Allegation investigated: Staff did not treat residents with dignity Staff did not assist resident with toileting needs

Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Leonor Carmona, Administrator LPA previously conducted interviews with residents, staff, and outside sources, made observations, and obtained and reviewed pertinent records. LPA Ruiz conducted the initial visit on June 2, 2022 and conducted a tour of the facility. It was alleged that the staff did not treat residents with dignity and the staff did not assist resident with toileting needs. Interviews with the staff revealed they treat the residents with respect and treat them like family. Interviews revealed that the residents that needed assistance with toielting would get receive the assistance. Interviews revealed that the residents that were there in 2022 are no longer at the facility due to passing away or moving out. Interviews with staff revealed that they denied making residents use the restroom in their rooms. Interviews with staff revealed them denying not treating the residents with dignity. The investigation did not produce supporting evidence to prove the staff did not treat residents with dignity and staff did not assist resident with toileting needs. The complaint allegations are unsubstantiated. Licensing Program Analyst (LPA) Janet Ngallo conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was allowed entry and discussed the purpose of the visit with Administrator Leonor Carmona. According to the facility’s license, the facility has a maximum capacity of six (6)residents. Four (4) of whom may be non-ambulatory. Hospice waiver approved for two (2) residents. LPA, accompanied by Administrator toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. Hot water temperature was compliant. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no toxic chemicals/poisons accessible to clients. Medications were labeled, as required, and stored in locked areas. Their are no bodies of water on the premises. Per licensee, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher(s) were present. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. Licensing Program Analysts (LPA) Ramin Hashemi and Nacole Patterson conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPAs were welcomed by and discussed the purpose of the visit to Licensee Leonor Carmona. The facility's license shows a maximum capacity of six (6) residents of which four (4) are non-ambulatory residents, ages 60 and over. The facility has an approved hospice waiver for two (2). During today’s inspection there were 5 residents in care. LPAs and Licensee Loenor Carmona toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility contained at least 2 days of perishable food, and at least 7 days non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. No toxic chemicals or poisons were accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water exist on the premises. Per Licensee Leonor Carmona, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and facility telephone were all in working order. Fire extinguisher(s) were serviced within the last 12 months. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPAs interviewed staff and clients, and reviewed facility records. The files reviewed by LPAs contained required documents. Confidential records were stored in locked areas. Deficiencies were cited per California Code of Regulations, Title 22 (refer to the attached LIC809-D). A plan of correction was jointly developed with the licensee.
