Type of visit: Case Management - Incident
On 05/29/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced Case Management incident visit. LPA met with Staff Lizeth Alonzo and explained the reason for today’s visit. The visit is pertaining to an incident that occurred on 04/26/2026 pertaining to Client #1 (C1). On 04/28/2026, a special incident report (SIR) was submitted to licensing department in regards to possible neglect and dehydration happening to C1. During visit, LPA conducted (3) staff interviews. LPA observed C1 is non-verbal. LPA received additional information in regards to incident. LPA observed no imminent health or safety concerns. Facility had enough food and water available for all clients in care. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations regarding this incident. Allegation investigated: Staff does not ensure resident is hydrated.
Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit to investigate and deliver findings on the allegation listed above. LPA met with staff Jennifer Rodriguez and House Manager Lizeth Alonzo and explained the purpose of the visit. The Administrator was also contacted and informed about today’s visit. The investigation consisted of staff interviews, resident interviews, facility tour and record review. For allegation, Staff does not ensure residents are hydrated. During staff interviews, three out of the three staff stated that hydration is provided to all residents. During resident interviews, four out of the five residents stated that hydration is provided. One out of the five residents was unable to collaborate on the above allegations listed above. During facility tour LPA observed the facility had water and drinks for residents. Unsubstantiated Based on the evidence found during the investigation, the one (1) allegation listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Raquel Hernandez made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Administrator Robert Cantoria and was granted entry to the facility. The facility is a Residential Care Facility for the Elderly (RCFE). The facility is defined as level 6 home vendorized by Inland Regional Center (IRC). Licensed capacity is (6) current census (6). LPA was accompanied by Administrator Robert Cantoria to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster were observed. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. There was a designated space for resident/staff files. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Facility has a variety of food available for residents. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (3) resident files for admission agreements, updated physician reports, and needs and services plans. LPA reviewed (3) resident medications. **Continuation on LIC809C* No issues were observed. Additionally, LPA also reviewed (3) staff files for First Aid/CPR/CPI certification, criminal record clearance, training's, and health screenings. No issues were observed. Based on the observations made during today’s visit, no deficiencies was cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to deliver findings on the allegation listed above. LPA met with Staff Lizeth Alonzo and explained the purpose of the visit. The investigation consisted of staff and resident interviews. LPA Hernandez conducted (2) resident interviews. 2 out of the 2 residents stated they are treated with respect and feel safe living at the facility. Resident #1 (R1) stated they have not experienced physical, verbal, or emotional abuse by any facility staff. LPA Hernandez conducted (2) staff interviews. 2 out of the 2 staff stated they treat residents with respect and provide a safe environment for residents in care. Staff #1 (S1) stated they have never been physical, verbal, or emotional abusive to any residents in care and have not witnessed any other facility staff do so. Unsubstantiated Based on the evidence gathered during today’s investigation, the allegation listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to deliver findings on the allegation listed above. LPA met with Staff James Smith and explained the purpose of the visit. The investigation consisted of staff and resident interviews. For the allegation, Staff pushed resident while in care. LPA Hernandez conducted (2) resident interviews. 2 out of the 2 residents stated they have never been pushed by any of the facility staff or have witnessed other residents in care being pushed by facility staff. LPA Hernandez conducted (3) staff interviews. 3 out of the 3 staff stated they have not pushed any residents in care or witnessed any other staff doing so. Unsubstantiated For the allegation, Staff handles resident in a rough manner. LPA Hernandez conducted (2) resident interviews. 2 out of the 2 residents stated facility staff has never handled them in a rough manner or have witnessed facility staff handle other residents in care in a rough manner. LPA Hernandez conducted (3) staff interviews. 3 out of the 3 staff stated they have not handled any of the residents in care in a rough manner or have witnessed any other facility staff doing so. Based on the evidence gathered during today’s investigation, the allegations listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Ryan Gardner made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met Caregiver Lizeth Alonzo Garcia and was granted entry to the facility. During today’s visit, Administrator Robert C. Cantoria was phoned and arrived at the facility during the visit. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for a capacity of six (6) non-ambulatory clients. The facility is defined as level 4i home vendorized by Inland Regional Center (IRC). The current census is five (5) clients. LPA was accompanied by Caregiver to conduct a general overall inspection, which included, but was not limited to, the following: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to interior and exterior passageways. The facility is maintained at a comfortable temperature. LPA inspected client bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperature in the bathrooms to be at 112.2 degrees F. The facility is equipped with operating smoke detectors and carbon monoxide alarms. The postings such as the facility license, personal rights, labor laws, the CCL complaint poster, and the disaster plan were posted in a common area. The cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. There was a designated storage space for client files and staff files. The medications are kept inside a kitchen cabinet inaccessible to the clients in care. The non-perishable and perishable food supply is sufficient for the clients in care. LPA reviewed five (5) client files for admission agreements, updated physician reports, and needs and services plans. LPA inspected the client’s medications and the client’s medication records. LPA did not observe any medication discrepancies. LPA reviewed six (6) staff files for First Aid/CPR certifications, criminal record clearances, trainings, and health screenings. During document review, LPA found that Staff S1 has been working at the facility since 2018 without obtaining a criminal record clearance. The facility will be issued a type A deficiency for allowing S1 to work at the facility without obtaining a criminal record clearance. The facility will also be issued a $500-dollar civil penalty. During document review, LPA found that Staff S2 does not have CPR/first aid training. Staff S2’s CPR/First aid training expired on 1/16/2023. S2 was working alone in the facility when LPA arrived at the facility. The facility will be issued a type B deficiency for allowing S2 to work alone with the clients without CPR/First aid training. Based on the observations made during today’s visit, two (2) deficiencies were cited per Title 22, Division 6, of the California Code of Regulations, along with a $500-dollar civil penalty.
