Type of visit: Required - 1 Year
On 06/13/2026,, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required visit. LPA introduced self, stated the purpose of the visit, and allowed entrance by direct care staff. Licensee, Olivia Roura was contacted by telephone and arrived a short time later to conduct facility visit. Currently, there are 5 residents in care. All were present during inspection and observed to be relaxing in the family room watching television. The facility was observed to be at a comfortable temperature, clean, and free of odor. LPA observed adequate seating available for all residents in common area. Resident bedrooms toured, LPA observed resident's bedroom to have required furnishing available. Bathrooms observed to have grab bars, shower chairs, and slip resistant mats available. Bathroom fixtures observes operational, hot water temperature was tested at 109 degrees F. Kitchen toured, LPA observed the sharps and knives to be locked and secured in kitchen drawer. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. Medications are locked and secured in hallway cabinet. Medications reviewed observed to have original labels and to be administered as prescribed. Fire extinguisher present with a service date of 4/30/2026, carbon monoxide and smoke detectors tested and observed operational at time of inspections. Outside of facility toured. Side gate was self-closing and self-latching. Adequate outdoor seatings observed available for residents. All exits observed to open freely and are free of obstruction. LPA reviewed resident and staff files during inspection. LPA received a copy of facility liability insurance and current copy of administrator certificate during facility visit. LPA requested the following documents to be submitted to Fresno Regional office no later than 7/03/2026: LIC 500, LIC 9020 and current copy of lease agreement. On 6/11/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, stated purpose of visit, and allowed entrance by Administrator, Rodelio Roura. Licensee/Administrator Olivia Roura contacted by telephone and arrived a short time later to conduct visit. Facility tour conducted with Administrator. Facility observed to be well lit, odor free, and comfortable temperature. LPA observed residents engaging in activities at the dining room table at time of inspection visit. Facility living room and dining room observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. Resident bathrooms toured, water temperature measured at 107 degrees F. Bathrooms observed to have grab bars in the shower and near toilet. Showers have shower chair and skid resistant mat available. LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Medications are centrally stored and locked in hallway cabinet. All medications observed to have original labels and be administered as prescribed. Outside of facility toured. All exits open free of obstruction. All exits open free of obstruction. No hazards observed. Facility has seating and a shaded area for residents. Fire extinguisher present with a service date of 3/25/2025. Smoke and Carbon Monoxide detectors present and observed operational. Last Fire drill conducted 4/11/2025 according to facility records. LPA reviewed resident and staff files. On 8/29/24, Licensing Program Analyst (LPA) M. Medina conducted an unannounced subsequent complaint visit to deliver findings. LPA introduced self and stated purpose of visit and allowed entrance by caregiver. Administrator contacted by telephone and arrived a short time later to meet with LPA regarding complaint findings. During course of the investigation, LPA toured facility, reviewed records, and conducted interviews. This department had insufficient information regarding the allegations listed above. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove or disprove that the allegations occurred therefore the allegations are UNSUBSTANTIATED. No deficiencies issued during this complaint visit . Exit interview conducted. Administrator, Olivia Roura contacted by telephone and arrived a short time later to conduct visit. Facility tour conducted. Facility observed to be clean, odor free, comfortable temperature, and well lit. Facility living room, dining room, and family all observed to have adequate seating available. Resident bedrooms toured, all bedrooms observed to have all required furnishings. Resident bathrooms toured, water temperature measured at 108 degrees F. Bathrooms observed to have grab bars in the shower and near toilet. Shower also has shower chair and non-skid mat available. LPA observed kitchen to have all sharp locked and secured in kitchen drawers. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. LPA observed medications to be centrally stored and locked. All medications observed to have original labels and be administered as prescribed. Outside of facility toured. All exits open free of obstruction. No hazards observed. Fire extinguisher present with a service date of 04/04/2024. Smoke and Carbon Monoxide detectors present and observed operational. Last Fire drill conducted 5/06/2024 according to facility records. LPA reviewed resident and staff files. LPA requested the following forms to be submitted to Fresno Regional Office no later than 6/28/24: Personnel Report (LIC 500), Client Roster (LIC 9020), and current liability insurance.
