Type of visit: Required - 1 Year
On 7/22/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced self, stated purpose of visit, and allowed entrance by caregiver. Christina Marsy, Administrator contacted by telephone and arrived a short time later to conduct visit with LPA. There are currently six (6) residents in placement, residents observed to be relaxing in the living room watching television and reading the newspaper, LPA observed facility to be clean, odor free, and temperature to be 75 degrees. Facility observed to be equipped with auditory alarms on exit doors. Resident rooms toured, and observed to have all required furnishings. All common areas observed to have adequate seating available for residents. Bathrooms observed to have grab bars, shower chairs, and non-skid mats available. Fixtures in the bathroom observed to be functional. Water temperature measured at 114 degrees F. Kitchen toured, facility has adequate food supply for the residents in care. Stove knobs observed to be off and secured. Knives observed to be locked and secured and inaccessible to residents. Medications observed to be locked and secured. Medications observed to have original labels, and to be administered as prescribed. LPA observed smoke detectors and carbon monoxide detector to be operational during facility inspection. Last fire drill conducted in July 2026 according to facility records. Fire Extinguisher present with a service date on 11/03/2025. Outside of facility has shaded area and seating available for residents. No hazards observed. Facility to submit the following documents to Fresno Regional Office no later than 7/31/2026: Register of Facility Clients/Residents and a copy of Liability Insurance. On 3/10/2026, Licensing Program Manager (LPM) S. Pidgirny and Licensing Program Analyst (LPA) M. Medina conducted an office meeting to deliver findings for this complaint. LPM and LPA met with Licensee, Valerie Civelli and Administrator, Christina Marsy. During the investigation, LPA conducted interviews, gathered information and toured facility. Based on information gathered, Licensee Valerie Civelli transferred the facility or facility property, operations, operational controls and financial obligations to prospective buyer as a part of agreement on/or after March 21, 2025. Licensee entered into a formal written agreement on March 25, 2025, then entered into a formal written contract on July 18, 2025. The facility was not licensed by Department until August 7, 2025. The preponderance of evidence standard has been met; therefore, the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 6, Chapter 8, deficiencies are being cited on the attached 9099-D. Currently, there are 5 residents in care, of those 4 are currently receiving hospice services. LPA observed facility to be clean and a comfortable temperature during visit. Facility has a 2-day supply of perishable food and a 7-day supply of non-perishable food. The following documents were provided to LPA during facility visit by Administrator: LIC 500 LIC 9020 Liability Insurance Copy of message sent to families on 11/17/2025 Per previous telephone conversation with Valerie Civelli, a spreadsheet will be provided via e-mail by close of business on 12/05/2025 which documents all utilities and account holder for the facility. Type of visit: Prelicensing
On 7/17/2025, Licensing Program Analyst (LPA) M. Medina conducted an announced Pre-licensing inspection. LPA introduced self, stated the purpose of the visit, and was allowed entrance into the facility. LPA met with Administrator, Christian Marsy and Licensee Representative, Antoinette Sillas. . The facility is 6 bedrooms and 3 bathrooms home. Fire clearance was granted for 6 residents, 5 non- ambulatory and up to 1 bedridden residents. Currently, there are 5 residents in care. Facility observed to clean, odor free, and a comfortable temperature. Common areas observed to be furnished and have adequate seating and lighting available. All bedrooms were observed to have required furnishings. Bathrooms toured, fixtures observed operational. Grab bars available and skid-resistant surfaces in showers observed. Kitchen was toured and observed to have pots, pans, plates, cups and utensils. Facility has a 2-day of perishable and a 7-day supply of non-perishable food available as of date of inspection. Knives observed to be locked and secured and stove knobs observed to be removed. Medication observed to be locked and secured in medication room. Cleaning supplies and chemicals are locked and secured under kitchen sink and in laundry room. Fire extinguisher present with a service date of 9/18/2024. Smoke detectors and carbon monoxide detectors were observed to be operational during this inspection. Water temperature measured at 117 degrees F. Facility has a working telephone of (661) 615-3054. . Outside of facility toured. Facility has a covered patio with seating available. Exits open free of obstruction. . Gate observed to be self-latching. Component III conducted during pre-licensing inspection. Pre-Licensing is complete and this facility has no deficiencies Exit interview conducted.
