F. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility contained at least two (2) days of perishable food, and at least seven (7) days non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. Knives were locked and inaccessible to residents in care. [Continued on LIC 809-C] [Continued from LIC 809] No toxic chemicals or poisons were accessible to clients. Medications were labeled, as required, and centrally stored in a locked area. However, as staff opened another, unlocked, drawer to retrieve the key for the medication cabinet, LPA observed a tray with cups of pre-poured medications. Per inquiry with staff, the medications in the cups were pre-poured for dinner time administration. Staff immediately moved the medications to the locked medication cabinet and LPA reviewed regulations on medication storage. One Type A citation was issued for the accessible medications. No pools or bodies of water exist on the premises. Per Caregiver Kevin, no firearms or ammunition are kept at the facility. Smoke and carbon monoxide detectors, emergency lighting, and facility telephone were all in working order. Fire extinguishers were serviced within the last 12 months, dated for February 2026. Per file review, last emergency drill was conducted in 2024. A Type B citation was issued for not conducting emergency drills as required per California Health and Safety Code. First aid kits were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA interviewed one (1) staff and one (1) client, and interviews did not reveal any licensing or regulatory concerns. LPA reviewed facility records. The files reviewed by LPA contained required documents, aside from signed copies of resident personal rights (LIC 613) to be retained in resident files. A Technical Violation (TV) was issued. Confidential records were stored in locked areas. Two (2) deficiencies were cited during the inspection and one (1) Technical Violation (TV). Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Nacole Patterson conducted an unannounced Case Management Visit. LPA was greeted by and met with caregiver Kevin Amoranto to discuss the purpose of the visit. Today's visit is in response to the self reported incident of Resident 1 (R1), who walked out of the facility in the middle of the night and was found on facility property. Staff interviews revealed that R1 did not leave the front yard of the facility and experienced an episode of confusion. R1 suffered minor abrasions and received medical care. Due to R1 experiencing a change in condition based on a mild cognitive impairment, consultation was provided to caregivers and Licensee regarding increased supervision and protocols to put in place to accommodate R1's new behavior of exit-seeking at night. Licensee will remain in contact with R1's Responsible Party and medical professional regarding R1's changes in condition as well as an updated care plan to accommodate R1's new behaviors. LPA conducted a wellness check at the facility; no health or safety issues were identified. No deficiencies were cited or observed on this date. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Rebecca Ruiz conducted an unannounced Case Management - Annual Continuation visit for a Required 1-Year inspection that was started on 2/29/2024. LPA was greeted by, identified herself to, and explained the purpose of the visit with Licensee Lady Catherine Amoranto. The facility is licensed for a maximum capacity of 6 residents, 6 of which may be bedridden. The facility has a waiver for 6 hospice residents. During today’s visit, the facility had a census of 6 residents, 1 of which was bedridden and 2 of which were on hospice. LPA did not observe any aspects of delayed egress or secured perimeter. The Administrator for the facility is Lady Catherine Amoranto and their certificate was valid and current. During the visit on 2/29/2024, LPA toured the facility and inspected each room of the facility, including resident and staff rooms, bathrooms for resident and staff use, kitchen, garage, common areas, and outside space. No bodies of water were observed on the premises. According to Catherine Amoranto, no firearms or weapons are stored on the premises. The facility was found to be clean, safe, and in good repair with no pathway obstructions. During today's visit, the facility’s water temperature was measured at 112.6 degrees Fahrenheit and 111.0 degrees Fahrenheit in two common bathrooms. The facility’s internal temperature was measured at 73 degrees Fahrenheit. LPA observed locked storage for all hazardous and/or toxic chemicals and were stored separately from food supplies. LPA also observed locked storage for resident medications and resident and staff files. Resident medications are stored in their original container and label. LPA observed a 2-day supply of perishable food and a 7-day supply of non-perishable food present at the facility. The facility refrigerator was kept at 40 degrees Fahrenheit, and the facility freezer was kept at 0 degrees Fahrenheit. LPA observed linens and hygiene products provided to the clients that are in good repair and sufficient to meet their needs. Continued on LIC809-C page… Staff present at the facility during the time of the inspection had a criminal background clearance, were associated to the facility, and had a first aid certificate. LPA reviewed multiple resident and staff records. Each resident record was complete and contained a signed admission agreement, updated physician’s report and medical assessment, documents regarding safeguarding personal property, and personal rights. Each staff file was complete and contained a personnel record, first aid certificate, fingerprint clearance and association, and a health screening. LPA spoke with staff and residents present at the facility during the time of the inspection and those interviews did not reveal any licensing or regulatory concerns. The Licensee will submit copies of the LIC500 Personnel Report, LIC610E Disaster Plan, and current liability insurance to the Department within 15 business days. No deficiencies were cited on today’s date. During today's visit, LPA briefly toured the facility and reviewed staff and resident records.
