Type of visit: Office
On 07/29/2026 at 11:00 AM, a meeting was conducted via Microsoft Teams. Those in attendance included Licensing Program Manager (LPM) Stephen Richardson, Licensing Program Analyst (LPA) Pang Lee, Licensee/Administrator Saima Ali, and Mobussar Ali, the Administrator's spouse. The purpose of the meeting was to discuss the facility's intent regarding a potential Change of Ownership (CHOW) and to provide guidance on the applicable licensing requirements. The following topics were discussed during the meeting: · The facility has had a census of zero since it was licensed. · The Licensee advised that there was a potential buyer interested in purchasing the facility; however, the buyer ultimately decided not to proceed with the Change of Ownership. · The Department explained the requirements regarding changes in a corporation or Limited Liability Company (LLC): - A transfer of 50% or less ownership is generally not considered a change in ownership. - A transfer of more than 50% (51% or greater) may constitute a change in ownership requiring a new application and licensure. - The Centralized Applications Bureau (CAB) is responsible for reviewing and processing changes involving corporate ownership of 51% or more. CONTINUED LIC 809-C · The Department discussed the Emergency Approval to Operate (EAO) process and explained that an EAO is reserved for emergency situations, such as the death of a licensee or a natural disaster, where immediate action is necessary to ensure continuity of care. Because the facility currently has no residents in care, there is no emergency that would warrant an EAO. Mobussar Ali asked whether the process could be expedited if residents were residing in the facility. The Department explained that the presence of residents alone does not constitute an emergency and would not qualify for expedited processing. · The Department advised the Licensee that the facility may request to place the license on inactive status until a final decision is made regarding the future of the facility, such as pursuing a Change of Ownership, voluntarily closing the facility, or admitting residents. The Department explained that while the facility is on inactive status: - No residents may be admitted. - A timeframe for inactivity provided to CCLD. - Annual licensing fees remain due but are assessed at a reduced rate. - The facility may not be leased or used as a regular rental property. The Licensee stated they would notify LPA Lee once a decision has been made. · The Department also discussed the option of maintaining the facility's active license regardless of whether residents are admitted. The Licensee was informed that the Department retains its authority to conduct inspections and that annual licensing fees remain due even if the facility has no residents in care. · If the facility is sold through a Change of Ownership (CHOW), the prospective owner must submit a LIC 200 Application for a Community Care Facility License and complete the licensing process before assuming responsibility for the facility. The Department further explained that if residents are in care during a CHOW, the current licensee remains legally responsible for the operation and care of the residents until the new license is issued. The Department also reviewed the requirement to provide at least 60 days' written notice to residents and their responsible parties, when applicable. CONTINUED LIC 809-C · The Department advised the Licensee not to sign or execute any agreements related to the sale, transfer, or ownership of the facility without first consulting CCLD or notifying LPA Lee. · The Department reminded the Licensee that resident records must be established and completed prior to admission. Required documents include, but are not limited to: - Tuberculosis (TB) documentation; - Health screening documentation; - LIC 603, Pre-Admission Appraisal; - LIC 603A, Resident Appraisal; - LIC 601, Identification and Emergency Information; and - LIC 625, Needs and Services Plan. · The Department also explained that the Centralized Applications Bureau (CAB) is responsible for reviewing and processing: - Initial license applications; - Change of Ownership (CHOW) applications; - Change of location applications; - Changes to the legal entity (corporation, LLC, partnership, etc.) Based on today's meeting, no deficiencies were cited. An exit interview was conducted with Saima Ali and Mobussar Ali. Dec 2, 2025Facility evaluation reportReport on file
Type of visit: Prelicensing
On 12/02/2025, Licensing Program Analyst (LPA) Pang Lee arrived announced to conduct a Pre-Licensing Inspection follow-up of the facility to ensure compliance with Title 22 regulations. LPA Lee met with applicant Saima Ali who assisted in today’s inspection. Saima Ali will be the administrator of this facility. Administrator certificate number # is 6072293740 and will expire on 10/20/2026. The facility has a fire clearance for 6 non-ambulatory residents to accept and retain at any given time. The facility will not employ staff who are live-in and will ensure that there are care in place for 24/7 for the residents. There were no residents in care at this time. LPA Lee informed applicant that the physical plant cannot be changed without notifying the department and that the submitted facility sketch needs to be adhere at all times. LPA Lee toured the facility with applicant Saima and observed no health and safety concerns. During today's visit, the following corrections were completed: • Applicant installed handrails on the front entry ramp. • Applicant leveled the step and added a ramp between the dining area and the common area to allow easy accessible passage. • Applicant posted Pub 475, If You See Something Say Something in the required size of 20" x 26". LPA Lee reviewed Component 3 with the applicant. The applicant has passed the pre-licensing component of the application process. LPA Lee will notify the CAB that the pre-licensing has been completed and passed. Nov 20, 2025Facility evaluation reportReport on file
Type of visit: Prelicensing
On 11/20/2025, Licensing Program Analyst (LPA) Pang Lee arrived, announced, to conduct a Pre-Licensing Visit. LPA met with Applicant Saima Ali and Mobussar Ali and explained the purpose of the visit. This Pre-Licensing inspection was conducted due to a change of ownership. The current census is zero. A brief interview with Applicant Saima Ali and Mobussar Ali was conducted. The facility is a single-story home with a fire clearance for six (6) non-ambulatory residents. Upon arrival, LPA observed a ramp at the front entrance that did not have handrails. LPA advised the applicant that a sturdy handrail should be installed for resident health and safety. Dining areas, living areas, and other resident-use areas were toured. LPA reviewed the facility sketch dated 10/09/2025 and confirmed that it aligns with the physical plant, both interior and exterior. LPA observed that the Complaint Poster (Pub 475) was not in the required size of 20" x 26" and informed the applicant of the size requirement. A kitchen tour was conducted. LPA observed a sufficient supply of food, including at least a 2-day supply of perishable items and a 7-day supply of non-perishable items. Knives were locked and inaccessible to residents. The pantry was also inspected; additional non-perishable food supplies were stored there. A locked centrally stored medication cabinet was located inside a kitchen cabinet. The First Aid Kit was present and contained all required components. The smoke and carbon monoxide detectors are in compliance with fire safety. A fire extinguisher was located in the dining room and is serviced and valid through 10/30/2026. CONTINUED LIC 809-C LPA observed that the dining room entrance leading to the common area contains a step, and the turn into the common area measures 35 inches. LPA advised that this area should be leveled to ensure wheelchair accessibility. A tour of the garage was conducted. The laundry units are located in the garage, and laundry detergent, bleach, and cleaning supplies were secured and inaccessible to residents. Four resident bedrooms were toured. Furniture and furnishings were in good repair. The entry door to Bedroom #1 measured 29.5 inches. LPA advised the applicant that bedrooms must be large enough to allow easy passage and provide comfortable living accommodations. Resident bathrooms were inspected, and hot water temperature was tested in a resident bathroom. The hot water measured 105.8 degrees Fahrenheit. The facility temperature measured at 72 degrees Fahrenheit. The exterior of the facility was in good repair with no observed hazards. The perimeter fence was stable, and gates were functioning properly. The following corrections will need to be completed before being licensed: • Install handrails on the front entry ramp • Level the step between the dining area and the common area to allow accessible passage • Post Pub 475 in the required size of 20" x 26" Due to the observations made during this Pre-Licensing visit, the applicant has not passed the Pre-Licensing component of the application process. The applicant will make the required corrections and notify LPA once they have been completed.
