Licensing Program Analyst (LPA) Sarina Ramirez conducted an announced visit to the facility for the purpose of a Change of Ownership evaluation. LPA met with Licensee Richie Garcia and Administrator Mary Saeed. An initial application for change of ownership to operate a Residential Care for the Elderly facility (RCFE) was submitted to the Central Applications Bureau (CAB) on 07/18/2025 for a total capacity of twenty (20) non-ambulatory residents. Fire clearance was granted on 10/10/2025. LPA observed the following: The facility has ten (10) bedrooms and five (5) bathrooms. There are no bodies of water. The physical plant, in general, was in good repair. Indoor and outdoor passageways were free of obstruction. There is adequate amount of seating and furniture in good repair in the common areas; There is enough space for resident activities. Outdoor activity area is enclosed with a gate and there is adequate outdoor shaded area for resident activities. The facility is equipped with telephone service, fully charged fire extinguishers, smoke alarms/carbon monoxide detectors, and laundry equipment. There is a designated locked cabinet where sharps and cleaning supplies are stored. Resident bedrooms had the required bedding, furniture, and sufficient lighting. LPA measured the hot water temperature in the resident bathrooms, and it ranged from 105 to 115.2 degrees F. The facility had a sufficient amount of linens and hygiene items for the residents. The facility had a sufficient amount of nonperishable and perishable food items. The food was kept in a safe and healthful manner. The facility menu was available for review. Refrigerator and freezer are clean and operating in good condition. Dishes, glasses, and utensils were in good condition. The facility had a designated area for staff and resident records. Posted in a common area were the Emergency disaster plan with emergency telephone numbers, personal rights, CCLD/Ombudsman posters, facility sketch, house rules, activities schedule. The facility was equipped with a complete first aid kit and manual. Pre-licensing inspection and Component III is complete, no corrections are needed to be made. Facility appears to be ready for licensure. COMP II by CAB successfully completed Method: Phone Call at CAB Applicant/administrator participated in COMP II at CAB telephone call with analyst at CAB. Identification of the applicant and administrator was verified by presenting photo ID via phone. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1. Facility operation: License type, client/resident populations, and program 2.
