Type of visit: Case Management - Annual Continuation
At approximately 10:00 AM Licensing Program Analyst (LPA) Stevenson arrived unannounced to CONTINUE the annual required inspection and met with Administrator Van Alvarez. At 10:20 AM LPA observed five(5) staff files and found that 5 of 5 staff files have all required records. At 10:40 AM LPA observed six(6) resident files and found that 6 of 6 residents have all required records as required by title 22 regulations. Facility knows to conduct an Emergency and Disaster drill every 3 months and held a fire evacuation drill with all staff 09/01/2026 At 12:25 PM P&I monies were observed to be secure and their accounting accurate. At 12:30 PM Medicine were observed to be centrally stored and secure and their administration accurate. Sep 8, 2026Facility evaluation reportReport on file
Type of visit: Required - 1 Year
At approximately 1:45 PM, Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to BEGIN a required 1-year annual inspection and was by two greeted staff members including staff member Julius Jumawid (RP) who called Administrator Van Alvarez to advise of the start of the required annual inspection. Administrator Van Alvarez arrived at approximately 2:15 PM to further assist with today's portion of the annual inspection. Facility is a Residential Care Facility for the Elderly (RCFE) with six (6) residents in care. LPA was informed that two (2) residents were away at Day Program and the remaining four (4) residents were present during today's visit. Facility has a hospice waiver for two (2), a bedridden waiver for one (1), is approved for all non-ambulatory residents, and is vendorized with North Bay Regional Center (NBRC). Administrator was advised that a simple "Statement of Information" with the Secretary of State's office was due and document left. At approximately 2:00 PM, LPA initiated a tour of the facility with RP and observed the following: Facility is a two story home with a gate preventing resident access to the upstairs. Facility was a comfortable temperature, and passageways were free from obstructions. Facility has a required evacuation chair mounted on the stairs. Water temperatures in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens and paper products available to clients. Clients' bedrooms were inspected and observed to have appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a covered seating area in the backyard with outdoor space for activities. LPA observed an activity schedule and was informed that each NBRC client has their own internet access device and the facility has an additional devices available for resident use. Continued from LIC809.. Facility's fire extinguishers were observed charged and were last serviced July 2026 Centrally wired smoke detectors in the main house were tested and automatic fire door to back bedrooms was noted to automatically shut to smoke detector testing. Carbon Monoxide detector was tested and operational during inspection. LPA observed emergency supplies, a first aid kit, and flashlights for emergency preparedness, as well as, a back-up lithium power station if needed. At approximately 10:00 AM Licensing Program Manager (LPA) Star Stevenson arrived unannounced to CONTINUE a required 1-year annual inspection and was met by caregivers who called Administrator Kristine Lorenzo who arrive at approximately 10:15 AM to help with today's continued inspection. At approximately at 10:15 AM, LPA reviewed five (5) staff files and six (6) resident files. Five (5) of five (5) staff files reviewed have the required paperwork and proof of current First Aid and CPR training. Six (6) of 6 resident files reviewed have all the required paperwork. At approximately 11:25 AM, LPA reviewed medications and medication records which are maintained and stored in compliance with regulation. Type of visit: Required - 1 Year
At approximately 12:35 PM, Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to BEGIN a required 1-year annual inspection and was by two greeted staff members who called Director and responsible party (RP) Arnel Silverio who arrived at approximately 1:00 PM. Facility is a Residential Care Facility for the Elderly (RCFE) with six (6 ) residents in care. LPA was informed that three (3) residents were away at Day Program and the remaining three (3) residents were present during today's visit. Facility has a hospice waiver for two (2), a bedridden waiver for one (1), is approved for all non-ambulatory residents, and is vendorized with North Bay Regional Center (NBRC). RP reported "Statement of Information" with the Secretary of State's office was updated 07/31/2026. RP was also advised that the LIC400 Affidavit Regarding Resident Cash Resources was filled out incorrectly despite the appropriate Surety bonding and a new appropriate LIC400 was obtained during today's inspection. At approximately 1:10 PM, LPA initiated a tour of the facility with RP and observed the following: Facility is a two story home, was a comfortable temperature, and passageways were free from obstructions. Facility has a required evacuation chair mounted on the stairs. Water temperatures in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens and paper products available to clients. Clients' bedrooms were inspected and observed to have appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a covered seating area in the backyard with outdoor space for activities. LPA observed an activity schedule and was informed that each NBRC client has their own internet access device and the facility has an additional device available for resident use. Continued from LIC809... Facility's fire extinguishers were observed charged and was last serviced July 2024. Centrally wired smoke detectors in the main house were tested and observed to be operation and Carbon Monoxide detector was tested and operational during inspection. Licensee was advised to replace isolated smoke detectors in separate office areas in garage. Facility conducts regular monthly disaster drills, and the most recent drill was conducted September 2025. LPA observed facility's infection control plan and emergency disaster plan which was last updated September 2025. LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights for emergency preparedness, as well as, a back-up lithium