Type of visit: Required - 1 Year

On 05/22/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Facility Designated Administrator (FDA), Victoria Kachan and explained the purpose of the visit. The purpose of the visit was to conduct an annual visit. Current census was 6. A brief interview with FDA Kachan was conducted. This facility is licensed to serve and retain 6 elderly residents. This facility has a dementia plan on file and a hospice waiver for 6. Bedrooms #1&3 may only have non-ambulatory residents. Bedrooms #2,4,5, and 6 may have bedridden residents. Bedroom #7 is for staff use only. It was learned that the facility has 1 resident on receiving hospice services. LPA Pascua reviewed 6 resident files and 6 staff files. All files were complete and up to date. FDA Kachan has an active administrator certificate #7016512740 and expires on 08/05/2026. The facility has conducted a fire drill and disaster training on 04/20/2026. A tour of the facility was conducted. A tour of the facility was conducted. Fire extinguisher located by the kitchen appeared to have been serviced by the local fire extinguisher company, Cinta, on 02/10/2026 and is in compliance. The kitchen area was toured. LPA Pascua observed a sufficient seven days of non-perishable as well as two days worth of perishable food supplies in the main kitchen. Additional perishable supplies were identified in an additional refrigerator located in the garage. Knives and additional cleaning supplies were locked and made inaccessible to the residents at this time. LPA Pascua observed a locked centralized stored medication cabinet. Along with Administrator, the LPA observed, reviewed, and compared resident medication with the medication dispensing logs. First Aid Kit was present and contained all of the required components. Common areas were toured. Living room, dining area and all other areas intended for resident use were in compliance and good repair. A tour of the garage was conducted. Additional storage for supplies were stored in cabinets. A washer and dryer were also identified in the garage. Laundry detergent, bleach, and all other cleaning supplies were observed to be locked and made inaccessible to the residents at this time. A tour of the bathrooms was conducted. Hot water temperature was measured and observed to be within the required range of 105-120 degrees. Grab bars were present and in good repair. A tour of the resident bedrooms was conducted. Resident furniture was observed to be sufficient to meet the resident needs at this time. A linen closet was located in the hallway and presented a sufficient amount of linens to adequately supply and meet the needs of the residents at this time. Additional incontinence supplies were also identified. The exterior of the physical plant was in good repair with no hazards present. Perimeter fence was observed to be stable and gates were in good repair. The following forms and documents were requested to be updated and submitted into CCL. -LIC 308 -LIC 400 -LIC 500 -LIC 610 Based on the observations made during today's visit, there are no deficiencies were observed or cited during this annual visit. Type of visit: Required - 1 Year

Licensing Program Analyst(s) Sommer Hayes and Kimberly Viarella arrived unannounced to conduct the 1-year Annual Inspection visit on 04/21/25. LPAs introduced themselves, stated the purpose of their visit and met with the Designated Facility Administrator, Victoria Kachan. LPAs compared the LIC 500 (the staff roster) with the Guardian roster to ensure that all employees had their required clearances to care for the residents. All were in compliance at the time of this inspection. The inspection began with a review of the medications which were located in locked cabinets and inaccessible to residents in care. LPA reviewed storage, administration and destruction procedures with Kachan. The first aid was also inspected and contained all the required elements to be in compliance. LPA Hayes inspected the food supply and found there to be a 7 day supply of non-perishable and a 2 day supply of perishable food items. All knives and sharps were locked and inaccessible to residents in care. LPA observed that the fire extinguisher was last inspected on 02/11/25 by River City Fire Co. and was in compliance at the time of this inspection. LPA Hayes then inspected all of the resident rooms to ensure that they contained the required furniture, furnishings, and lighting . All rooms were in compliance at the time of this inspection. LPA inspected 3 bathrooms and observed that they all contained the required soap, paper towels, grab bars and trash cans as required at the time of this inspection. Hot water was measured and found to be 107 degrees Fahrenheit and in compliance. The exterior of the building was also inspected by LPA Hayes. All windows, window screens, and gutters were in good repair and there was a shaded area with seating for residents to enjoy. LPAs reviewed a sample of one staff and two residents. According to California Code of Regulations Title 22, no deficiencies were cited during this visit. Exit interview held. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to conduct a Required - 1 Year visit on 3/15/24 at 1:00pm. Administrator certificate expires 8/5/24. LPA met with Victoria Kachan, Administrator and stated the purpose of todays visit. The facility is licensed for a capacity of 6 non-ambulatory residents of which 6 may receive hospice care services and 4 maybe bedridden. There is 1 residents receiving hospice care services during this visit. LPA toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents. LPA observed 2-day perishables and 7-day non-perishables. The temperature inside the facility was observed to be at 71*F which is within the required range of 68-85*F. The hot water temperature was measured at 106.8*F which is within the required range of 105-120*F. LPA observed fire extinguisher(s), smoke and carbon monoxide detectors, and central heating and air in the facility. The most recent disaster drill was conducted on 1/18/24. LPA observed the centrally stored medications area to be locked and inaccessible to residents. The first aid kit contained the required items such as sterile dressings, bandages, adhesive tape, scissors, tweezers, thermometers, antiseptic solution and guide. LPA reviewed 2 staff and 2 resident files and conducted interviews during this visit.
