On 10/13/2025 at approximately 01:15PM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Residential Care Facility for The Elderly (RCFE). LPA was greeted by Administrator, Jose Arsenio Angeles. Facility has an approved fire clearance and capacity of 4 non-ambulatory residents. Facility has an approved hospice waiver for 2 individuals. Facility is vendorized with the North Bay Regional Center (NBRC). Upon arrival, LPA was informed that there were 4 residents in care and 3 staff members on-site. Facility is a 1 story building with 4 Resident bedrooms, 2 bathrooms, 1 staff bedroom, and common spaces. At approximately 1:45PM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 02:05PM, LPA and Licensee toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for resident use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Staff were in the process of cleaning up kitchen at the time of this inspection. Toxins are stored in a locked under the sink and housekeeping closet in the laundry room. Water temperature measured 109.6 degrees F and 107.4 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents. There is a covered seating area in the backyard with outdoor space for activities. Fire Extinguishers found to be last charged on 05/02/2025 at the time of visit. Carbon Monoxide and smoke detectors were present and in order. There was enough lighting in all common areas, resident rooms, and hallways. Medication is centrally stored and secure in a cabinet cabinet next to the dining room. Continued on LIC809-C... Continued from LIC809... At approximately 2:35PM, LPA reviewed 3 resident records and found 3 of 3 residents have current care plans, signed admission agreements, and physician's report on file. Medication records are thorough and contained physician's orders for each resident. At approximately 2:55PM, LPA reviewed 2 staff records. 2 of 2 records did contain documentation of completed training records as required. Evidence of current first aid and CPR training were current. LPA was presented with proof of current CPR & 1st Aid certification for the whole staff. Administrator Certificate is for Jose Arsenio Angeles #6073061740 expires 06/17/2026. LPA reviewed P&I, medications, and medication records which are maintained and stored in compliance with regulation. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility conducted and documented a disaster drill on 09/02/2025. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by due date of 10/25/2025: - LIC 308 Designated - LIC 500 Personnel Summary - LIC 610 Emergency Disaster Plan (If changed) - LIC 9020 Register of Facility Client’s/Resident’s - Copy/Proof of Updated Certificate of Liability Insurance No deficiencies were observed in the areas inspected, No citations were issued during today’s visit. At approximately 12:15 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and was greeted by Jose Arsenio Angeles, Administrator. Facility is a Residential Care Facility for the Elderly (RCFE) with four (4) residents in care. All residents were present during today's inspection. Facility has a hospice waiver for two (2) and is approved for all non-ambulatory residents. Facility is vendorized with the North Bay Regional Center (NBRC). At approximately 12:45 PM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one story home, was a comfortable temperature, and passageways were free from obstructions. Water temperatures in Residents' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of hygiene products, clean linens, paper products, and incontinent care briefs available to residents. LPA advised Administrator to ensure that hand towels are not used in the bathrooms and that paper products are always available to residents. LPA also advised Administrator to ensure that all trash cans in the facility are covered to remain in compliance with regulation. Residents' bedrooms were inspected and observed to have appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked, except in the main facility full bathroom, where LPA observed the magnet used to unlock the cabinets stored on the cabinet which contained Draino, Lysol, Goo Gone, Soft Scrub, and other chemicals which pose a risk to residents in care, (see LIC809D). Additionally, LPA advised Administrator to store the facility's red sharps container inaccessible to residents in care and suggested the locked medication cabinet as an option. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a covered seating area in the backyard with outdoor space for activities. Facility has an internet access device available for resident use. Facility has internet available to residents in care and the phone was tested an operational. Continued on LIC809-C... Continued from LIC809... Facility's fire extinguisher was observed charged and was last serviced 5/2024. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Facility conducts monthly disaster drills with the most recent drill was conducted 11/2024. LPA observed facility's infection control plan and emergency disaster plan which was last updated 9/2024. LPA observed a supply of PPE, emergency supplies, a first aid kit, flashlights and a back up generator for emergency preparedness. Administrator provided LPA with a copy of the facility's current liability insurance. At approximately 1:30 PM, LPA reviewed four (4) staff files and four (4) resident files. Four (4) of four (4) staff files reviewed have all of the required paperwork and proof of current First Aid and CPR training. Four (4) of four (4) resident files reviewed have all the required paperwork. Administrator and residents' families coordinates medical and dental visits for the residents and transportation to and from their appointments. At approximately 3:30 PM, LPA reviewed P&I, medications, and medication records which are maintained and stored in compliance with regulation. Facility has a current bond on file. Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Appeal rights were given. On, 10/13/2023, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection visit for this facility and was greeted by Administrator, John Francoius Delos Reyes. The facility is licensed for all non-ambulatory rooms and a hospice waiver for two. The facility currently provides care for three residents, none of which are receiving hospice services and some of which with a diagnosis of dementia. LPA continued with a tour of the facility with Administrator, facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Facility exits were properly equipped with auditory alarms for residents with dementia, tested and found to be in working order. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguisher was found to be last charged on 6/10/2023. Both smoke detectors and carbon monoxide detectors throughout the facility were interconnected, tested and found to be in working order. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations with food properly stored and labeled. Toxins, sharps and other items that could pose threat if readily available to residents were kept secured in the kitchen, laundry room and garage area. Residents were observed engaging in discussion with staff in common areas and watching television in their bedrooms. Residents appear to have a positive relationship with staff based on LPA observations. Staff were observed continuously assisting and supervising residents and conducting room checks during LPA's inspection. Continued onto LIC809-C There was a supply of hygiene products, continence products, paper products and clean linens available for residents. All resident bedrooms have lighting & appropriate furnishings. Medications are stored in a designated medication cabinet located in the common area and were found to be secured. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. LPA found one prescription for client R2 not properly administered the previous evening of 10/12/2023. Licensee, Administrator and LPA discussed corrections. Technical Violation Issued. A sample file review for staff was conducted and LPA found all staff to have sufficient 1st Aid & CPR certification and annual training on file. LPA also conducted a file review for all residents and found a majority of resident records updated. LPA found resident (R1) physician's report in need of updating. Technical Violation issued. Facility to submitted update facility sketch indicating all bedrooms are non-ambulatory based on current fire clearance.