power station if needed. No deficiencies are cited today. At approximately 9:40 AM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and was greeted Staff 1 (S1) and Staff 2 (S2). Kristine Lorenzo, House Manager/Designated Responsible Party (RP) was contacted and arrived at approximately 10:05 AM. Facility is a Residential Care Facility for the Elderly (RCFE) with six (6 ) residents in care. LPA was informed that Resident 1 (R1) and Resident (2) were away at Day Program and the remaining four (4) residents were present during today's visit. Facility has a hospice waiver for two (2), a bedridden waiver for one (1), is approved for all non-ambulatory residents, and is vendorized with North Bay Regional Center (NBRC). At approximately 10:30 AM, LPA initiated a tour of the facility with RP and observed the following: Facility is a two story home, was a comfortable temperature, and passageways were free from obstructions. Licensee stopped by at approximately 11:15 AM and ordered the facility an evacuation chair. Water temperatures in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens and paper products available to clients. Clients' bedrooms were inspected and observed to have appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA advised House Manager and Licensee to ensure all such items are secured and inaccessible to residents when not being used. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a covered seating area in the backyard with outdoor space for activities. LPA observed an activity schedule and was informed that each NBRC client has their own internet access device and the facility has an additional device available for resident use. Facility has internet available to clients in care and the phone was tested an operational. Facility's fire extinguisher was observed charged and was last serviced July 2024. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Continued on LIC809-C... Continued from LIC809... Facility conducts regular monthly disaster drills, and the most recent drill was conducted September 2024. LPA observed facility's infection control plan and emergency disaster plan which was last updated January 2024. LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights for emergency preparedness. RP states the facility has a backup solar battery if needed. At approximately 12:00 PM, LPA reviewed five (5) staff files and six (6) resident files. Five (5) of five (5) staff files reviewed have the required paperwork and proof of current First Aid and CPR training. However, six (6) of six (6) staff were short one hour of medication training. (See LIC809D). Five (5) of five (5) resident files reviewed have all the required paperwork. RP coordinates medical and dental visits for the clients and take them to their appointments. At approximately 3:00 PM, LPA reviewed medications and medication records which are maintained and stored in compliance with regulation. However, LPA observed 7/9 instances where staff did not follow the physician's order and instructions for the proper administration of a medication for Resident 3 (R3). (See LIC809D). LPA reviewed P&I monies and logs, which were organized, maintained, and stored according to regulation. RP informed LPA that the Licensee has appointed them as the new facility Administrator. LPA discussed the process and the documents that need to be submitted to CCL to make the change official with Licensing. Type of visit: Case Management - Incident
Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a Case Management - Inspection. The purpose of this case management visit was to follow up on a self reported incident report that was submitted to Community Care Licensing (CCL) on 6/17/2024 regarding client C1. During visit LPA went over incident details, gathered records, took statement from House Manager, and made observations. Per review of today's records and interviews, staff addressed the incident as needed, when it occurred. No deficiencies were found at the time of visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Carol Fowler conducted a Required- 1 Year visit, on 10/06/2023 at approximately 9:10am, and met with Lourdes Macabento, Caregiver. Administrator Grissel Dean arrived at approximately 10:00am. Certificate, #6063576740, is current- expires 08/04/2024. LPA observed two caregivers working at the time of arrival. There are currently six (6) residents in care. Facility has a required infection control plan. Facility has an emergency disaster plan as required. The facility conducted a fire drill and an earthquake emergency drill on 10/02/2023. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. All exits were free and clear of obstruction. Fire extinguisher, was last serviced on 07/07/2023 and tagged as required. LPA observed eleven (11) smoke alarms with combined carbon monoxide detectors, working properly during the inspection. Facility was found to be clean, orderly, and at a comfortable temperature. Hot water was checked at 115.7 F, which is within regulation. Medications were stored and locked making them inaccessible to residents. There was a sufficient supply of hygiene products, cleaning supplies, and paper products for use as needed. All bathrooms had grab bars, and non-slip mat/flooring for bathing/showering as needed. Facility has a sufficient supply of personal protective equipment(PPE) for use as needed. LPA observed sufficient supply of food, perishable and non-perishable for residents in care. Continue on LIC809C Continue from LIC809 There are currently six (6) residents in care. LPA reviewed four (4) of six (6) resident files; All resident files were found to be complete. LPA reviewed four (4) staff files. LPA reviewed staff training. All four (4) staff have criminal record clearance, and are associated as required. All staff had required annual training. All staff had current First Aid and CPR Certification. LPA is requesting the following documents be updated and submitted by 10/16/2023: LIC308 - Designation of Administrator Responsibility LIC500 - Personnel Report LIC610E-Emergency Disaster Plan (ensure to provide all information in all boxes as required) Infection Control Plan Control of property Copy of Current Liability Insurance Copy of current Administrator Certificate No deficiencies cited during inspection.
