Allegation investigated: Unlicensed Care.

Licensing Program Analyst (LPA) Angela Panushkina conducted a subsequent complaint visit to follow up on an Unlicensed Care Complaint. LPA met with the Operator/Administrator and explained the reason for the visit. Based on complaint review, the substantiated findings shall remain the same, however the civil penalty assessed on 07/23/2025 shall be dismissed. The Operator/Applicant submitted an application on 03/21/2025 and worked with the department to be licensed. As of 10/22/2025, Abundant Senior Care - #197610790 was licensed. Type of visit: Prelicensing

At 09:20am, Licensing Program Analyst (LPA) Angela Panushkina and Licensing Program Manager (LPM) Nichelle Gillyard, arrived at the facility to conduct Pre-Licensing/Component III. LPA and LPM met with the Administrator and explained the reason for the visit. A tour of the physical plant was initiated at 09:30am and the following was observed: The facility is cleared for five (5) Non-Ambulatory and one (1) Bedridden (room #1) residents. Kitchen: The kitchen area is equipped with a refrigerator, microwave oven and sink. LPA and LPM observed there to be sufficient stock of perishable and non-perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. All knives and resident medications were observed to be locked in the kitchen cabinet. There is a complete first-aid kit with all required supplies. The fire extinguisher was last services on 08/11/2025 and observed to be full. Bedrooms: The facility has a total of six (6) bedrooms, five (5) of which are designated for residents’ use, and one (1) room is designated for live-in staff. The bedrooms are appropriately furnished with sufficient closet space and have sufficient lighting. All exit doors will have an auditory alarm which was observed operational and in good repair. Bathrooms: Facility has three (3) bathrooms. Bathrooms have soap and paper towels. Extra towels and linens were readily available. The hot water temperature measured at 120°F. Continue on LIC809-C Common Area: The facility maintains a comfortable temperature of 73°F. LPA observed all common areas are furnished with adequate furniture to accommodate a maximum capacity of six (6) residents. There is a functioning telephone on the premises. Smoke detectors are located throughout the facility. There are also two (2) carbon monoxides (one is located in the hallway and the second is by the kitchen) and at 10:20am they were tested and observed to be operational. Laundry Room: The laundry is located in the garage. The washer/dryer appears to be in good condition. Laundry supplies are kept inaccessible when not in use with supervision. Outside Areas: The back of the facility has sufficient yard space. At 10:45am, LPA and LPM observed appropriate outdoor furniture, with a covered shaded area for the residents. LPA discussed the importance of maintaining care and supervision to meet the needs of residents. The outdoor area had random items on the right side of the home and LPA/LPM advised the Administrator to clean or place them in a storage to prevent immediate hazards. There are no bodies of water. Component III was also conducted at 11:30am. During today’s visit, LPA and LPM observed the following that must be corrected prior to licensure: · Remove all cameras · Place a lock under the kitchen sink cabinet · Place a lock on a hallway cabinet, by room #2 · Update the facility sketch: room #3 - staff and staff – office · All trash cans must have a lid · Random items in the backyard must be removed · Outside furniture cushions have to be replaced · Walls/doors to be cleaned and or repainted · Outlet in room #3 and the office has to have a cover plate. Administrator agreed to make corrections and submit proof of picture by November 1st, 2025. Once licensed the Administrator will purchase liability insurance and also submit a proof to LPA. Type of visit: Case Management - Health Checks

At 12:50pm, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced case management visit to this home. Upon arrival, LPA met with Staff #1, who granted access to the facility. LPA explained the reason for the visit. At 1:00pm, LPA conducted a tour of the facility and observed the home to be clean and have sufficient food supply. Per staff, there have not been any unusual incidents with the residents since the Departments last visit which was conducted on 09/11/25. During today’s visit, a staff member contacted the Administrator. The Administrator was not able to come to the facility and designated staff to sign for the report. LPA was informed that on 09/15/25 Component II was complete but the Fire Clearance is still on a pending status. LPA reminded the Administrator that no new residents shall be admitted at this time due to pending status. Type of visit: Case Management - Health Checks

At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced case management visit to this home. Upon arrival, LPA met with the Administrator and explained the reason for the visit. On 03/21/2025 an application for licensure was submitted. While the applicant requested a Fire Clearance for five (5) Non-Ambulatory and one (1) Bedridden residents, it was discovered (on 08/15/25) that this address (20601 Mayall St., Chatsworth, CA 91311) was under investigation with Los Angeles Department of Building and Safety (LADBS). Thus, the Fire Clearance was placed on hold. On 09/09/25, LADBS conducted their final inspection, and all pending permits were approved and submitted to the Regional Office (RO). During today’s visit, LPA was informed that a Fire Clearance was already requested, and the Administrator is currently waiting for the Component II which was already scheduled for 09/15/2025 at 11:00am. At 10:10am, LPA conducted a tour of the facility and observed the home to be clean and have sufficient food supply. LPA also confirmed that the home continues to provide care and supervision to four (4) residents. Per Administrator R1 passed away on 08/16/25 and a notification was submitted to the RO and no additional incidents with other residents had happened since. LPA reminded to Administrator that the application for Residential Care Facility for the Elderly (RCFE) license for this location is currently pending. LPA also reminded the Administrator that no new residents shall be admitted at this time due to the pending status. Allegation investigated: Unlicensed Care.

Licensing Program Analysts (LPAs) Angela Panushkina and Huma Rahimi conducted a subsequent complaint visit to this house to follow up on an Unlicensed Care Complaint. LPAs were greeted by Staff #1, who granted access to this home. LPAs contacted the Operator and explained the reason for the visit.The Operator was unable to come and designated S1 to sign the report. On 07/23/25, an initial complaint visit was conducted and interviews with the Operator and staff revealed that five (5) out of five (5) individuals residing in the home required and were receiving elements of care and supervision. Notice of Violation of Law (NOVL) was also issued, and Operator was advised to cease the operation. The purpose of today’s visit is to confirm that R1, R2, R3, R4 and R5 are no longer residing at this home. At 10:00am, during the physical plant tour, LPAs observed that five (5) out of five (5) of the aforementioned individuals that required care are still living in the home. Continue on LIC9099-C Substantiated At 10:15am, LPAs interviewed the Operator and were informed that they did submit an application on 03/21/25. However, the initial Fire Clearance request was for the capacity of six (6) Ambulatory only, which was denied on 07/18/2025. Although the second request for the Fire Clearance (STD850) had been submitted on 08/05/2025 for the capacity of five (5) Non-Ambulatory and one (1) Bedridden residents, the STD850 has not yet been approved nor received by the Community Care Licensing Division (CCLD). It was determined that the Operator has not ceased the operation and failed to relocate five (5) individuals residing in the home who required and are still receiving elements of care and supervision. Elements of care and supervision include assistance with Activities of Daily Living (ADLs), such as bathing, dressing, using the toilet, and arranging medical and dental services. Based on interviews and observation, it was determined that the Operator continues to provide unlicensed care for five (5) individuals residing in the house. Therefore, a $100 civil penalty per resident per day will be assessed retroactively from 07/23/25 and $200.00 (two hundred) per resident per day will be assessed beginning on the 16th day (08/07/25) until the operation is ceased. Deficiency/citation is issued on LIC9099-D. Exit interview conducted.
