Type of visit: Required - 1 Year

On 5/18/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection visit. LPA arrived, stated purpose of visit, and allowed entrance by Direct Care Staff. Licensee, Maria Emma Pangilinan contacted by telephone and arrived a short time later to conduct visit with LPA. LPA observed facility to be clean, odor free and a comfortable temperature. Common areas observed to well furnished and have seating available for all residents in care. Resident bedrooms toured and observed to have required furnishings. Resident bathrooms observed to have grab bars, shower chairs, and slip resistant mats observed. Hot water measured at 108 degrees F. Kitchen toured, facility observed to have a 7-day supply of non-perishable and 2-day supply of perishable food. Medications observed to be locked and secured. Centrally stored medication logs, MARS and resident medication reviewed. Medication observed to have original labels and to be administered as prescribed. Fire extinguisher present with a service date of 2/05/2026. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Outside of facility toured. Facility has a seating area with patio umbrella for residents. Side gate was observed to be self-latching. No hazards observed. LPA reviewed staff and client records. LPA requested the following documents be submitted to Fresno Regional Office no later than Friday, 5/29/2026: Personnel Report (LIC500), and Register of Facility Clients/Residents (LIC 9020). No deficiencies observed during inspection. Exit interview conducted. On 04/11/2024, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection visit. LPA arrived, stated purpose of visit, and allowed entrance by Licensee, Maria Emma Pangilinan. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free, well lit, and a comfortable temperature. Common areas observed to well furnished and have seating available for all residents in care. Resident bedrooms toured and observed to have required furnishings. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Hot water measured at 109 degrees F. Kitchen toured, appeared clean, observed a 7-day supply of non-perishable and 2-day supply of perishable food. Medications observed to be locked and secured. All medications observed to have have original labels and to be administered as prescribed. Fire extinguisher serviced on 07/26/24, Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. First Aid kit observed to have required items. Outside of facility toured. Facility has a seating area with patio umbrella for residents. Side gate was observed to be self-latching. No hazards observed. LPA reviewed staff and client records. Exit interview conducted. A copy of this report provided to Administrator, Maria Pangilinan for facility records. LPA received the following documents during inspection visit: Administrator Certificate, Liability Insurance, Personnel Report (LIC500), and Register of Facility Clients/Residents (LIC 9020). Type of visit: Required - 1 Year

On 04/11/2024, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA was granted entry to the facility by facility staff. Facility staff contacted Administrator via telephone. Administrator arrived a short time later, LPA met with Administrator, Maria Pangilinan. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Resident rooms appeared clean and had required furnishings. LPA observed a long, wooden stick on the track of the sliding glass door in the shared bedroom. The stick prevented the door from being opened. LPA observed an adequate supply of linen. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Hot water measured at 128.8 degrees F. Kitchen toured, appeared clean, observed a 7-day supply of non-perishable and 2-day supply of perishable food. Exterior tour conducted. Side gate was observed to be self-latching. Fire extinguisher serviced on 07/17/2023. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Last fire drill conducted on 01/26/2024. LPA reviewed staff and client records. LPA found that R5 did not have current hospice care plan that included facility caregiver responsibilities. Medications reviewed. LPA found that the facility has been administering medication and medication supplements without authorization from a physician to 3 out of 5 residents in care. First Aid Kit observed. Deficiencies are being cited in accordance to California Code of Regulations, Title 22, Division 6 on the attached 809D. Exit interview conducted and a plan of correction was reviewed and developed. A copy of this report and appeal rights were discussed and provided to Administrator, Maria Pangilinan, whose signature on this form confirms receipt of this document. LPA is requesting the following documents be submitted to the Fresno CCL office by 04/25/2024: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A), Surety Bondthe state’s words, verbatim · CDSS document, Apr 11, 2024

Type of visit: Collateral

On 1/12/24, Licensing Program Analyst (LPA) M. Medina arrived at the facility to complete the required 10-day complaint visit and observed the following during facility tour and review of facility records: Staff 1 (S1) and Staff (S2) are fingerprint cleared and not associated to facility but were observed working during facility visit. Resident rooms for R2 and R3 contained half bed rails, Resident room for R4 contained full bed rails. Residents in these rooms are not on hospice or do not have a physician order with an exception from the department. During interview with Licensee for complaint, facility was not able to show any record of incident report being sent to department for a fall on 12/24/23 for R1. Based on today’s visit, deficiencies are being cited in accordance with Title 22 on the attached LIC 809D for the date of 1/12/2024. An immediate civil penalty is being assessed.
