On 07/10/26, Licensing Program Analysts (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit and requested to meet with Administrator. LPA met with House manager (S1) Whitney Hatton. LPA was informed by S1 that Licensee/Administrator Frances Augdon Assistant was unavailable to attending meeting. Administrator Renee Arreguin was called arrived shortly and left during visit unable to stay for meeting. LPA toured facility with S1. All two residents were present during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Medications observed kept locked in kitchen shelf. Fire extinguishers were observed with a service date 02/5/26. Last fire drill was completed on 05/30/26. LPA observed 2 day supplies of perishable and 7 day supplies of non-perishable food. Cleaning supplies observed stored and locked under kitchen sink and in laundry shelves. All bedrooms were observed to have required furnishings and with adequate lighting. Bedroom 2 door observed in disrepair and hole was observed in the lower wall in bedroom 2. Bathroom was toured. LPA observed with securely fastened grab bars and non-skid mats. Hot water temperature was tested at 114 degrees F in bathroom 3, 115.2 degree F in bathroom 2, and 113.5 degree F in bathroom 1. Extra linens and towels observed in hall closet. (Continued to Lic 809C) (continued from Lic 809) Outside of facility toured and observed to be free of debris with adequate outdoor seating available for residents. Carbon monoxide and smoke detectors were tested and observed to be operational. All residents and sample of staff files were reviewed to have all required documents. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6 see attached 809D. Exit Interview conducted. On 08/11/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct a required Annual Inspection. LPA introduced self, stated the purpose of the visit, and met with caregiver Tyler Flores and Julie Pimentel. House Lead Whitney Hatton called and arrived shortly. All three residents were present during inspection. Administrator Renee Arreguin arrived later during inspection. LPA toured facility with Licensee and caregiver. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. Fire extinguisher was observed with the service date 02/05/25. Medications were observed kept locked in the kitchen cabinet. Residents’ MARS was reviewed. An adequate supply of perishable and non-perishable food was observed. Cleaning chemicals were observed locked under kitchen sink and laundry room shelf. Extra linens observed. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. Bathrooms were toured and observed functional. Non-skid mats and grabbed bars were observed. Hot water temperature was tested maintained at 109.9 degrees F in bathroom 1, 110.8 degree F in bathroom 2, and 107.2 degree F in bathroom 3. Outside of the facility toured. Exit side gate was self-closing and self-latching. Adequate outdoor seating is available for residents. Carbon monoxide and smoke detectors were operational during inspection. All residents’ and sample staff files were reviewed to have all required records. No deficiencies issued during this inspection. An exit interview was conducted. The following documents are requested and submitted to Fresno CCL by: 08/18/25. The following updated forms were requested: Lic 308, Lic 500, Lic 610E, current liability insurance, and current Administrator Certificate. On 08/08/2024, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. Facility staff granted LPA entry to the facility and contacted Licensee, Whitney Hatton via telephone. Licensee arrived a short time later. LPA met with Licensee and Administrator, Renee Arreguin. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Resident rooms appeared clean and had required furnishings. LPA observed an adequate supply of linen. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Hot water measured between 112.7 degrees F through 114.4 degrees F. Kitchen toured, appeared clean, observed a 7-day supply of non-perishable food and 2-day supply of perishable food. Exterior tour conducted, all exits open and free of obstructions. Side gate was observed to be self-latching. Fire extinguisher serviced on 01/05/2024. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Last fire drill conducted on 04/27/2024. All cleaning supplies are locked and secured under kitchen sink. LPA reviewed staff and client records. Medications reviewed and observed to have original labels and be administered as prescribed. LPA is requesting the following documents be submitted to the Fresno CCL office by 08/22/2024: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents (LIC9020A), Surety Bond No deficiencies issued. Exit interview conducted.
