On 08/24/2026 Licensing Program Analysts (LPAs) Raquel Hernandez and Matthew Aguilar conducted an unnanounced visit to facility to conduct annual inspection. LPA Hernandez knocked on door and no response. LPA called Administrator Emmanuel Garcia who stated all residents are at day program and no staff are currently at facility and will not be able to come to facility for another hour. Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to deliver findings on the allegation listed above. LPA met with Administrator Emmanuel Torres Garcia and House Manager Priscila Castanon and explained the purpose of the visit. The investigation consisted of staff and client interviews and request of documentation. For the allegation, Staff did not prevent client in care from being assaulted at the facility LPA interviewed Client #1 (C1) who stated C1 has not been assaulted at the facility. C1 reported the facility provides a safe living environment. Based on client interview, there was not enough evidence to corroborate client in care was assaulted while at the facility. Unsubstantiated Based on the evidence gathered during today’s investigation, the allegation listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Raquel Hernandez made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Administrator Emmanuel Torres Garcia and was granted entry to the facility. Licensed capacity is (6) current census (4). LPA was accompanied by Administrator Emmanuel Torres Garcia to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA tested water temperature to be at 107 degrees Fahrenheit. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster were observed. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. There was a designated office for resident/staff files. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Facility has a variety of food available for residents. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (2) resident files for admission agreements, updated physician reports, and needs and services plans. LPA reviewed (2) resident medications. **Continuation on LIC809C* No issues were observed. Additionally, LPA also reviewed (2) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings. No issues were observed. Based on the observations made during today’s visit, no deficiencies was cited per Title 22, Division 6, of the California Code of Regulations. LPA met with House Managers Priscilla Castnon and Emmanuel Torres and explained the purpose of the visit. The investigation consisted of staff and resident interviews. For the allegation, Facility staff hit client. LPA Hernandez conducted (2) staff interviews. 2 out of the 2 staff stated facility staff do not hit any of the rsidents in care nor have witnessed any facility staff hit residents in care. LPA Hernandez spoke with Resident #2 (R2) who stated they have not witnessed any facility staff hit any of the residents in care. Unsubstantiated Based on the evidence gathered during today’s investigation, the allegations listed above are deemed UNSUBSTANTIATED. Licensing Program Analysts (LPA) Beena Singh conducted an unannounced visit to the facility to deliver findings on a complaint alleging Staff hit Resident #1 (R1) LPA Singh met with Staff-Ashley castanon, facility representative, and was granted entry into the facility. The facility Manager-Emanuel Torres arrived during the visit. The investigation conducted by LPA Singh consisted of interviews and records review. For the allegation, Staff hit Resident LPA Singh conducted (2) resident interviews. 2 out of the 2 stated that no staff have ever hit resident#1 or any residents in care and have not witnessed staff hitting any residents in care. 2 residents were out in the community attending day program including Resident#1. LPA Singh conducted (2) staff interviews. 2 out of the 2 staff stated no staff has ever hit any residents in care. Unsubstantiated Based on the evidence found during the investigation, the allegations listed above are deemed UNSUBSTANTIATED. LPA met with House Managers Priscilla Castnon and Emmanuel Torres and explained the purpose of the visit. The investigation consisted of staff and resident interviews and request of documentation. LPA Hernandez conducted (4) resident interviews. 3 out of the 4 stated that no residents have physically abused them nor have witnessed other residents being abused by residents in care. 1 out of the 4 residents were unable to answer question. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with House Manager Emanuel Torres, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6), a current census of (4). LPA Ramirez conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility has no swimming pool or similar bodies of water. Outdoor shaded area is sufficient for resident activities and is enclosed with self-latching gates. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility is equipped with operating smoke detectors/carbon monoxide alarms, laundry equipment, and telephone service. Resident’s showers, toilets, and hand washing areas were operating in a safe and sanitary conditions. The hot water temperature in two (2) resident bathrooms measured at 111 degrees F. Four (4) resident’s bedrooms had beds, bed linen, chairs, dresser, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, complaint poster, Ombudsman poster, facility license, facility sketch, emergency telephone numbers, and staff schedule. Food Service: Facility kitchen and dining areas are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. Sharps and cleaning chemicals were kept locked and inaccessible to residents in care. Continuation on LIC – 809C: Care & Supervision: Facility has 24-hour/7days a week care staff. Facility staff have current CPR/first aid training. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet. Record Review: Four (4) Staff files reviewed were observed to be complete. Four (4) Resident files reviewed were observed to be complete. Based on observations and record review no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to conduct a complaint investigation regarding the above allegations. LPA Prieto met with Administrator Robert Cantoria and explained the elements of the complaint. Regarding the allegations that the facility staff not assisting client with dental hygiene needs; LPA Prieto interviewed resident #1 (R1) in question, who stated that they can handle their own dental hygiene needs and does so on a daily basis. R1's medical and assessment records indicate that R1 is able handle their own hygiene needs, including dental, with no assistance from staff. R1 has a dental service and LPA Prieto obtained those records to show that dental services are being provided. Facility administrator states they have collaborated other agencies to cover cost of R1's dental needs. ***continued on LIC 9099C** Unsubstantiated Regarding the allegation that the facility staff not ensuring clients have sufficient intake of food. LPA Prieto interviewed R1, who states that the facility staff does prepare food that R1 enjoys eating and looks forward to eating. LPA Prieto obtained a sample of the food being served to the residents for the last 3 days. Those meals consisted of a protein, greens, vegetables and protein drinks. Administrator ask staff to take pictures of food being served to assure residents are being prepared food that is nutritious and of good quality. Based on the information obtained there is not enough evidence that facility staff not assisting client with dental hygiene needs and facility staff not ensuring clients have sufficient intake of food. Therefore, the allegations are deemed UNSUBSTANTIATED at this time.
