On 01/26/2026, Licensing Program Analyst (LPA) Pang Lee arrived at the facility to conduct an unannounced annual inspection. LPA Lee met with Administrator Carmen Navarro and explained the purpose of the visit. The current census is 5 with 2 facility staff. This facility is a single story building licensed to served six (6) non-ambulatory clients. LPA Lee inspected the physical plant including but not limited to the common area, kitchen, dining area, client bedrooms, client bathrooms, laundry room, garage, staff room and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPA Lee observed the facility to be free of odor, clean and in good repair. LPA Lee observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present. LPA Lee toured the kitchen and observed sufficient seven-day non-perishable and two-day perishable food supplies. Hot water temperature was measured at 115.9 degrees Fahrenheit in client bathroom sink, which is within the required regulation of 105 to 120 degrees Fahrenheit. The smoke and carbon monoxide detectors are in compliance with fire safety. The fire extinguisher is located in kitchen and was last serviced on 12/10/2025. The last fire drill was conducted on 12/15/2025. LPA Lee observed the facility has a has a public telephone in the kitchen and the facility has the required posters posted. Facility thermostat was observed at 73 degrees Fahrenheit, which is within the required regulation of 68 to 85 degrees Fahrenheit. LPA Lee observed toxins located in the kitchen sink and kept locked and inaccessible to client. LPA Lee observed sharp knives kept locked in the kitchen cabinet and inaccessible to client. LPA Lee checked medication storage and found medication to be locked away and inaccessible to client. The first aid kit was checked and contained the required components. CONTINUED LIC 809-C LPA Lee audited medications for 2 out of 5 clients by comparing the medications on hand with their Medication Administration Records (MARs) and it was complete. LPA Lee audited the Personal and Incidental (PNI) funds for all five clients, and the records were found to be incomplete. Four of the five clients’ PNI balances were over by amounts ranging from $0.01 to $3.50, and one client’s PNI balance was under $0.75. It was learned that the administrator did not have exact change available. During the audit of a client’s PNI, LPA Lee observed the administrator holding additional money being added to client’s money. When questioned about who’s money it was, the administrator was stated that the money was her personal funds, taken from her handbag. LPA Lee informed the administrator that client funds cannot be commingled with the clients PNI.LPA Lee reviewed 5 out of 5 client files, and they were complete. LPA Lee reviewed 2 staff files, and it was also complete. LPAs reviewed staff criminal record clearances, and a review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility. On 01/06/25 at 8:30 AM, Licensing Program Analyst (LPA) Pang Lee arrived at the facility to conduct an unannounced annual inspection. LPA Lee met with direct care giver Cynthia Navarro who then called the facility designated administrator (FDA) Carmen Navarro to informed that CCLD was present in the facility. FDA arrived approximately 30 minutes later. Administrator assisted with today’s visit. The current census is 5 with 2 facility staff. This facility is a single story building licensed to serve six (6) non-ambulatory residents. LPA Lee inspected the physical plant including but not limited to the common area, kitchen, dining area, resident bedrooms, resident bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPA Lee observed the facility to be free of odor, clean and in good repair. LPA Lee observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present. LPA Lee toured the kitchen and observed sufficient seven-day non-perishable and two-day perishable food supplies. Hot water temperature was measured at 112.3 degrees Fahrenheit in resident bathroom sink, which is within the required regulation of 105 to 120 degrees Fahrenheit. Grab bars and non-slip mat were observed to be stable and in good repair at this time. Smoke and carbon monoxide detectors are in compliance with fire safety. The fire extinguisher is located in common area and was last serviced on 01/03/25. The last fire drill was conducted on 12/20/24. LPA Lee observed the facility has a has a public telephone in the kitchen and the facility has the required posters posted. Facility thermostat was observed at 72 degrees Fahrenheit. LPA Lee observed toxins located in resident bathroom cabinet and in the laundry room and they both were not kept locked and made accessible to residents at this time. LPA Lee observed sharp knives kept locked in the kitchen cabinet and inaccessible to residents. Continued LIC 809-C LPA Lee checked medication storage and found medication to be locked away and inaccessible to residents. LPA Lee reviewed and compared 3 out of 5 medication administration record (MAR) and it was complete. The first aid kit was checked and contained all the required components. LPA Lee requested resident and staff files for review. LPA Lee reviewed 5 out of 5 resident files and they were not complete. 2 out of 5 resident files did not have a current Individual Progress Plan (IPP) and the most current IPP in the file are dated for 2022. LPA Lee reviewed 3 staff files and it was not complete. Staff 1 (S1) is missing 20 hours of annual training for 2024 and S2 did not have any documentation of dementia training prior to caring for dementia residents in the facility. LPA Lee reviewed staff criminal record clearances and a review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility. Type of visit: Case Management - Other

