LPA arrived at the facility, met with Medication Technician Dora Islas and explained the reason for the visit. Administrator and House Manager were not at the facility. On 1/21/2025, LPA conducted an initial complaint investigation visit at the facility. LPA conducted an interview with staff and collected information and documents from the House Manager. During today’s visit, starting at 10:26 a.m. LPA conducted interviews with four staff and a visitor. Allegation investigated: Staff inappropriately touched residents in care

On 01/17/2024, Licensing Program Analyst (LPA) Brian Phillips conducted an unannounced subsequent complaint investigation visit to the facility above. LPA arrived at the facility, met with Direct Support Staff Catalina Gonzalez in person and Operations Manager Angelica Arambulo by telephone as the administrator was unavailable, and announced the purpose of the visit. On the allegation: Staff inappropriately touched residents in care. It is alleged that a facility staff, Staff #1 (S1), has been sexually abusing and inappropriately touching Resident #1 (R1) and other residents in the facility. The Reporting Party (RP) stated that they were informed of this allegation by Witness #1 (W1), but it was unknown how W1 learned this information. The names of the alleged victims were not known, and it was unknown if there were any witnesses to corroborate the allegation. Licensing Program Analyst (LPA), Esther Cortez conducted a subsequent complaint visit for the above allegations. Upon arrival, LPA met with staff and was explained the reason for the visit. Office Manager Angelica arrived at the facilty at approximately 12:00 p.m. On 1/09/2024, between 03:30 p.m. and 4:55 p.m., the LPA interviewed the Office Manager, staff, two residents, and obtained copies of resident records and other pertinent documents relevant to the investigation. During today's inspection, between 10:15 a.m. and 5:30 p.m., the LPA conducted a file review, interviewed the Office Manager, staff, residents, conducted a tour of the physical plant and obtained copies of resident records and other pertinent documents relevant to the investigation. Licensing Program Analyst (LPA) Ashley Smith arrived unannounced to issue the findings for the above allegations. The LPA met with staff Endy Guadarrama and explained the reason for the visit. On 11/10/2022, the Department received a complaint, which alleged that staff failed to seek timely medical attention for Resident #1 (R1) after R1 suffered a fall. Community Care Licensing Division’s Investigation’s Branch (IB) Investigator Heidy Bendana was assigned to the case. On 11/15/2022, the LPA conducted the initial visit, and the LPA toured the facility, reviewed video footage at 11:55 a.m., and interviewed staff at 9:35 a.m., 9:50 a.m., 11:11 a.m., and 11:40 a.m. Investigator Bendana obtained facility video footage on 12/08/2022, interviewed three (3) staff members on 12/08/2022, interviewed R1’s responsible party on 1/12/2023 and 1/24/2023; interviewed a hospice nurse on 1/12/2023; and, interviewed two (2) staff members on 1/12/2023. Licensing Program Analyst (LPA) Olson conducted a subsequent complaint visit to deliver findings for the above allegation. LPA met with Joy Ward-Michayluk, Administrstor. Concerns were that the facility did not inform resident #1 (R1)’s responsible party regarding change in resident’s physical therapy to Innerjoy Home Health. On 7/14/21 starting at 10:53 am interviews were conducted with staff, Administrator and residents. On 7/29/22 starting at 10:55 am interviews were conducted with resident family members. On 8/10/21 starting at 1:59 pm interviews were conducted with staff from Los Robles Home Care Services and staff from Doctor Steven Barr’s office. On 8/12/21 starting at 9:40 am interview was conducted with Licensee/Representative. Interview with Administrator revealed that R1’s responsible person did not want R1 to see any other physician besides Doctor Barr. Administrator stated that Doctor Barr was still R1’s physician.
