Licensing Program Analyst (LPA) Jill Nakagawa conducted a complaint investigation regarding the allegation listed above. LPA arrived unannounced on this day for the purpose of delivering findings of the above allegation and met with Benjie Pulan, Care Staff with Paulin Pantig, Administrator (via phone). It is alleged facility is not meeting the resident’s nutritional needs. During the investigation LPA reviewed records, made observations at the facility on 3 separate occasions (10/21/2021, 10/29/2021, 11/03/2021) and conducted interviews and obtained photos. At today’s inspection, LPA observed that the facility was well-stocked and had a two day supply of perishable foods (including watermelon, pineapple, apaplesauce, juices) and a seven day supply of non-perishable foods as required in regulation Title 22, Division 6, Chapter 8, Article 10, Food Services. At approximately 9:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to deliver findings for the above allegation. LPA was met by Staff 1, and was asked Covid-19 screening questions and temperature was taken. LPA received copies of training records on 08/05/2021 that documented the required training hours for staff. At approximately 9:45AM, LPA reviewed additional training records at the facility. Staff are trained using a computerized training system as well as in person trainings that are conducted with a Vendorized instructor. Administrator Paulin Pantig arrived at approximately 10:25AM, and provided LPA with additional training records. This agency has investigated the above allegation. At approximately 12:30PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility to deliver findings from an investigation conducted by the department into the above allegation. LPA was met by caregiver and screened upon entry. Based on interviews conducted and a review of records the above allegation is found to be Substantiated. Licensee did not ensure medical attention was sought in a timely manner. A fall occurred between approximately 7:30AM and 8AM on 3/16/2021, resulting in an injury to a resident. Emergency medical personnel were not contacted until 9:41AM, approximately 2 hours after the fall occurred. Based on the Departments investigation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
