On 01/22/2026 at 3:15 PM, Licensing Program Analyst (LPA) Shakaricka Hughes arrived unannounced to conduct a case management regarding a complaint control nbr: 27-AS-20250825132439 that was filed with CCLD unrelated to this facilities operations. LPA met with the facility administrator Gladys, and discussed the complaint allegations, and delivered findings. No citations were issues during today's visit. On 01/14/2026 at 3:45 PM, Licensing Program Analyst (LPA) Shakaricka Hughes arrived unannounced to conduct a case management regarding a complaint control nbr: 27-AS-20250825132439 that was filed with CCLD unrelated to this facilities operations. LPA toured the inside of the facility, and spoke with the facility administrator (S1) and facility staff (S2). LPA observed (2) residents in their rooms watching television, and (2) residents watching television in the common area. LPA conducted an interview with resident (R1) during the visit. Type of visit: Prelicensing

On 10/16/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their pre-licensing inspection visit. LPA met with Administrator, Gladys Quiambao, and stated the purpose of the visit. The Licensee, Guida Velasco, was notified of the visit and arrived later during the visit. Overview: Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 6 elderly residents, up to 6 may be non-ambulatory. Facility is not cleared to accept/retain bedridden residents. Facility has a waiver to accept up to 6 residents receiving hospice services. Initial Observation: Upon arrival LPA was greeted by one of the staff on duty. The Administrator was also present. There were 4 residents present during this visit with 2 staff on duty. Room temperature was 74 degrees Fahrenheit. Physical Inspection: Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA observed the inside of the facility to be clean and in good repair at this time. LPA inspected 5 resident bedrooms and were observed to be equipped with the required furniture and sufficient lighting throughout. Bedrooms #4 and #5 on the facility sketch are currently vacant. LPA measured the hot water temperature in the 2 of 2 bathrooms to be between 105 and 111 degrees Fahrenheit. Both resident bathrooms were observed to be clean and in good repair at this time. One of the bathrooms was in bedroom #2 of the facility sketch. Each bedrooms has its own exit and ramp. Exit doors have alarms. Staff and Office rooms were also inspected. The office room stores medications and records. {1 of 2} Fire extinguishers were observed in the kitchen area and laundry area and were last inspected on 5/20/2025. Smoke and carbon monoxide detectors were observed throughout, tested and found operable at this time. LPA observed centrally stored medications, toxins, and sharp objects were kept locked and inaccessible to residents in care. In the kitchen area, LPA observed at least 7-day nonperishable and 2-day perishable food supplies. Pantry was observed to be fully stocked with non-perishable food items. Proper storage of food items was observed. Kitchen refrigerator and freezer were maintained at regulatory temperature. Garage stores additional refrigerator and freezer. Advisory was provided to switch the door knob so that it can be locked and not accessible to residents, only if they were to store chemicals and other dangerous items inside the garage. Laundry room was inspected and laundry supplies were locked. Outdoor area was inspected. LPA observed covered patio with outdoor furniture for resident use. Ramps were observed to be in good repair at this time. Emergency walkways were observed to be unobstructed. No bodies of water were observed. The storage shed was observed and did not find chemicals. Facility has one exit gate located at the garage side of the facility. Advisory was provided to lock all gardening tools. Shut-off valves were identified. Advisory was provided to licensee to ensure all staff can operate each shut-off valves, in case of emergency. Record Reviews: Review of 3 staff files (Administrator, S1, S2) include review of background clearance, First Aid/CPR certificate, Health Screen, Initial and Ongoing Training. Licensee will not be managing residents cash resources. Component III: LPA reviewed component III with Licensee and Administrator. Pre-Licensing is complete, and this facility has no deficiencie.
