Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct the annual inspection. Upon arrival LPA met with caregiver, and spoke to Administrator Elomense Aburekhanlen over the phone. Currently there are 5 residents residing at the facility. LPA toured facility with administrator to ensure health and safety of residents in care. LPA toured 5 resident rooms, 1 staff room, 2 bathrooms, kitchen, common living spaces, backyard and the garage area. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA toured the backyard and all exits are accessible and unlocked. There is a locked storage for medications and toxins. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA observed an adequate amount of linens and found the first aid kit to be complete. During today's inspection Administrator was out of town and LPA was unable to review facility files. LPA will return on a later date to complete annual inspection. Deficiencies cited on 809-D. Exit interview conducted and copy of reports provided. Allegation investigated: Staff withheld resident's mail. Facility staff forced care on resident.
Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to deliver complaint investigation. LPA spoke to administrator on the phone during today’s inspection. LPA investigated allegation, “Facility staff forced care on resident”. Reporting party indicated care staff are forcing R1 to shower and forced R1 to take the flu shot. LPA interviewed administrator in which she stated resident has only lived at the facility for 1 week. Administrator stated no flu shot was provided at the facility and that she encouraged resident to shower but did not force care on him. LPA interviewed R1 in which they stated the hospital staff encouraged him to take the flu shot but he consented. R1 stated facility staff encouraged him to take a shower but did not force him. R1 stated he has memory impairment and resident gets facilities mixed up. Due to the information gathered LPA finds allegation to be UNFOUNDED. Continuation on 9099-C. Unfounded LPA investigated allegation, “Staff withheld resident’s mail”. Reporting party indicated that facility took R1’s mail from him. LPA interviewed administrator in which she stated R1 moved into the facility 1 week ago. Administrator stated R1 has received 1 piece of mail since moving in and staff gave resident that mail. LPA interviewed R1 in which he stated he is waiting to receive financial documents in the mail. R1 stated he has memory impairment and gets past facilities mixed up. R1 stated past facilities have stolen from him but the current facility has not taken anything. Due to the information gathered LPA finds allegation to be UNFOUNDED. Type of visit: Prelicensing
Licensing Program Analyst (LPA) Cassandra Mikkelson arrived at the facility and met with Administrator to conduct a secondary Pre- Licensing visit. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. Hot water temperature was observed to be 113.3 degrees F. LPA observed smoke detectors and carbon monoxide detectors to be operational in the care home. Component III was completed on March 11, 2025. Application is pending and LPA will forward findings to the Centralized Application Bureau (CAB) for final review and approval. CAB will further contact applicant on final status of application. Type of visit: Prelicensing
Licensing Program Analyst (LPA) Cassandra Mikkelson and Cheyenne Ratajczak arrived at the facility and met with Administrator, to conduct a Pre- Licensing visit. The facility has a fire clearance for four (4) non-ambulatory residents and two (2) ambulatory residents in bedroom #1. Administrator has an active certificate (#6071118740 with expiration date 06/06/2026). LPAs conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are four (4) bedrooms and two (2) bathrooms for resident use. Bathrooms were in sanitary condition and properly maintained. LPAs observed facility has the ability to prepare and store food, to lock away cleaning products and other toxins, and lock medications to make inaccessible to residents. First aid kit is maintained and ready for emergency use. LPAs observed smoke alarm over bedroom 1 is not functioning properly and hot water temperature was 97.1 degree F. LPAs informed Applicant another pre-licensing inspection will be conducted Thursday March 13, 2025. Component III was completed. A copy of this report was provided to the facility. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
