Type of visit: Case Management - Incident
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 6/23/26 to conduct a case management inspection to follow up on a choking incident on 6/2/26 for resident, R1 at the facility. LPA met with Staff, Hector Zarate and explained the purpose of the visit. Facility submitted incident report to department on 6/2/26 about resident, R1 who had choking incident on 6/2/26 in the dining room around 12:30PM. Facility staff took appropriate measures and performed Heimlick Manuever on R1. R1 was back to their baseline after this incident. Facility notified R1s responsible party, hospice agency, physician and other required agencies as required. After reviewing the incident report and information gathered, it has been determined that facility took appropriate measures to address R1s choking incident on 6/2/26. Type of visit: Required - 1 Year
On 04/23/26, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA met with staff and explained the purpose of the visit. Business Development Assistant, Lauren Louderback came after short while and assisted LPA with today's visit. LPA and staff conducted a tour of the facility for inside and outside area. Areas toured included but not limited to the kitchen, dining room, residents bedrooms, bathrooms, common areas and backyard. LPA observed the facility to have sufficient food supplies for seven (7) day non-perishable and two (2) day perishable. LPA observed toxins, knives and centrally stored medications to be locked and inaccessible to residents in care. All required Licensing posters are present in common areas in the facility. Hot water temperature was measured at 105 degrees Fahrenheit at the kitchen sink, which is within the required range of 105 to 120 degrees. The temperature in the facility was 70 degrees. Fire extinguishers was last inspected on 11/14/25 and were ready for emergency use. Facility has a pool in the backyard which was found to be locked and properly secured during inspection. Facility has working smoke detectors and carbon monoxide alarm as required. Facility is conducting quarterly fire and disaster drills. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed two residents and two staff files. Residents and staff files were found to be complete. LPA completed the full care tool and no deficiencies were observed per Title 22 Regulations. Allegation investigated: Licensee allows uncleared staff to work in the facility. Licensee does not ensure staff have the required training.
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 01/06/26 to deliver complaint findings for above allegations. LPA met with Anatoliy Molitvenik, Administrator and explained the purpose of the visit. The department conducted records review ,facility observations and interviews to investigate the complaint. **Report continued on LIC9099-C** Unsubstantiated **Report continued from 9099.... Allegation: Licensee allows uncleared staff to work in the facility. UNSUBSTANTIATED LPA reviewed staff roster on file at the facility and observed that all staff listed received a criminal background clearance from the Department. Staff Interviews indicated that facility do not let staff work without obtaining a criminal background clearance. Interviews with residents and staff indicated that they have not observed any unfamiliar people at the facility and have no concerns regarding staff not obtaining a criminal background clearance prior to employment. Allegation: Licensee does not ensure staff have the required training. UNSUBSTANTIATED During investigation, LPA obtained and reviewed staff records for staff members S1, S2, S3 and S4. LPA also reviewed the facility's Plan of Operation in regard to staff training. LPA observed that S1, S2, S3 and S4 received training in accordance with Title 22, the Health and Safety Code, and the facility's Plan of Operation. Staff Interviews indicated that staff are trained in accordance with Title 22, the Health and Safety Code, and the facility's Plan of Operation. Residents Interviews indicated that they feel that staff are adequately trained at the facility and they have no concerns regarding staff training. Based on interviews conducted and records reviewed, the preponderance of evidence standards has not been met. Therefore, the above allegations are found to be UNSUBSTANTIATED. A finding that a complaint allegation is unsubstantiated means that, although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Allegation investigated: Staff handled resident in a rough manner resulting in bruising to the resident. Staff pulled resident's catheter.
