Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an annual required visit and inspection of the facility. LPA met with Caregiver and gained entry. The reason for this visit was explained. Caregiver contacted Administrator via telephone and joined the visit. At approximately 1:45p.m., LPA and Administrator conducted a physical plant tour. LPA observed required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights located alongside the entrance hallway leading towards the kitchen. The facility is a single-story building with three (3) bedrooms and two (2) bathrooms currently occupying six (6) residents. The facility has an approved fire clearance for six (6) non-ambulatory residents of which one (1) may be bedridden. Dual Smoke and Carbon Monoxide detectors were tested and function properly. They are in each bedroom and hallway. Facility Emergency Fire/Earthquake Drills were conducted on 03/02/2026. The fire extinguisher is in the dining room with purchase date of 02/14/2026. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Common areas: The living room and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 74°F. A fireplace was observed to be covered and inaccessible to residents. A working telephone was observed. Kitchen: The kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrapped, Cont. on LIC 809-C Cont. from LIC 809 dated, and stored properly as well. Knives were stored in a locked cabinet in the kitchen. Food storage and preparation areas are clean and inaccessible to pests. Garbage cans have tight fitting covers. Cleaning supplies, pesticides or toxic cleaning supplies were stored and locked away in the kitchen cabinet. Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lit appropriately. Extra linens/covers were observed stored in storage closet located near the entrance. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations at 118.1 °F. LPA observed there to be appropriate slip-resistant strips and grab-rails within the bathrooms, all in good repair. Washcloths are not shared. Laundry Service: There are enough linen in hallway closet available to change weekly or more if needed. Cleaning supplies are being stored in a locked cabinet in the attached garage. The garage is also used for PPE and extra food storage. First Aid Kit and Book: First aid kits have been inspected and have the following: thermometer, tweezers, scissors, antiseptic, bandages, gauze, first aid manual are updated and it's available for staff use but inaccessible for residents. Surrounding Grounds: Entry and exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was shaded and free of hazards. No bodies of water. Due to time constraints, LPA had to terminate the visit and will return on a later date to complete the Required - 1 Year inspection by reviewing medication, staff files, resident record, interviewing residents and complete the Compliance & Regulatory Enforcement (CARE) tools. No immediate health and safety hazard is noted during this visit. An exit interview was conducted. On 6/02/2025 at approximately 9:30 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the Administrator Antuan Nazaryan and stated the reason for their visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 11:40 AM and the following was noted: The facility is a single-story building with three (3) bedrooms and two (2) bathrooms currently occupying five (5) residents. There is no designated staff room. The facility has an approved fire clearance for six (6) non-ambulatory residents of which one (1) may be bedridden. Sign in sheet, hand sanitizer, gloves and masks are available upon entrance. Common areas: The living room and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 78°F. LPA observed a fire extinguisher to be located in the kitchen and dated 07/12/2024. LPA observed required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights located alongside the entrance leading towards the kitchen. A fireplace was observed to be covered and inaccessible to residents. A working telephone was observed. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were observed. Knives/sharps were observed to be kept in a locked kitchen drawer. The cleaning solutions/toxins were observed to be kept locked in a kitchen cabinet. Kitchen appliances were observed to be working and in proper condition. (Continue to LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed stored in storage closet located near the entrance. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations at 120.0 °F. LPA observed there to be appropriate slip-resistant strips and grab-rails within the bathrooms, all in good repair. Garage: The garage cannot be accessed from inside of the facility and is located towards the front of the facility. LPA observed the garage to be used for storage purposes and laundry. Laundry Room: The laundry room was observed to be located within the garage. LPA observed cleaning solutions and toxins stored in a locked storage cabinet and inaccessible to residents. The laundry appliances were observed to be working and in proper condition. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. There is no body of water located at the facility. Medications: The medications were observed stored in a locked cabinet located aside the dining room. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer and First-Aid Manuel. Smoke detectors and carbon monoxide observed to be working properly and were tested. Residents/Staff Records: LPA observed staff and resident files to be kept locked in a filing cabinet located aside the dining room. LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of staff records. There were no immediate health and safety hazard observed during the day of inspection. Licensing Program Analyst (LPA) Leizl de la Cerra conducted an annual required visit and inspection of the facility. At 10:00AM Antuan Nazaryan facility administrator met LPA at the entrance and LPA explained the reason for the visit. At 10:05AM, required records were requested. Resident and Staff Records reviews and interviews: At approximately 10:15AM four 12:30AM four (4) out four (4) resident records and four (4) staff records were reviewed to ensure compliance. Interview of one (1) staff was conducted. Interview of two (2) residents were conducted. At 12:30PM LPA with the assistance of the administrator took a tour of the facility. Common Area: LPA observed the living room and furniture to be clean and in good repair. The fireplace located in the living room is adequately closed and inaccessible to residents. The facility maintains a comfortable temperature at 77 degrees Fahrenheit. The air conditioner is operational. The facility smoke alarm system is hard wired and interconnected. The facility uses a dual Carbon Monoxide/Smoke alarm detectors all over the common areas of the facility. At 1:15PM they were tested and deemed operational. Facility maintains a telephone land line and it was observed to be operational. Required postings were observed in the hallway. The fire extinguisher is in the kitchen with purchase date of 7/11/2024. The facility is fire cleared for six (06) non-ambulatory residents; one (1) maybe bedridden in bedroom #3 approve for four (4) hospice waiver. Kitchen: The kitchen appliances were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough food for at least three (3) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods and refrigerated foods are wrapped, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. Food preparation areas are clean. Garbage cans have tight fitting covers. Kitchen cleaning supplies were stored in a locked cabinet. Residents' dining table fits enough for six (6) people. -Continue to LIC 809-C Bedrooms: There are three (3) bedrooms that were properly furnished with appropriate dresser, night stand, chair, beddings, and linens with sufficient lighting. Extra linens and beddings are stored in the hallway closet. Bathrooms: There are two (2) bathrooms designated for residents use. The bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 116.8 degrees. Surrounding Grounds: There is a covered patio with furniture appropriate for outdoor use. The outdoor area was free of hazards. The facility does not have any bodies of water. The garage is attached but does not have a connecting door to access inside the house. Garage is used for storing extra food supplies, toiletries, PPE supplies, and incontinence supplies. Facility has a washer and dryer located inside the garage. Laundry chemicals are stored in the garage. The garage has a lock and will remain locked and inaccessible to residents in care. LPA reviewed the facility's dementia plan of care and LPA observed that the facility is in accordance with the plan. The LPA observed that the facility is a fenced facility with self-closing latches and gates. All exterior doors for the front, back and client rooms are equipped with an operational bell/buzzer system to alert staff when door is being opened. These devices are set off by motion of the door. Passageways observed to be unblocked. In addition, administrator informed LPA that knobs on the stove are removed during the night time. LPA observed a locked cabinet that contains all the residents' records, the staff records and the residents' medications. LPA observed the cabinet with top and bottom locks which is inaccessible to residents in care. First-aid has all proper items and is current. The facility was in compliance with Title 22 regulations.
