On July 13, 2026, Licensing Program Analyst (LPA), Jarred Torres, arrived at the facility unannounced to conduct an annual inspection, and met with Administrator (AR), Jaime Apostol. A facility file review was conducted at the regional office, and additional records were requested and reviewed at the facility. Their files were stored in a secure location. The facility is licensed for four residents and was operating at a census of three. LPA toured the facility along with AR, and made observations pertaining to the annual inspection. LPA inspected the facility inside and outside. There were no obstructions to the indoor and outdoor passageways at the time of this visit. During the tour, LPA observed the facility's phone to be operational. The facility's phone number is 951-208-0978. LPA observed the residents' bedrooms to be equipped with the required furniture as stated in Tittle 22 of the California Code of Regulations (CCR). The furniture in all areas of the facility was observed to be in good condition. The facility's appliances were observed to be operational at the time of this visit. Required postings regarding safety, personal rights, and emergency exits were posted in the home. Sharp items were kept locked and inaccessible to residents in care. The facility's cooling and heating system is operational and the air temperature was at 78 degrees Fahrenheit (F) and the hot water was measured at 106 degrees F. Continued on LIC 809-C... LPA observed the medications to be locked and inaccessible to the residents in care. The medication supply was sufficient for their current census, and there were no discrepancies with the centrally stored medications. In the kitchen, the food supply of non-perishable and perishable foods consisted of, but not limited to, bread, milk, vegetables, eggs, juice, rice, pasta, and meat. The facility goes grocery shopping twice every week to maintain an appropriate food supply for the residents. The facility has a designated area for storing activity supplies, and activities are conducted inside and outdoors. Telephone numbers and floor plans were posted in the facility. Client files and staff files were reviewed and no deficiencies were observed. The files were securely stored. The administrator had a valid administrator's certificate on file. LPA reviewed the emergency disaster plan and fire clearance. The last emergency drill was on June 21, 2026. The fire alarms and carbon monoxide detectors were tested and operational. The fire extinguisher was in good condition and was inspected on January 14, 2026. The emergency disaster plan meets the department's standards. LPA inspected the bathrooms and observed the hand washing stations to be stocked with soaps, bath tissue, and single-use hand towels. Furthermore, the bathrooms are free of dust and debris. Additionally, the facility had an approved infection control plan in their files. During this annual inspection, LPA did not observe any health, safety, or personal rights risks to clients in care. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Janira Arreola, conducted an announced visit for the purpose of conducting the prelicensing inspection. LPA met with Applicants, Rosamila Apostol and Jaime Apostol who were informed on the purpose of the visit. The applicant is seeking a change in facility type application from an adult residential facility to a residential care facility for the elderly licensed for (4), (3) of which may be non ambulatory and (1) of which may be bedridden. The applicants requested a hospice waiver for (4) residents which is pending review. LPA conducted a walk through of the interior and exterior of the facility. The home is a (5) bedroom and (2) bathroom, one story home with attached garage. No pools or firearms are being kept at the facility. The current administrator has a current administrator certificate. The facility currently has liability insurance that was reviewed at the time of the visit. All current staff have criminal record clearance and documented training in their file. Each client has a designated file. LPA observed the rooms had the appropriate linens, furniture such as bed, dresser, closet space, light, night stand and chair. The home has extra linens and bath towels. Locked medication storage will be kept in the kitchen. Emergency supplies such as first aid kit with all requirement items were observed. The facility kitchen has enough pots and pans, cooking utensils, plates and cups. The kitchen had the appropriate food items. LPA observed the bathrooms in the facility to have hand hygiene supplies, and personal care items. The outdoor area is free of hazards. All facility exits are clear of obstructions. The hot water temperature was measured at 105F and the carbon monoxide and smoke alarms are operation at the time of the visit. There are no objections for the applicant to move forward in the licensing process. No immediate health or safety issues were observed during the time of the visit.
