Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. The Administrator Mark Fisk could not be at the visit. Mark Fisk's Administrator's certificate expires on January 26, 2028. LPA met with backup Administrator Brigitte Fisk and explained the reason for the visit. The facility is a single story home with 7 bedrooms (1 bedroom is for staff only), 7 bathrooms, living room, kitchen, dining room and a detached two car garage. LPA and Administrator toured the facility. LPA observed the See Something, Say Something poster (PUB 475) posted in the main entry way of the facility. LPA observed the kitchen is clean and organized. There is a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. Knives are kept locked in a kitchen drawer. The fire extinguisher in the kitchen is fully charged. Cleaning supplies and chemicals are kept locked in the closet. Medications are kept locked in a separate closet. LPA observed all 6 resident rooms have the required furnishings. LPA observed all 6 resident bedrooms are clean and organized. Hot water measured 114.2 degrees Fahrenheit in the shared bathroom in the hallway next to the living room. Smoke detectors/carbon monoxide detectors tested operational. LPA inspected the first aid kit. The first aid kit had all the required elements. LPA and Administrator toured the garage. The garage is kept locked and used for storage. LPA and Administrator toured the backyard. No bodies of water observed. There is a patio with tables and chairs for residents to sit outside. There is no umbrella or other fixture for shade. The exit gate is operational. The Northeast side yard is taped off and undergoing renovations. The Administrator reported none of the residents go outside unassisted and no one is allowed in the side yard until the project is completed. LPA reviewed 5 resident files, no discrepancies observed. LPA reviewed 5 resident medications. No discrepancies observed. LPA reviewed 2 staff files, no discrepancies observed. Both staff had the required training and CPR/First Aid training. Both staff are background cleared and associated to the facility. No obstacles or hazards observed inside of the facility. No deficiencies are being cited as a result of the visit. Allegation investigated: Financial abuse of residents. Licensee did not follow the reporting requirements. Licensee did not safeguard resident's resources.

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required 10-day visit to begin the investigation into the allegations listed above. LPA met with Administrator Mark Fisk and explained the reason for the visit. The investigation into the allegation, financial abuse of residents, revealed the following. It was alleged Staff 1 (S1) financially abused Resident 1 (R1). LPA interviewed R1 who reported none of the staff at the facility, has financially abused them. Resident 2 (R2) reported they have not been financially abused. The 4 remaining residents were not available for interview. LPA interviewed 2 staff present at the facility who denied the allegation. The Administrator reported that S1 has never worked at the facility. A review of facility records and of the Guardian Background Check System shows S1 has never worked at or been associated to the facility. Based on the evidence gathered the allegation is deemed unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Unfounded The investigation into the allegation, licensee did not follow the reporting requirements, revealed the following. It was alleged that the facility did not report the possible financial abuse of R1. The Administrator reported that no incidents have occurred in the last 30 days that are required to be reported. LPA interviewed 2 staff members who reported there hasn't been any incidents in the last 30 days. R1 reported that nothing has happened to them while they have lived at the facility that would need to be reported to anyone. Based on the evidence gathered the allegation is deemed unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. The investigation into the allegation, licensee did not safeguard resident's resources, revealed the following. It was reported that the licensee took no action to assist R1 with the protection of their financial resources and did not safeguard R1's credit card information which was taken by staff without permission. R1 reported none of the staff at the facility has accessed their credit card or bank information. R1 reported they handle their own finances and the staff have not taken advantage of them. The Administrator reported that none of the staff have taken any residents financial information. The Administrator reported that none of the residents have entrusted any items for the facility to safeguard. 2 staff interviewed reported they have not taken any residents credit card or financial information. Based on the evidence gathered the allegation is deemed unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Hot water measured 111.6 degrees Fahrenheit in the shared bathroom in the hallway next to the living room. Smoke detectors/carbon monoxide detectors tested operational. The last fire drill was on February 3, 2025. LPA inspected the first aid kit. The first aid kit had all the required elements. LPA and Administrator toured the garage. The garage is kept locked and used for storage. LPA and Administrator toured the backyard. No bodies of water observed. There is a patio with tables and chairs for residents to sit outside. Licensing Program Analyst (LPA) Ruth Martinez conducted an announced visit to the facility for purpose of a pre-licensing evaluation. LPA arrived at facility was greeted and granted entry. LPA met with Donald, Brigitte, Erin, and Mark FISK, applicants and explained the visit. An initial application to operate an Adult Residential Facility for the Elderly, age 60 years and over, for (6) capacity, (2) ambulatory, (1) non-ambulatory, and (3) bedridden resident was submitted to CCL on 05/01/23. Structure: The facility is a one story house with a garage with 6 resident bedrooms, 1 staff bedroom, 7 full bathrooms, a living rooms, a dining room, and a kitchen. The resident’s bedrooms are all private and are spacious, will easily accommodate the resident’s furnishings. There is a back yard with a patio and one exit walkway on one side of the house with seating for the residents. Air/Heating: Central air/heating system installed with a central panel to control entire house. Bedrooms Residents: Bedrooms will accommodate 6 residents with 6 private rooms. All 6 resident bedrooms have a full bathroom. Bedrooms Staff: Bedroom #4 is designated for staff. Bathrooms: Facility has seven full bathrooms. All bathrooms have a working toilet, wash basin, walk in shower.Linens & Hygiene Supplies: Adequate supply of linen in closet storage.Emergency Phone Numbers, Exit Plan & Menu: Posted & readily available for review an emergency disaster plan with means of exiting and emergency phone numbers listed. Menus posted and available. Menus prepared one week prior and listed for food serve for one week. Food Service: Adequate supply of 7-day non-perishable and 2-day perishables are stored in the kitchen. Smoke Detectors: Smoke detectors and carbon monoxide alert Continued on LIC809-C systems are hardwired, were tested, and found operational. Fire extinguishers mounted in kitchen and resident bedroom hallway and charged. Appliances: Electric five burner stove, single oven, refrigerator, microwave, dishwasher, washer, and dryer are clean and noted to be operational. Toxins: All and any toxic chemicals, cleaning solutions and disinfectants are inaccessible to residents are stored in the laundry unit and garage. Water Temperature: Tested and recorded maintained at a comfortable temperature and the water temperature measures 116.6 Fahrenheit degrees in facility bathrooms. Medications, First-Aid Kit & Book: Medication and First Aid kit stored in locked storage closet. Resident & Staff Files: Records will be kept locked in a storage cabinet. Reading Material, Games, Equipment & Materials: The facility has board games, books, and other recreational materials for the resident's use, commensurate with the plan of operation. Fire clearance: Was approved on 12/19/23. Component III: Component three waived during visit. Applicant is Licensee/Administrator of other licensed facilities. The applicant has met all pre-licensing requirements. LPA will submit notification to CAB in Sacramento for final review prior to license being issued.
