On February 24, 2026, Licensing Program Analyst (LPA), Jarred Torres, and LPA, Venus Mixson, arrived at the facility unannounced to conduct an annual inspection and met with Administrators Linda Thomas and Erik Thomas. A facility file review was conducted at the regional office and additional records were requested and reviewed at the facility. The facility is licensed for six adults and is currently operating at a census of five adults. LPA Torres and LPA Mixson toured the facility along with administrator Thomas and made observations pertaining to the annual visit. The LPAs inspected the facility inside and outside. There were no obstructions or debris to the indoor nor outdoor passageways at the time of this visit. Additionally, there were no bodies of water on the premises. The facility is a single-story home located at 7675 Woodview Street, Riverside, CA 92506. The LPAs were given a tour of the premises. The facility phone number is 951-534-0843 and is operable. The LPAs observed the residents' bedrooms which were equipped with the required furniture as stated in Tittle 22 of the California Code of Regulations (CCR). The LPAs observed the bathrooms to be equipped with grab bars and anti slip pads. The facility's appliances were observed to be operational at the time of this visit. The facility is equipped with operational smoke detectors and a carbon monoxide detector. The LPAs observed fire extinguishers in good condition which had been serviced on 3/24/2025. The LPAs observed the required postings regarding safety, personal rights, and emergency exits. The cleaning supplies and sharp items were kept locked and inaccessible to the residents in care. There was a designated storage space for the resident and staff files. The furniture in the facility is in good condition. The facility's cooling and heating system is operational and the LPAs observed the air temperature at 75 degrees Fahrenheit (F) and the hot water at 112 degrees F. The LPAs observed the medications to be locked and inaccessible to residents in care. The medication supply was sufficient for the five residents in care. No discrepancies with medication were observed. The LPAs observed an adequate food supply of non-perishable and perishable foods which consisted of fresh fruit, fresh vegetables, starchy foods, frozen meats, and snacks. The supply of dishes and utensils was sufficient for the residents in care. The LPAs observed adequate staff being present for the supervision of the residents in care. Floor plans, telephone numbers and personal rights were observed posted in the facility. The administrator, Linda Thomas, has a valid Residential Care Facility for the Elderly license which expires on 8/7/2027. The LPAs reviewed staff files and client files. The staff files are current, which includes, but is not limited to, TB tests showing a negative result, completed trainings, and first aid certifications. Resident files were reviewed and the LPAs found no discrepancies. The LPAs reviewed the emergency disaster plan and fire clearance. The last emergency drill was conducted on 1/24/2026. The emergency disaster plan meets the department's standards. The LPAs observed the hand washing stations in the facility restrooms. The restrooms contained the required non-medicated soaps and single-use hand towels. Additionally, the facility has an approved infection control plan in their files. Licensing Program Analyst (LPA) Yolanda Delgado arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction, visit purpose and provided the facility caregiver with LPA identification and business card. Resident record review began- Six (6) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility is meeting documentation requirements. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 112.0 degrees F. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals in the home. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA verified there is a telephone working at this location. Food Service- Food supply meets the requirement of one week supply of nonperishable and 2 day supply of perishables food on hand. A menu is posted, foods are dated to assure safety. Food prep areas are clean and organized. LPA began review of employee records- Three (3) records were reviewed. LPA reviewed employee record for first aid certification, fingerprint clearance, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain (Continued on next page) (Continued on from Page 1) cleanliness and meet the needs of the clients in care. Administrator certification is present and current. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers are tested or replaced annually and were last done so on 03/29/2024. The facility is conducting emergency disaster drills. The last disaster drill was conducted on 01/26/2025. LPA allocated time to prepare this report for delivery. Based on the information received during this visit today, there zero (0) deficiency is being cited per Title 22, Division 6 of The California Code of Regulations. Licensing Program Analyst (LPA) Sara Martinez conducted an announced visit to complete the Pre-licensing inspection. LPA met with Licensee Linda Thomas, for a Residential Care Facility for the Elderly (RCFE) with a capacity of six (6) residents. The facility is a five (5) bedroom, four (4) bath home. There are five (5) resident bedrooms, kitchen/dining area, one (1) living room area, a laundry room, attached garage, a detached garage, and a backyard. The facility does not contain any bodies of water or firearms or ammunition. LPA toured the interior and exterior areas of the facility. The following were inspected: All bedrooms have the required bedding and furniture, such as, clean mattresses/linen, nightstands, dressers, chairs, lighting, and emergency lighting. The bathroom appliances were operating in safe and sanitary condition. The bathrooms have non-slip mats and grab bars. Utensils and dishware are in good repair and ready for client use. Kitchen appliances and counter top were free of debris and in good repair. The knives and sharp objects were locked in the drawers. The water temperature was measured by LPA, the thermometer read at 115 degrees F. There is adequate seating in the common areas. The facility will have a storage locker that will hold resident files and staff files locked in a hallway closet. The facility will have medication locked in a hallways closet. The facility will keep the detergents and cleaning chemicals in locked cabinets located in the laundry room. An adequate supply of clean linens and hygienic supplies for the residents were readily available at the facility. There are two (2) charged fire extinguishers in the facility. LPA observed operating smoke detectors and carbon monoxide alarms in the facility. LPA observed required postings including the visitation polices, emergency/disaster plans, complaint procedures, and personal rights. The facility was equipped with a complete first aid kit and manual. The facility has working telephone for resident use. LPA observed that the physical plant is clean, in good repair, and to be hazard-free during today’s visit. LPA has determined that the facility met the operational requirements for licensure. The Pre-licensing inspection is complete, and the facility has no deficiencies. The facility has satisfied all requirements in accordance with Title 22, California Code of Regulations.
