On 11/17/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA met with Administrator Okgi Min (AD), and stated the purpose of the visit. Overview: Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 6 elderly residents. Facility is cleared to accept up to 6 non-ambulatory, 2 of which can be bedridden status. Facility is approved to retain/accept up to 3 residents receiving hospice services. Facility has fire sprinkler system installed. Initial Observation: Upon arrival LPA observed AD with one staff on duty. All 6 residents were present. LPA observed required posters and facility license at the entrance. Room temperature was at 74 degrees Fahrenheit upon arrival. Physical Inspection: Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected 5 resident bedrooms and the 3 bathrooms. LPA measured the hot water temperature in the 1 of 2 bathrooms to be at 105 degrees Fahrenheit. Resident bathrooms were observed to be in clean and good repair at this time. Fire extinguisher was observed in the kitchen area and was last inspected on 8/13/2025. Smoke and carbon monoxide detectors were observed throughout. LPA observed centrally stored medications, toxins, sharp objects and other dangerous items were kept locked and inaccessible to residents in care. {1 of 2} In the kitchen area, LPA observed sufficient seven day non-perishable and two day perishable food supplies. Pantry was observed to be fully stocked with non-perishable food items. Proper storage of food items were observed. Kitchen refrigerator and freezer were maintained at regulatory temperature. The garage houses additional food supplies and chemicals were observed to be locked. Outdoor area was inspected. LPA observed outdoor furniture for resident use. Ramps were observed to be in good repair at this time. Emergency walkways were observed to be unobstructed. Fence and gate were in good repair. Record Reviews: Review of 2 of 6 resident files was conducted, included review of Admission Agreement, Physician Reports, Needs and Services Plan, Centrally Stored Medication Record and Ambulatory Status. No issues were noted at this time. Review of 2 staff files, included review of background clearance, First Aid/CPR certificate, Health Screen, Initial and Ongoing Training. No issues were noted at this time. LPA also reviewed fire drill/disaster drill records; facility conducts quarterly drill. Documents Requested: LPA requested a copy of current Liability Insurance Certificate, LIC500, LIC308 and updated Dementia Care Plan to be emailed to arvin.villanueva@dss.ca.gov. Type of visit: Case Management - Other

On 11/22/24, at 4:03 pm, Licensing Program Analyst (LPA) Arvin Villanueva arrived unannounced to this facility to conduct a case management visit for the purpose of delivering an Order to Individual of Immediate Exclusion from all facilities and the Order to Licensee/Facility of Immediate Exclusion from Facility. LPA met with Okgi Min, Administrator, and explained the purpose of the visit. Staff (S1) is excluded as a result not related to this facility. Per Administrator, S1 resigned on 11/15/24. LPA served notice of "ORDER TO LICENSEE/FACILITY OF IMMEDIATE EXCLUSION FROM FACILITY" for S1 who was not present at the time of visit. Administrator was advised an immediate removal is warranted and requested the Personnel Report (LIC500) and Guardian account be updated to remove S1 from the facility staff roster. A notice of completion shall be submitted to Community Care Licensing (CCL). LPA informed the Administrator that S1 is not allowed to be employed and/or on any facility premises. The Order to Individual of Immediate Exclusion From All Facilities will be in effect as of 11/22/24 upon receipt of the letter. A copy of the letter was given to the facility during this visit. The facility understands this is an Immediate Exclusion and has agreed S1 cannot be allowed to work, live in, and/or have contact with clients in any residential facility licensed by the California Department of Social Services unless otherwise ordered by the Department. Type of visit: Case Management - Annual Continuation

On 11/12/24, Licensing Program Analyst (LPA) Arvin Villanueva conducted an unannounced Case Management - Annual Continuation visit at the facility to continue with the Annual Required Inspection visit initiated on 11/7/24. LPA with Administrator (Adm), Okgi Min, and stated the purpose of the visit. Present during today's visit were 5 residents in care with 2 staff on duty (Adm and S1). The LPA continued with facility visit to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. Facility is fire cleared for 6 non-ambulatory residents, 2 of which may be bedridden. Note that this facility is equipped with fire sprinkler system. Review of 3 sample resident files (R1, R2, R3) include review of Admission Agreement, Physician Reports, Needs and Services Plan, Centrally Stored Medication Record and Ambulatory Status. No issues were noted at this time. Medication review of 2 sample residents (R2 and R3) include review of physician orders for over-the-counter medications. No issues were noted at this time. Review of 3 sample staff files (S1, S2, and S3) include review of background clearance, First Aid/CPR certificate, Health Screen, Initial and Ongoing Training, and hospice training. No issues were noted. Per facility record review: staff conduct quarterly disaster drill and last drill was on 10/9/24; facility has a