Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Licensee Clifford Tan and Administrator Ford Tan to conduct an annual inspection utilizing the inspection tool. Facility recently re-opened to residents after a fire that happened in February 2025. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed resident rooms, common area bathrooms, common areas, kitchen and perimeter of care home. LPA observed rooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition and properly maintained. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two day perishable and (7) seven day non-perishable food supply on hand. Smoke detectors and carbon monoxide detectors are operational in the care home. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed three (3) resident files, two (2) staff files and resident medications. Facility has a current copy of certificate of liability insurance and LPA obtained a copy. As a result of this visit, no deficiencies were cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Licensing Program analyst (LPA) Cassandra Mikkelson met with Ford Tan during visit. The facility is ready to reopen after fire that occurred in February 2025. LPA toured facility and observed no deficiencies. Fire Department has approved floor plan for six (6) non-ambulatory residents. Multiple topics discussed regarding reopening process. Type of visit: Required - 1 Year

On 5/7/2025 LPA Tryon visited the home. The house burned on 2/3/2025, and water damage from the fire fighters destroyed everything in the house. Residents were moved partially to Alder Grove Senior Living, and partly to other facilities. At the moment, the house is gutted/torn apart. The plan is to tear down the house, and rebuild a new house on the footprint of the old house. The licensee is still working with the insurance company and so forth. At this time, there are obviously no residents or staff at the house. Licensee plans to keep the license and rebuild the house. Type of visit: Case Management - Other

On 2/6/2025 LPA Tryon visited the licensee's private home, where 4 of the residents from Alder Grove Senior Living II are staying temporarily. LPA toured the house. The house has 4 bedrooms/2 bathrooms, a large kitchen with 2 refrigerators and plenty of cabinets/cupboards. There are cleaners under the sink in hall bathroom. Mr. Tan assured LPA that the handyman is coming this afternoon to place locks where needed such as under sink cabinets in bathrooms and kitchen, potentially hazardous items, etc. Bedrooms are spacious. 2 of the rooms currently have large beds, which will be replaced by smaller beds if the decision is made to apply for a license for this house, as the space is a little tight right now, but would be fine with smaller beds. There is a hot tub currently on the back patio, which Mr. Tan said will be removed if application is pursued. There is currently no water inside, and a cover over it. Also, the back yard slopes off at maybe a 45 degree angle at the back; a fence would need to be constructed There is plenty of food in the house, furnishings are appropriate, etc. Mr. Tan is meeting with the fire department this afternoon to have them do a pre-inspection of his house to see if it is feasible to convert it to a care home. LPA spoke with 2 verbal residents who were available. Both said they are doing well, staff is taking good care of them; the staff got them out of the care home quickly during the fire and it went well. They told me they are okay with the situation, and they wish to stay with Mr. Tan. Mr. Tan will contact CCL as soon as he learns what the fire department says; and with the decision to go forward with an application for this home or not. Type of visit: Required - 1 Year

On 4/9/2024 LPA Tryon visited the facility to do an annual visit using the CARE Tool. LPA met with Licensee/Administrator Clifford Tan. LPA toured the facility including common areas, kitchen, food storage, laundry room, bedrooms, bathrooms, hallways, outside areas. The home is nicely decorated and furnished, is clean and in good repair. Food supplies are adequate to meet the requirement of 2 days perishable and 7 days non-perishable. Medications are centrally stored and locked. Cleaners and chemicals are locked. Smoke detectors/carbon monoxide detectors installed and functional. Fire extinguishers present and charged. No hazards noted. LPA Reviewed 2 resident files and 2 staff files; Interviewed one staff and 2 residents LPA reviewed the CARE Tool with licensee.