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 01/06/26 to deliver complaint findings for above allegations. LPA met with Anatoliy Molitvenik, Administrator and explained the purpose of the visit. The department conducted records review ,facility observations and interviews to investigate the complaint. **Report continued on LIC9099-C** Unfounded **Report continued from 9099...... Allegation- Staff handled resident in a rough manner resulting in bruising to the resident. UNFOUNDED The department conducted record review, interviewed residents and staff to investigate this allegation. Record reviews did not indicate that residents R1 was bruised by staff manner. Three residents’ interviews reflected that they feel safe with staff and their care needs were met. Three staff interviews indicated that R1 bruised easily due to their health conditions and it was not due to staff’s lack of care. During complaint investigation, it was learnt that law enforcement came to do the welfare check on R1 regarding this allegation and there were no findings. Based on the information gathered, this allegation was found to be UNFOUNDED. Allegation - Staff pulled resident's catheter. UNFOUNDED The department conducted record review, interviewed residents and staff to investigate this allegation. It was learned that resident, R2 was receiving hospice care at the facility before they passed away under hospice care. Record review reflected that there were incident(s) where R2 would get agitated and pull out their catheter and staff notified hospice agency, physician, responsible party and other agencies and addressed the issue per R2’s care needs. Three staff interviews indicated that they were providing care to R2 per their needs and service plan and there were no concerns. Three resident’s interviews reflected that staff were meeting their care needs, and they were satisfied with their care at the facility. Based on the information gathered, this allegation was found to be UNFOUNDED. A finding that the allegations are Unfounded means that the allegations are false, could not have happened, and/or is without a reasonable basis. Exit interview conducted. On 04/22/25, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA met with Resident Care Manager, Lauren Louderback and explained the purpose of the visit. LPA and staff conducted a tour of the facility for inside and outside area. Areas toured included but not limited to the kitchen, dining room, residents bedrooms, bathrooms, common areas and backyard. LPA observed the facility to have sufficient food supplies for seven (7) day non-perishable and two (2) day perishable. LPA observed toxins, knives and centrally stored medications to be locked and inaccessible to residents in care. All required Licensing posters are present in common areas in the facility. Hot water temperature was measured at 118 degrees Fahrenheit at the kitchen sink, which is within the required range of 105 to 120 degrees. The temperature in the facility was 72 degrees. Fire extinguishers was last inspected on 11/15/24 and were ready for emergency use. Facility has a pool in the backyard which was found to be locked and properly secured during inspection. Facility has working smoke detectors and carbon monoxide alarm as required. Facility is conducting quarterly fire and disaster drills. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed two residents and two staff files. Residents and staff files were found to be complete. Facility was using Centrally Stored Log for medications administration as required. LPA requested a copy of the LIC500, LIC 308 and current liability insurance to be sent to LPA Bains by 05/10/25. LPA completed the full care tool and no deficiencies were observed per Title 22, CCR Regulations. May 20, 2024Facility evaluation reportReport on file
Type of visit: Prelicensing
On 05/20/24, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility announced to conduct a Pre- Licensing visit. LPA met with Administrator, Toly Molitvenik and explained the purpose of the visit. Co-administrator, Galina Chikivchuk was also present during this visit. The facility currently has no residents during today's inspection. Facility has a fire clearance for six (6) bedridden residents (dated- 04/23/24). LPA and Licensee conducted a tour of the facility. Areas toured include but are not limited to: six (6) residents bedrooms, seven (7) bathrooms, common areas, kitchen, dining area and backyard. LPA observed the facility to be clean and sanitary. The food supply is within compliance, two (2) days of perishable and seven (7) days of non-perishable food items. Smoke and carbon monoxide detectors are operational. The Fire extinguisher was last serviced on 01/18/24. Grab bars were present at the toilets and in the showers. All exits were unobstructed. All toxins, medications, and sharps were locked and stored away. Facility's temperature was 72 degrees Fahrenheit during inspection. Hot water temperature was observed to be 107 degrees Fahrenheit, which is within the regulation range of 105-120 degree. First aid kit found to be complete. LPA observed the facility to have the required posters of Community Care Licensing Division and Long Term Care Ombudsman. Pre-licensing passed and Component III was waived. Applicant has satisfied all requirements in accordance to Title 22, California Code of Regulations. Application is pending and LPA will forward findings to the Centralized Application Bureau (CAB) for final review and approval. CAB will further contact applicant on final status of application.