On 12/27/2023 at 12:10 PM Licensing Program Analyst (LPA) Pang Lee arrived at facility location unannounced. LPA Lee was met by care staff Cynthia Navarro and explained the purpose of today’s visit. Care staff then called administrator Carmen Navarro. Administrator arrived approximately 40 minutes later. Today’s visit is in regard to a recent facility action report (FAR) from Alta Regional Center. A brief tour of the facility was conducted. LPA reviewed a copy of the FAR that was received from Alta. Upon review of the FAR, it was learned that client 1 (C1) was seated in a wheelchair near a window with a gait belt attached. When care staff was questioned regarding the gait belt, it was learned that there was no medical prescription for the use of the gait belt nor behavioral support documentation for its use. It was also learned that the facility was given corrective action plans for deficiencies cited under CCR Title 17 section 56054(a)(1) and section 56054(a)(3). It was also learned that the facility was placed on sanctions by ALTA Regional until all corrective action plans are completed in full. During today’s visit administrator, Carmen was able to provide LPA Lee a copy of a note from (C1) Physical Therapist, Jolie Gonzalez authorizing (C1) using the gait belt. It was also learned that Jolie have trained (C1) caregivers to use the gait belt to assist (C1) with transfers and ambulation. Per Jolie the belt is donned around (C1) waist to help with balance to prevent a fall and is not attached to (C1) chair to prevent (C1) form getting up or used in anyway as a restraint. Based upon review of information obtained Per California Code of Regulations, Title 22, no deficiencies were observed during this visit. On 12/05/2023 at 1:05 PM Licensing Program Analysts (LPAs) Pang Lee and Arvin Villanueva arrived at the facility to conduct an unannounced annual inspection. LPAs met with direct care staff, Cynthia Navarro, who then called administrator, Carman Navarro. Administrator arrived approximately 20 minutes later. LPAs explained the purpose of the visit. Administrator assisted with today’s visit. Administrator certificate # is 6022122740 and will expire on 12/14/2024. The current census is 4 with 2 facility staff. This facility is a single story building licensed to serve six (6) non-ambulatory residents. LPAs inspected the physical plant including but not limited to the common area, kitchen, dining area, resident bedrooms, resident bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPAs observed the facility to be free of odor, clean and in good repair. LPAs observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present. LPAs toured the kitchen and observed sufficient two-day perishable and seven-day non-perishable food supplies. LPAs also observed hygiene products stored in the pantry with the non-perishable food. Hot water temperature was measured at 110.0 degrees Fahrenheit in resident bathroom sink, which is within the required regulation of 105 to 120 degrees Fahrenheit. Grab bars and non-slip mat were observed to be stable and in good repair at this time. Smoke and carbon monoxide detectors are in compliance with fire safety. The fire extinguisher is located in common area and was last serviced on 01/12/2023. The last fire drill was conducted on 11/18/2023. LPAs observed the facility has a has a public telephone in the kitchen and the facility has the required posters posted. The facility has an infection control plan and an emergency disaster plan. Facility thermostat observed at 72 degrees Fahrenheit. LPAs observed toxins located in laundry and kept locked and inaccessible to residents. LPAs observed sharp knives kept locked and inaccessible to clients. Continued LIC 809-C LPAs observed the sliding door and window in the recreation room are missing window screens. LPAs checked medication storage and found medication to be locked away and inaccessible to residents. LPAs reviewed and compared 4 out of 4 medication administration record (MAR) and it was complete. The first aid kit was checked and contained all the required components. LPAs requested residents and staff files for review. LPAs reviewed 4 out of 4 resident files and 4 out of 4 staff files and they were complete. LPAs reviewed staff criminal record clearances and a review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility.
