Type of visit: Required - 1 Year
At approximately 10:00 AM, Licensing Program Analysts (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and was greeted by staff. Christine Arnke, Administrator, was contacted via telephone and arrived approximately ten (10) minutes later. Facility is a Residential Care Facility for the Elderly (RCFE) with five (5) residents in care. Facility has a Dementia Care Plan, a Hospice waiver for six (6), with two (2) Hospice residents currently in care, and is approved for five (5) non-ambulatory residents and one (1) bedridden resident. At approximately 11:00 AM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one story home, was a comfortable temperature, and passageways were free from obstructions. Water temperatures in residents' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, and hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a shaded seating area in the backyard with outdoor space for activities. LPA observed a piano, musical instruments, books, puzzles, and games available for residents in care. Facility has live music approximately once per week. Facility has internet access and Administrator agrees to ensure an internet access device is designated for resident use. Continued on LIC809-C... Continued from LIC809... Facility telephone was tested an operational during inspection. Facility's fire extinguisher was observed charged and was last serviced 10/2025. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Facility conducts quarterly disaster drills with the last one completed on 08/2025. LPA observed the facility's infection control plan, first aid kit, PPE, flashlights, and emergency supplies. LPA observed a generator for emergency preparedness. LPA reviewed facility's emergency disaster plan which was last updated 11/2024. At approximately 12:00 PM, LPA conducted file review. LPA reviewed three (3) staff and three (3) resident files and observed the following: Three (3) of three (3) staff files reviewed contained the required paperwork per regulation. Administrator agrees to ensure that all staff have proof of current FA/CPR certification at all times moving forward in order to remain in compliance with regulation. Administrator showed proof of registration to renew this certification during today's inspection. Three (3) of three (3) resident files reviewed contained the required paperwork per regulation. LPA discussed the requirement that care plans be updated no less than every twelve (12) months. Licensee and Administrator agreed to date and sign care plans moving forward. At approximately 2:30 PM, LPA reviewed medications and medication records which are maintained and stored in compliance with regulation. Administrator states that the residents families coordinate residents' medical and dental appointments and transportation to and from visits. However, facility will assist with coordinating these appointments and transportation for residents upon request. Facility does not manage P&I. Updated copies of the following documents are to be submitted to CCL within 30 days of this visit: -LIC610E - Emergency Disaster Plan (updated) -LIC500 Personnel Report (updated) No deficiencies were cited during today's inspection. Allegation investigated: Not Meeting Residents’ Care Needs Insufficient Staffing Staff Training
On 09/25/2025, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to deliver complaint #21-AS-20250610133209 investigation findings regarding the above allegations and met with Christine Arnke, Administrator. Reporting Party (RP) alleges that the facility is not meeting residents’ care needs, there is insufficient staffing, and staff are not properly trained. LPA Florio conducted 10-day complaint investigation visit on 06/17/2025 and obtained documents, made observations, and conducted interviews. During this visit, it was revealed through an interview with Licensee and the facility's LIC500 Personnel Report that facility is sufficiently staffed, with two (2) care staff and the administrator present M-F and every other Saturday during daytime hours; two (2) care staff every Sunday and every other Saturday during daytime hours; and one awake, non-live in care staff every night, Monday through Sunday. Continued on LIC9099C... Unsubstantiated Continued from LIC9099... On 06/10/2025, LPA received a string of email communications between Licensee and R1's responsible party and a 30-day eviction notice both detailing the increased care needs of R1 requiring 1:1 care that the facility was unable to provide without additional staffing. The facility offered R1's responsible party the option to hire a 1:1 caregiver or relocate the resident in these communications. On 09/25/2025, LPA obtained copies of R1's third party care plans dated 01/03/2025 and 06/9/2025; R1's LIC602 physician's report dated 10/20/2024; R1's Pre-Placement Appraisal dated 10/23/2025; and R1's re-appraisal dated 06/06/2025. These documents revealed an increase in R1's care needs requiring a hirer level of care from when R1 first moved into the facility 10/2024. On 06/17/2025 R1 was relocated to a new care facility. On 09/23/2025 LPA received copies of additional email and text communication between R1's responsible party and the Licensee discussing an incident that occurred on 06/04/2025 where R1's responsible party expressed staff training concerns due to their witnessed response of care staff to an emergency situation. On 09/25/2025, LPA obtained copies of S1 and S2's current first aid and CPR certifications, staff hoyer lift training, and observed all the required training documented in S1 and S2's personnel files. Based on observations made, interviews conducted, and records reviewed, the department received conflicting information. Based on interviews conducted and records obtained, the allegations the facility is not meeting residents’ care needs, there is insufficient staffing, and staff are not properly trained are UNSUBSTANTIATED. A finding that a complaint allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are UNSUBSTANTIATED. Type of visit: Required - 1 Year