dementia care plan, infection control plan and updated Emergency Disaster Plan (LIC610E). Administrator provided current Liability Insurance Certificate, updated LIC500 and LIC308 during this visit. On 11/7/24, Licensing Program Analyst (LPA) Arvin Villanueva arrived to this facility unannounced to conduct an annual required inspection. LPA met with Administrator, Okgi Min, and stated the purpose of the visit. Upon arrival, there were 5 residents in care with 2 staff on duty. LPA evaluated the physical plant to ensure the health and safety of the residents in care. The facility is a one-story home located in a residential neighborhood. Facility is licensed for 6 non-ambulatory residents, 2 of which maybe bedridden. Facility is also approved for 3 hospice residents. Facility has a sprinkler system and a pull alarm in place. Areas inspected are including but not limited to the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor, clean and in good repair at this time. Fence and gate were observed to be in good repair. Facility has a covered patio at the back equipped with outdoor furniture for outdoor activities. Room temperature was observed at 75 degrees F. LPA observed 5 bedrooms to be equipped with the required furniture and sufficient lighting throughout the facility. Facility observed 2 1/2 bathrooms. LPA measured the hot water temperature measured at 110 degrees F in the hallway bathroom. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Food supplies were observed to be adequately stored. Fire extinguisher observed in the kitchen/dining was last inspected on 7/30/24. Smoke and carbon monoxide detectors were observed, tested and operable at this time. LPA observed centrally stored medications, toxins, and sharp objects were kept locked and inaccessible to residents in care. Fireplace is screened and not being used. Due to insufficient time, this annual visit will require a continuation visit. On 10/16/2023 at 10 am Licensing Program Analyst (LPA) Arvin Villanueva arrived at this facility unannounced to conduct a required annual inspection visit. LPA met the administrator in record Okgi Min and explained the purpose of the visit. Per administrator, facility is COVID-free status. Present during this visit are 4 residents in care and 1 staff members on duty. At 10:30am LPA and administrator toured the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, living area, garage, other common areas, and outside of the facility to ensure compliance with Title 22 regulations. Facility is a single-story home with a fire clearance to serve 6 non-ambulatory elderly residents. Facility is approved for 3 hospice residents and fire cleared for 2 bedridden residents (in rooms 1 and 2). Facility has 5 resident bedrooms and 2 and 1/2 bathrooms for resident use. Currently, 4 of the bedrooms are being occupied. LPA observed all bathrooms to contain grab bars, non-skid flooring, shower chairs, close lid trash containers and hygiene supplies. Resident bedrooms were sanitary, furnished, well-lit and had adequate storage for resident’s belongings. The facility common areas are cleaned and furnished. LPA observed the kitchen to be sanitary and free of clutter. Additionally, the kitchen knives and other sharp objects are kept in a locked drawer. Toxins and cleaning supplies are kept locked. LPA observed the facility to have adequate food supply with at least 2 days’ worth of perishables and 7 days’ worth of non-perishables. LPA observed the garage with 2 additional fridges for food storage, and functioning washer and dryer. LPA observed the dining room to be free of clutterl. The front yard and the backyard are observed to be free of obstruction and well-maintained. Additionally, the backyard is furnished with outdoor furniture for outdoor activities. Water temperature reads 105*F to 120*F in one of the bathrooms and room temperature reads 74*F. Smoke and carbon detectors were in good working condition. Fire extinguisher was serviced on 7/31/23. Medication storage area was observed to be locked and inaccessible to residents in care. Medication records were reviewed and in compliance with regulation. First aid kit was observed to have adequate supplies and accessible to staff. {Con't on LIC809-C} {Con't from LIC809} During this inspection 4 out of 4 resident files and 3 staffing files were reviewed for regulatory compliance. All files contained required contents including staff training requirements. All staff noted on LIC 500 contained criminal background clearances. Resident files reviewed contained all required contents including updated admission agreements, medical assessments, and updated appraisal forms as required. Facility’s liability insurance is current and update to date per regulatory requirements. LPA observed personal rights poster. Facility has appropriate internet access available for resident use. LPA observed sufficient equipment and supplies to meet activity program needs of residents in care. LPA reviewed facility’s disaster plan to ensure regulatory compliance. LPA's record review revealed evidence of quarterly fire drills. LPA also conducted the inspection using the CARE tool. The facility has an approved infection control plan in place. LPA requested an updated copy of Liability Insurance, facility sketch, LIC 308 and LIC 500. Per California Code of Regulations, Title 22 and Health and Safety Codes, no deficiencies were observed during this visit.
