At approximately 12:40 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and met with Christine Arnke, Administrator. Facility is a Residential Care Facility for the Elderly (RCFE) with six (6) residents in care. Facility is approved for five (5) non-ambulatory residents, one (1) bedridden resident, has a Dementia Care Plan, and has a Hospice waiver for six (6). At approximately 1:15 PM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one story home, was a comfortable temperature, and passageways were free from obstructions. Water temperatures in residents' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, and hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in regulation. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a shaded seating area in the front and backyards with outdoor space for activities. LPA observed a locked structure on the property which contained mostly tools, equipment, and chemicals. Facility has internet access and an internet access device available for resident use as required per regulation. Residents were observed engaging with staff and visitors, reading, and watching TV during inspection. LPA observed an activity schedule with music scheduled regularly. LPA observed supplies for other activities. Facility telephone was tested and operational during inspection. Continued on LIC809-C... Continued from LIC809... Facility's fire extinguisher was observed charged and was last serviced 10/2025. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Facility conducts quarterly disaster drills and the last one was conducted 03/2026. LPA observed the facility's infection control plan, first aid kit, PPE, and emergency supplies for emergency preparedness. LPA reviewed facility's emergency disaster plan last updated 11/2025. At approximately 2:00 PM, LPA reviewed three (3) resident records and three (3) staff records. Three (3) of three (3) residents' records contained all the required documentation. Three (3) of three (3) staff records reviewed contained all the required paperwork including proof of training and current CPR/FA certification. At approximately 3:15 PM, medication records were reviewed and were observed managed and stored in compliance with regulation. Administrator states that most of the residents' family members coordinate residents' medical and dental appointments and transportation to and from visits. However, facility assists with coordinating appointment and third party transportation when needed. Facility does not handle P&I. Administrator provided LPA with proof of liability insurance. No deficiencies cited during today's inspection. At approximately 12:25 PM, Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by Staff. Administrator Christine Arnke arrived at approximately 12:30 PM. Facility is a Residential Care Facility for the Elderly (RCFE) with six (6) residents in care. Facility has a Dementia Care Plan, a Hospice waiver for six (6), with five (5) Hospice residents currently in care, and is approved for five (5) non-ambulatory residents, and one (1) bedridden. At approximately 12:40 PM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one-story home, was a comfortable temperature, and passageways were free from obstructions. Fire extinguishers were last inspected 10/2024. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperatures measured 119.4 degrees F and 107.5 degrees F which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed at least a 2-day supply of perishable and 7-day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered. There is a shaded seating area in the backyard with outdoor space for activities. LPA was informed that facility hosts various live musical performances by choirs and students on a regular basis as well taking residents for walks in the community. Facility has an internet access device designated for resident use and internet service available. Continued 809C... Continued from 809... All required documentation present. At approximately 2:30 PM LPA and Administrator conducted a spot check of medication and medication records. Medication is centrally stored and locked. Christine Arnke Administrator Certificate 702438740 expires 08/08/2025. All fees are current as of this time. LPA and Administrator discussed facility's Emergency Disaster plan, No new updates. Facility’s last quarterly disaster drill was conducted on 1/2025. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: Liability Insurance LIC500- Personnel Report LIC308- Designation of Responsibility LIC604 – Admission Agreement (blank form) No deficiencies cited. Licensing Program Analyst (LPA) Helena Rummonds arrived unannounced at approximately 9:15AM to conduct an Annual Required inspection and was greeted by staff. LPA and staff discussed the purpose of the visit. Administrator, Christine Arnke arrived shortly after. LPA and Administrator initiated a walk through of facility at approximately 9:30AM and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperature in bathrooms used by residents measured at 115 and 116 degrees F which are within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Cabinets containing cleaning supplies were locked. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. Medications were centrally stored and locked. Emergency food and water is rotated and stored in the hallway closet. LPA observed an ADU in the backyard. Per conversation with Administrator, only the Owner/Administrator stays in the house when they are in town. LPA and Administrator discussed that only people who have been background cleared and are associated to this facility are able to stay in the ADU. In addition, LPA and Administrator discussed that Community Care Licensing (CCL) must be notified prior to any construction on facility grounds. Fire extinguishers were last serviced 10/06/2023. Smoke and Carbon Monoxide detectors located throughout the facility were tested and operational during inspection. Most recent fire/disaster drill was conducted 01/19/2024. Continued on LIC 809-C Continued from LIC809 Five staff files and five resident files were reviewed. Staff have required First Aid and CPR certificates. Training records were reviewed. Administrator certificate for Administrator, Christine Arnke (6058377740) expired on 08/08/2023 and is on the pending list for renewal. Medications and medication records were reviewed. During medication review, LPA observed that (Resident 1's) R1's Centrally Stored Medication Log reflected a start date of a new bubble pack with the date 02/07/2024. However, Resident 1 moved into facility on 02/10/2024. Should the medications have begun on 02/07/2024, there would be a total of 7 medications administered from the bubble pack. In addition, facility keeps a Medication Administration Record that indicated that medications were successfully administered to R1, however the medication bubble pack reflected that less than 7 days of medications were successfully administered. Centrally Stored Medication Log was not accurate. LPA is requesting the following to be submitted to CCL by 03/15/2024: Permits for ADU Updated facility sketch reflecting ADU on property Proof that ADU was cleared by Fire Department LIC308 Designation of Facility Responsibility LIC 500 Personnel Report Liability Insurance LIC 9020 Resident Roster Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code.