At approximately 10:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a Required-1 Year inspection. LPA met with Licensee Krystal Smith and explained the purpose of the visit. Administrator certificate is current. LPA toured the facility to ensure the health and safety of residents in care. Areas toured include but are not limited to resident rooms, common areas, bathrooms, kitchen, storage areas and back yard. In the areas toured no immediate health, safety, or personal rights violations were observed. The common areas, bathrooms and kitchen were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Cooking/dining equipment and utensils were present. Food appears to be stored and prepared properly. Facility has the required seven-day non-perishable and two day perishable food supply. Medication is locked and not accessible. The facility was observed to be at a comfortable temperature. First aid kit was present. Fire extinguishers were fully charged. Smoke detectors are all operational. Carbon Monoxide Detector was present. All employees requiring background checks are cleared. No pools/bodies of water are on the premises. Facility has been conducting drills every 3 months. At approximately 10:45AM, LPA reviewed 4 Staff and 5 Resident files. All resident files contained the required documentation. Staff files reviewed contained evidence of completed annual training. First Aid/CPR certification was current. Exit interview conducted and copy of report was provided to administrator. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Helena Rummonds arrived unannounced at approximately 9:30AM to conduct an Annual Required inspection and to follow up on an Incident Report that was recieved by Community Care Licensing (CCL) on 12/22/2023. LPA was greeted by Administrator, Christine Arnke. LPA and Administrator discussed the purpose of the visit. LPA and Administrator initiated a walk through of facility at approximately 9:45AM and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperature in bathrooms used by residents measured at 115 and 114 degrees F which are within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Cabinets containing cleaning supplies were locked. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. Medications were centrally stored and locked. Emergency food is rotated and water is stored in the garage along with Personal Protective Equipment. Fire extinguisher was last serviced 10/06/2023. Smoke and Carbon Monoxide detectors located throughout the facility were tested and operational during inspection. Most recent fire/disaster drill was conducted 11/26/2023. Five staff files and five resident files were reviewed. Staff have required First Aid and CPR certificates. Training records were reviewed. Administrator certificate for Administrator, Christine Arnke (6058377740) expired on 08/08/2023 and is on the pending list for renewal. Medications and medication records were reviewed. Continued on LIC809C Continued from LIC809 During review of resident records, it was noted that Resident 1 (R1) is bedridden as per their physicians report. Per conversation, R1 is unable to reposition themselves without assistance. LPA is requesting the following documents in order to update the facility fire clearance: -Updated facility sketch indicating which bedroom will be for bedridden resident -Updated LIC200 requesting bedridden fire clearance in section 1b (printed and provided to Administrator) During visit, LPA followed up on Incident Report received by CCL on 12/22/2023. Incident report states that on 12/20/2023 and 12/21/2023, Resident 2 (R2) was aggressive towards staff and made multiple attempts to leave the facility. Staff stayed with resident during all attempts to leave the facility. On 12/20/2023, R2's family and Licensee discussed relocation. On 12/21/2023, R2 attempted to leave the facility and Administrator followed. While outside R2 struck Adminstrator. Law enforcement was contacted as well as R2s family. Family member requested that they take R2 to their home. R2 went to the family members home and Licensee further discussed with family members R2s relocation. Per conversation with Administrator, R2 was appropriately placed into facility based on R2s physicians report and resident appraisal. R2s Power of Attorney (POA) informed Administrator(s) that R2 has difficulties with their mental health. However, their physicians report revealed no diagnosis' other than their dementia diagnosis. Administrator confirmed that R2's POA was in favor of relocating R2 due to their need for a higher level of care. LPA reviewed R2's physicians report and confirmed that there were no mental health diagnosis' that would reveal R2 needing a higher level of care than what facility could provide. No deficiencies cited during inspection. LPA is requesting the following documents to be submitted to CCL by 01/28/2024: LIC 500 Personnel Report Liability Insurance LIC 9020 Resident Roster Exit interview conducted.
